Arthroplasty of the Knee: Current Techniques for Implant Alignment
Z Orthop Unfall. 2021 Mar 1. doi: 10.1055/a-1304-3854. Online ahead of print.ABSTRACTWhen a total knee arthroplasty (TKA) is implanted using the traditional mechanical alignment technique, this typically results in a straight leg, independently of pre-operative or even pre-arthrotic varus or valgus alignment. With mechanical alignment, we distinguish between 2 different alignment techniques: ligament balancing and bony referencing according to bony skeletal landmarks. In ligament balanced technique beside the straight mechanical axis, the prosthesis is implanted at 90° to the latter. The rotational alignment of the fem...

Outcomes in Unicameral Bone Cyst Management Utilizing a Decision Tree Algorithm Approach.
CONCLUSIONS: Although we reject our hypothesis that a decision tree algorithm for the management of UBCs in the pediatric population could improve the healing rate, we believe that we uncovered some utility in applying an algorithm to this pathology. Our algorithm was designed to minimize risk to the child and maximize healing with the least number of surgical events. Treating surgeons should consider this proposed pathway to determine the best treatment and to help families understand that these lesions rarely heal with a single-event surgery. Analysis of Preoperative and Operative Factors Influencing Postoperative Shoulder Imbalance in Lenke Type 1 Adolescent Idiopathic Scoliosis.
Authors: Berlin C, Quante M, Freifrau von Richthofen E, Halm H Abstract BACKGROUND: Postoperative shoulder imbalance is a common complication in adolescent idiopathic scoliosis and may impair patient satisfaction. The little data in the literature on the prediction and treatment of postoperative shoulder imbalance is incongruous. According to some studies, postoperative shoulder imbalance depends on the superior instrumented vertebral bodies, skeletal maturity, extent of correction and flexibility of the major curve. QUESTION: Can preoperative radiological parameters from X-ray and traction films as well as cor...

Management of Modular Mega-Implant Infection of the Lower Extremity.
CONCLUSION: There is still no gold standard therapeutic regimen for the management of periprosthetic infection of mega-implants, though radical surgical debridement and lavage accompanied by systemic antibiotic therapy are the most important therapeutic tools in all cases of periprosthetic infections, regardless of the time of onset. Further studies are needed to standardize management strategies of such infections. Nevertheless, it is not uncommon for compromises to be made based on the particular condition of the individual. Latest Trends in the Current Treatment of Proximal Humeral Fractures - an Analysis of 1162 Cases at a Level-1 Trauma Centre with a Special Focus on Shoulder Surgery.
CONCLUSION: There is an increase in both the complexity of fractures and the number of surgically treated fractures between 2009 and 2012. Furthermore, an increase in osteoporosis numbers can be observed. New implants (PEEK, fenestrated screws for cement augmentation) and new surgical techniques (double plating osteosynthesis) were used as a result of increasing fracture complexity. Moreover, reverse total shoulder arthroplasty was used more commonly. PMID: 33530111 [PubMed - as supplied by publisher] (Source: Zeitschrift fur Orthopadie und Unfallchirurgie)

Spinal Tuberculosis.
Authors: Heyde CE, Lübbert C, Wendt S, Rodloff A, Völker A, von NHH Abstract Tuberculosis is one of the most common infectious diseases worldwide. The frequency in Germany is low, however, an increase has been observed in the past few years. The incidence of extrapulmonary manifestation accounts for up to 10 to 20%. In 50% of these cases the spinal column is affected. Although literature reveals worldwide experiences in the treatment, in Germany spinal tuberculosis remains a rarity. Different pitfalls and specific characteristics regarding diagnosis and therapy are to consider. Therefore, a presentation o...

Correction of Torsional Deformity Following Nail Osteosynthesis of a Subtrochanteric Fracture.
Authors: Ahrend MD, Herbst M, Ihle C, Schröter S Abstract Torsional deformity is a frequent complication following nail osteosynthesis of a subtrochanteric fracture. This complication is difficult to assess intraoperatively, but can cause major functional restrictions. The authors consider that a clinical assessment should be complemented by a postoperative radiological control standardised with torsion CT. Thus, torsion deformity can be recognised immediately and easily corrected if necessary. This can help to avoid long-term damage that requires considerable surgery. The video shows details of torsion correc...

Late Implant Failure in Cervical Disc Arthroplasty (M6-C, Spinal Kinetics) Causing Radiculopathy and Myelopathy.
Authors: Pingel A, Hoffmann CH, Scholz M, Kandziora F Abstract Cervical disc arthroplasty (CDA) is an approved surgical treatment option in selected patients with cervical spinal disc degeneration. Even though CDA is a standard procedure since 20 years, there is a lack of information about long term performance. The published reoperation rates after CDA are low and comparable to anterior cervical fusion. The authors describe a severe failure and dysfunction with a partial core dislocation of a cervical prosthesis into the spinal canal (M6-C, Spinal Kinetics, Sunnyvale, CA, USA). Six years after implantation of a cT...

Fractures of the Humeral Ulnar Condyle in Children - a Rare but Complicated Injury.
CONCLUSION: Very good clinical and functional results can be expected if the injury is diagnosed without delay followed by adequate therapy. Misdiagnosis of ulnar condyle fractures can be associated with the development of nonunions and functional restrictions as well as, after operative therapy, trochlear necrosis. Children up to the age of 6 in particular are at risk of misdiagnosis due to faulty assessment of the cartilaginous trochlea. Authors: Roessler PP, Mueller-Rath R, Wirtz DC, Schildberg FA Abstract Cartilage regeneration with cell-free matrices has developed from matrix-associated autologous cartilage cell transplantation (MACT) over ten years ago. Adjustments to the legal framework and higher hurdles for cell therapy have led to the procedures being established as an independent alternative to MACT. These procedures, which can be classified as matrix-induced autologous cartilage regeneration (MACR), all rely on the chemotactic stimulus of a cross-linked matrix, which mostly consists of collagens. Given the example of a commercially availa...
