The "Lazio Network" experience. The first Italian regional research group on the Enhanced Recovery After Surgery (ERAS) program. A collective database with 1200 patients in 2016-2017.
CONCLUSIONS: The adoption of the ERAS program is rapidly increasing even in central Italian regions, even though the total rate of minimally invasive surgery procedures still low. Benefits in terms of clinical outcomes will be evaluated from the analysis of a multi-center database of patients treated between January 2016 and December 2017, including more than 1200 patients. Efficacy of fibrin sealant in thyroid surgery. Is drainage still necessary?
CONCLUSIONS: Our study demonstrates that there is no evidence that the use of suction drains improves patients outcome and that routinely use of fibrin sealant can be advocated in thyroid surgery as an adjunct to a good surgical procedure. Sex reassignement with or without surgery. New ethical and juridical pathways.
CONCLUSIONS: The authors point out that critical issues and obstacles to the full implementation of the right to gender identity remain, in that this right is still subject to the authorization of a judge. This approach does not seem to be in line with the recent World Health Organization (WHO) decision to remove the "gender incongruity" from the list of mental and behavioural disorders (as it is in the current International Classification of Diseases-10, so far), to decrease the stigma surrounding such a condition. Transanal hemorrhoidal dearterialization versus Milligan-Morgan hemorrhoidectomy in grade III/IV hemorrhoids.
CONCLUSIONS: For grade III hemorrhoids THD technique provides the same results of MM, while for grade IV hemorrhoids we believe that better result can be achieved with MM technique. However, we deem that in cases of grade IV hemorrhoids the choice between THD and MM can be more rationally made on the basis of objective examination with the patient in the operating position and already anesthetized and therefore in complete relaxation. Intragastric enucleation of stromal tumors through a combined laparoscopic and endoscopic approach.
Authors: Vecchio R, Catalano R, Distefano RE, Intagliata E Abstract Dear Editor, We read with great interest the paper entitled "Laparoscopic organ-preserving gastric resection improves the quality of life in stromal tumor patients: an observational study with 23 patients" written by Ozcan et al. in Ann Ital Chir. 2018 Oct 23;7 1. We agree with the Authors that preserving the stomach after excision of stromal tumors is essential to improve the patients' quality of life. We would like, however, to discuss some issues that the paper published by Ozcan et al. might raise. When feasible, laparoscopic surgery ...

Hyperthyroidism with thyroid cancer: more common than expected?
DISCUSSION: Prevalence of cancer on a background of hyperthyroidism has gradually increased in recent years. It is not realistic to determine thyroid cancer prevalence only based on autopsy examinations. This rate significantly increases in the presence of nodule in clinical studies. Most of the carcinomas are microcarcinomas that do not have lymphovascular invasion and have very low lymph node metastasis. Thus, their treatment is still under debate. Endoscopy-assisted, single trans-gastric trocar, laparoscopic pseudocysto-gastrostomy A minimally invasive alternative for drainage of large pseudocysts.
CONCLUSIONS: Internal drainage of pancreatic pseudocyst inside the stomach, by the technique described above, is facile, giving the patient the advantages of endoscopy and minimally invasive surgery. In these circumstances, we consider it superior to endoscopic drainage, by providing a broad communication between the pseudocyst and stomach cavity, reducing the risk of relapse and abscess formation. Moreover, it offers the possibility of exploring the pseudocyst cavity and of removing any necrotic tissues. The endoscopic assistance makes possible the single trans-gastric trocar approach, limiting the injury of the gastric w...

Non-anastomotic aneurysmal degeneration of great saphenous vein graft A case report and review of the literature.
CONCLUSIONS: Aneurysmal degeneration of deep lower extremity vein conduits implanted for vascular reconstruction has been rarely reported, but when detected they can lead to graft thrombosis, distal embolization, acute rupture, or skin ulceration. Therefore, Doppler ultrasound guided surveillance of GSV grafts should be mandatory and long time from vein graft creation to onset of aneurysms makes long-term graft surveillance even more imperative.
