Severe neonatal thrombocytopenia due to anti-HPA-4b alloantibodies: Third report described in a Caucasian mother
CONCLUSION: We described the third case of anti-HPA-4b alloantibody discovered in a Caucasian mother. This case strengthens the need for reference laboratory to genotype a panel of HPA alleles reflecting local genetic population diversity and for crossmatch of maternal serum with fresh paternal platelets in clinical suspected cases of neonatal alloimmune thrombocytopenia.PMID:38693097 | DOI:10.1111/trf.17863 (Source: Transfusion)

Optimization of tracheoesophageal fistula model established with T-shaped magnet system based on magnetic compression technique
CONCLUSION: Use of the modified T-shaped magnet scheme is safe and feasible for establishing TEF and can achieve a more stable and uniform fistula size compared with ordinary magnets. Most importantly, this model offers better controllability, which improves the flexibility of follow-up studies.PMID:38690021 | PMC:PMC11056911 | DOI:10.3748/wjg.v30.i16.2272 (Source: World Journal of Gastroenterology : WJG)

Treatment of peri-implantitis with a flapless surgical access combined with implant surface decontamination and adjunctive systemic antibiotics: A retrospective case series study
CONCLUSIONS: A flapless surgical approach led to disease resolution in 54.4% of the implants with peri-implantitis. Several risk/protective predictors for disease resolution were identified.PMID:38690660 | DOI:10.1111/jcpe.13993 (Source: Journal of Clinical Periodontology)

Aspirin interruption before neurosurgical interventions: A controversial problem
World J Cardiol. 2024 Apr 26;16(4):191-198. doi: 10.4330/wjc.v16.i4.191.ABSTRACTAspirin is widely used for primary or secondary prevention of ischemic events. At the same time, chronic aspirin consumption can affect blood clot formation during surgical intervention and increase intraoperative blood loss. This is especially important for high-risk surgery, including neurosurgery. Current European Society of Cardiology guidelines recommend aspirin interruption for at least 7 d before neurosurgical intervention, but this suggestion is not supported by clinical evidence. This narrative review presents evidence that challenges ...

Genetic polymorphisms and major bleeding risk during vitamin K antagonists treatment: The BLEEDS case ‐cohort
ConclusionsCYP4F2 polymorphism was associated with major bleeding, especially in combination withVKORC1 genetic variants. These variants could be considered to further personalize anticoagulant treatment. (Source: Pharmacotherapy: The Journal of Human Pharmacology and Drug Therapy)

Periodontal grading —estimation of responsiveness to therapy and progression of disease
ConclusionThe present data indicate that indirect evidence may lead to inaccuracies compared to direct evidence regarding the estimation of periodontal progression. However, indirect evidence seems to be more suitable in the estimation of responsiveness to therapy than direct evidence, helping to identify cases that are more likely to require additional therapies such as re-instrumentation or periodontal surgery.Clinical relevanceRegarding the estimation of disease progression and responsiveness to periodontal therapy, accuracy and reliability of both direct and indirect evidence are limited when grading periodontitis. Hemophilic pseudotumor of the knee joint: Emphasizing prevention and early diagnosis in a rare disease
Key Clinical MessageHemophilic pseudotumors are rare complications occurring in individuals with severe hemophilia, characterized by progressive cystic swellings in muscles and/or bones due to recurrent bleeding. Timely initiation of factor VIII replacement is crucial.AbstractHemophilic pseudotumors are rare complications occurring in individuals with severe hemophilia, characterized by progressive cystic swellings in muscles and/or bones due to recurrent bleeding. Although their incidence has decreased with the advent of factor VIII replacement therapy, they still create challenges, particularly in regions with limited ac...

Perineal Squamous Cell Carcinoma Arising in an Epidermal Cyst
Acta Med Okayama. 2024 Apr;78(2):197-200. doi: 10.18926/AMO/66930.ABSTRACTA 90-year-old Japanese woman who had been aware of a subcutaneous mass on the right perineal region for 5 years was referred to our hospital for further examination and treatment because of the rapid growth of the mass and bleeding that began 3 months earlier. A biopsy of the mass revealed a diagnosis of well-differentiated squamous cell carcinoma. On preoperative examination, the tumor was 90×40 mm in size and was suspected to have partially invaded the levator ani muscle and external sphincter. Since a preoperative cardiac evaluation indicated sev...

Management of Deep Vein Thrombosis: An Update Based on the Revised AWMF S2k Guideline
Hamostaseologie. 2024 Apr;44(2):97-110. doi: 10.1055/a-2178-6574. Epub 2024 Apr 30.ABSTRACTDeep vein thrombosis (DVT) and pulmonary embolism (PE) are the most common manifestations of venous thromboembolism (VTE). Most DVTs affect the lower-extremity veins. Since the symptoms of DVT are non-specific, a prompt and standardised diagnostic work-up is essential to minimise the risk of PE in the acute phase and to prevent thrombosis progression, post-thrombotic syndrome and VTE recurrence in the long-term. Only recently, the AWMF S2k guidelines on Diagnostics and Therapy of Venous Thrombosis and Pulmonary Embolism have been rev...
