A Simple Bleeding Risk Score for the Complex Elderly Patient
Coronary artery disease (CAD) is a leading global cause of death worldwide, and in an aging society, the burden of percutaneous coronary intervention (PCI) in the elderly population is increasing.(1) An important feature that distinguishes this cohort is the significant increase in the likelihood of experiencing bleeding and ischemic events.(2) Given the comparable prognostic significance of both bleeding and ischemic events(3), the selection of the appropriate intensity and duration of antiplatelet therapy assumes a crucial role from a clinical perspective. Viral entry and translation in brain endothelia provoke influenza-associated encephalopathy
AbstractInfluenza-associated encephalopathy (IAE) is extremely acute in onset, with high lethality and morbidity within a few days, while the direct pathogenesis by influenza virus in this acute phase in the brain is largely unknown. Here we show that influenza virus enters into the cerebral endothelium and thereby induces IAE. Three-weeks-old young mice were inoculated with influenza A virus (IAV). Physical and neurological scores were recorded and temporal-spatial analyses of histopathology and viral studies were performed up to 72  h post inoculation. Histopathological examinations were also performed using IAE human a...

Clinical care pathway and management of major bleeding associated with non-vitamin k antagonist oral anticoagulants: a modified delphi consensus from saudi arabia and uae
Conclusion: The present Delphi consensus provides evidence-based recommendations and protocols for the management of NOAC-associated bleeding in the region. Patients with major NOAC-induced bleeding should be referred to a well-equipped ED with standardized management protocols. A multidisciplinary approach is recommended for establishing the association between NOAC use and major bleeding. Treating physicians should have prompt access to specific reversal agents to optimize patient outcomes. Real-world evidence and national guidelines are needed to aid all stakeholders involved in NOAC-induced bleeding management. (Source...

Oestrogens in oral contraception: considerations for tailoring prescription to women's needs
CONCLUSION: Today various COCs contain different oestrogens. Prescribers must be familiar with the different properties of each oestrogen for a tailored contraceptive recommendation, considering their safety and contraceptive efficacy, as well as women's needs and preferences.PMID:38683756 | DOI:10.1080/13625187.2024.2334350 (Source: The European Journal of Contraception and Reproductive Health Care)

Management of Injuries to the Parenchymal Abdominal Organs
Zentralbl Chir. 2024 Apr 29. doi: 10.1055/a-2301-7951. Online ahead of print.ABSTRACTThe most common organs affected by abdominal trauma are the spleen and the liver, often in combination. Pancreatic injuries are rare. In the case of blunt abdominal trauma, which is much more common, a clinical and laboratory examination as well as sonography should be performed. In the initial assessment, the circulatory situation must be screened. If there is haemodynamic instability and presentation of free fluid, an emergency laparotomy is indicated. If the situation is stable or stabilised and a pathological sonography is present, it ...

Postoperative Bowel Obstruction as a Rare Complication of an Abdominal Drain
Ann Ital Chir. 2024;95(2):132-135. doi: 10.62713/aic.3294.ABSTRACTAlthough routine intra-abdominal drain insertion following surgery represents a common practice worldwide, its utility has been questioned during the last decades. Several comparative studies have failed to document significant benefits from routine draining, and drain insertion has been correlated with various complications as well. Drain-related complications include, but are not limited, to infection, bleeding, and tissue erosion. Herein, we present the case of a 32-year-old patient with perforated peptic ulcer and purulent peritonitis, whose postoperativ...

Prevalence of High-Risk HPV Detection and HPV Vaccination in Cervical Cancer Screening During the HPV Vaccination Era at Siriraj Hospital - Thailand's Largest National Tertiary Referral Center
CONCLUSION: Among the 216 women who underwent cervical cancer screening in this study, there was a 9.3% prevalence of HR-HPV infection, and a 6.9% prevalence of HPV vaccination. Among the 15 vaccinated women, 2 tested positive for HPV16 (1 normal cytology, 1 abnormal cytology).PMID:38679983 | DOI:10.31557/APJCP.2024.25.4.1241 (Source: Asian Pacific Journal of Cancer Prevention)

Improving platelet function following prophylactic platelet transfusion in patients with hematological malignancies
CONCLUSION: Prophylactic transfusion improved platelet function. Platelet function significantly improved in patients with a CCI >4500, those with the same blood types as that of apheresis platelets, or those with platelet-derived microparticle levels <4.7%. No significant improvement in platelet function was noted after the transfusion of different blood types with acceptable compatibility or the transfusion of incompatible blood types. Our results suggest that transfusing platelets with the same blood type remains the optimal choice.PMID:38682289 | DOI:10.1111/ijlh.14283 (Source: International Journal of Laboratory Hematology)

Advancements in hemostatic strategies for managing upper gastrointestinal bleeding: A comprehensive review
World J Gastroenterol. 2024 Apr 21;30(15):2087-2090. doi: 10.3748/wjg.v30.i15.2087.ABSTRACTUpper gastrointestinal (GI) hemorrhage presents a substantial clinical challenge. Initial management typically involves resuscitation and endoscopy within 24 h, although the benefit of very early endoscopy (< 12 h) for high-risk patients is debated. Treatment goals include stopping acute bleeding, preventing rebleeding, and using a multimodal approach encompassing endoscopic, pharmacological, angiographic, and surgical methods. Pharmacological agents such as vasopressin, prostaglandins, and proton pump inhibitors are effective, bu...
