Primary malignant melanoma of the bowel diagnosed following a bowel intussusception in an adult patient. A case report and review of the literatureport
CONCLUSION: The diagnosis and the treatment of primary intestinal melanoma is a challenging due to the lack of scientific indications. Our case shows how an early diagnosis, although accidental, can offer a good survival free from disease. Moreover, a careful follow-up of our patients allows us to promptly identify neoplasm recurrence or distant metastasis that can be treated with surgery and systematic therapy.KEY WORDS: Intussusception, Primary bowel melanoma.PMID:33650989 (Source: Annali Italiani di Chirurgia)

One-step versus two-step procedure for management procedures for management of concurrent gallbladder and common bile duct stones. Outcomes and cost analysis
CONCLUSIONS: The data of the study confirm the superiority of the Rendez-vous technique because it resolves cholelithiasis associated with choledocholithiasis in a single surgical act, with greater acceptance of the patient who avoids a second invasive surgical act, and with a reduction in complications; moreover, it requires shorter hospitalization, resulting in reduced costs. We propose this option in the management of cases where preoperative ERCP-ES has failed.KEY WORDS: Common bile duct stones, Cholecysto-choledocholithiasis, Endoscopic retrograde cholangiopancreatography, Endoscopic sphincterotomy, Laparoscopic chole...

Laparoscopic versus open complete meso-colic excision for right-sided colon cancer. Analysis of short-term outcomes
CONCLUSIONS: Pathological (specimen lengths, resection margin lengths, number of lymph nodes, and R0 resection) and short-term outcomes of the laparoscopic CME group were comparable. Moreover, laparoscopic CME conferred short-term benefits in terms of shorter incision lengths, less operative blood loss, reduced time to mobilization, early regain of bowel motion, shorter time to soft diet, and reduced length of hospital stay. Based on these results, laparoscopic CME can be considered as a routine elective approach for right-sided colon cancer.KEY WORDS: Colon cancer, Complete mesocolic excision, Right hemicolectomy.PMID:336...

CONCLUSION: Robotic technology is feasible and associated with a shorted recovery and better cosmetic results. The endowrist movement of the robotic arms allows wide instrument articulation in a confined space, bypassing the limitis of laparoscopic instruments. In this way, it is possible to realize an accurate dissection of important elements, to reduce operative timing and intraoperative and postoperative complications.KEY WORDS: Ehlers-Danlos syndrome, Inguinal hernia, TAPP.PMID:33617482 (Source: Annali Italiani di Chirurgia)

Sister Mary Joseph's nodule: the tip of an iceberg.
We present here a rare case of a 67-year-old woman with a Sister Mary Joseph's nodule. On surgical exploration, a left ovarian mass with anterior abdominal wall metastasis, ascites and extensive intra-abdominal metastatic lesions were observed. Our case report shows the importance of a careful physical examination as an invaluable diagnostic tool in modern medicine. High index of suspicion and awareness of this clinical sign may lead to the detection of the primary source, to its diagnosis and more appropriate treatment options in order to achieve the best survival possibility. Diagnosis of micropapillary carcinoma of the breast by endometrial biopsy in a postmenopausal patient with abnormal uterine bleeding.
Authors: Colak E, Erinanc OH, Ozdemir D Abstract Metastasis of breast cancer to the uterus is quite rare. The majority of reported cases have evolved from previously known invasive ductal and lobular carcinomas of the breast. Herein, a rare case of invasive micropapillary carcinoma of the breast that was diagnosed via endometrial biopsy, without a previous diagnosis or history of breast cancer, was presented. A 58-year-old postmenopausal patient, who had no history of cancer, presented with complaints of abnormal uterine bleeding (AUB) that had begun 2 weeks prior. After undergoing transvaginal ultrasonography, end...

Is early anvil placement an alternative technique to reduce anastomotic leak after rectosigmoid cancer resection?
DISCUSSION: Our study shows that the anvil must be placed at the beginning of the dissection to evaluate the anomalies that cause anastomotic leaks. We think that this method increases the reliability of the anastomosis line. Thus, the hospitalization period of the patients was shortened and they returned to their active lives faster. In addition, patients used less antibiotics and they needed less medical treatment. KEY WORDS: Anastomotic leaks, Anvil, Rectosigmoid cancer placement, Stapler colorectal. Clinicopathological significance of insulin-like growth factor-1 receptor expression in breast cancer.
Authors: Şahin S, Cakir A, Gonul II, Seckin S, Uluoglu O Abstract OBJECTIVE: Insulin-like growth factor 1 receptor (IGF1R) is a receptor protein tyrosine kinase that is claimed to be related with tumor development and progression of breast cancer with some conflicting results in the literature. The aims of the study are to investigate expression of IGF1R, and correlate with clinicopathological parameters to clarify the significance of IGF1R on breast cancer. MATERIAL AND METHODS: IGF1R and Ki67 were applied immunohistochemically to the tissue microarray sections of 370 female breast cancer patients. The result...

Treatment of completely obstructed anastomosis after low rectal resection A trans-anal plus endoscopic trans-colostomy rendez-vous approach and a review of the literature.
CONCLUSIONS: The treatment of COA after LRR can be very demanding, particularly after CAA. Few data are reported in literature to define the best approach to treat these conditions. Our described rendez-vous technique can represent a valid choice, especially after CAA. KEY WORDS: Colorectal anastomosis, Endoscopic treatment, Low-rectal resection. Perineal scar endometriosis involving the anal sphincter. A case report and review of the literature.
CONCLUSION: Perineal endometriosis is a rare disease and may involve the anal sphincter muscles. Incomplete excision to protect the sphincters is associated with high recurrence, while extensive excision can cause anal sphincter damage that may cause anal incontinence. Endoanal ultrasonography may be necessary in surgical planning. Primary sphincteroplasty with excision may be necessary in cases of perineal endometriosis with external anal sphincter muscle involvement. Acute pancreatitis secondary to non-functioning pancreatic neuroendocrine tumor: uncommon clinical presentation. Clinical case and review of literature.
CONCLUSION: Although rarely, nf-PNETS may be the cause of severe non-biliary acute pancreatitis from pancreatic ductal system compression. In cases where PET/CT68Ga cannot be performed, total-body scintigraphy with Octreoscan® remains the most widely used method for the diagnosis of PNETs and the identification of extra-pancreatic lesions. Chromogranin and Synaptophysin are confirmed as specific markers of neuroendocrine differentiation. Gastric perforation in blunt abdominal trauma. Report of two cases and review of literature.
Authors: De Martino C, Della Corte M, Smaldone P, Pollio A, Armellino MF Abstract Gastric perforations as a result of blunt abdominal traumas are rare, with a reported incidence of less than 2%. Usually associated with other solid visceral injuries, isolated gastric ruptures following a blunt abdominal injury are extremely uncommon. The severity of injury, timing of presentation, time elapsed since the last meal, as well as the presence of concomitant injuries are important prognostic factors. Contrast-enhanced CT scan is the gold-standard diagnostic tool in haemodinamically stable patients and allows to detect or ...
