Hospitalization costs associated with diabetic foot ulcers treated by a multidisciplinary team in clinical practice. A retrospective study from southern Turkey.
CONCLUSION: The study revealed that the cost of DFU could show variability in relation to countries' level of development. We highlighted similar studies in other countries; the major factors of total expenses were length of hospitalisation, medication prescription and use of surgery. A rare case of jejunal gastrointestinal stromal tumor.
We present here a case of jejunal GIST which is the rarest subtype. A 54-year-old man presented with lower right side abdominal pain. On workup, images showed a 7cm solid-cycstic lesion adjacent to ascending colon. On surgical exploration, a large jejunal tumor en bloc resected and jejuno-jejunal primary anastomosis was performed. Pathologic results showed a 9cm jejunal GIST with 5% proliferation index. Immunohistochemistry results demonstrated high expressions of CD117, whereas CD34 negative. The patient was discharged uneventfully. GISTs should be considered in patients with abdominal pain. The mainstay treatment of the ...

Update on the sentinel lymph node biopsy after neoadjuvant chemotherapy in breast cancer patient.
Authors: Franceschini G, Di Leone A, Sanchez AM, D'Archi S, Scardina L, Masetti R Abstract Today, sentinel lymph node biopsy (SLNB) is considered the gold standard for axillary staging in early breast cancer patients with clinically negative lymph nodes (cN0). SLNB allows to determine the axillary lymph node status sparing the axillary dissection (AD) and its potential complications (seroma formation, loss of sensation, shoulder dysfunction and lymphedema) On the other hand, SLNB for nodal staging in breast cancer patients with clinically negative lymph nodes after neoadjuvant chemotherapy (ycN0) is a highly debate...

The cutaneous antisepsis of the operative field with chlorhexidine gluconate, the improvement of the surgical technique, the protection of the cutaneous margins of incision, the use of new devices, the accurate hemostasis and the reduction of surgery time lead to a lack of SSIs and systemic infection complications in all patients undergoing thyroid surgery in NO-AP. A prospective comparative study of ultrasonography, contrast-enhanced MRI and 18F-FDG PET/CT for preoperative detection of axillary lymph node metastasis in breast cancer patients.
CONCLUSIONS: There is no single absolute modality for de tecting ALNMs in breast cancers to replace sentinel lymph node biopsy or axillary lymph node dissection. If ALNM is suspected based on PET/CT, axillary lymph node dissection without sentinel lymph node biopsy might be a better option because it is related to high possibilities of ALNM. A prospective comparative study of contrast-enhanced CT, contrast-enhanced MRI and 18F-FDG PET/CT in the preoperative staging of colorectal cancer patients.
CONCLUSION: PET/CT is a useful tool in the evaluation of colorectal cancer, and it allows to metabolically characterize undetermined lesions suspected for recurrence of disease, to perform a complete pre-surgical staging and to identify occult metastatic disease. An unusual case of large endometrioma within the rectus abdominis muscle misdiagnosed as desmoid tumour.
We describe an unusual case of giant intramuscular abdominal endometrioma clinically misdiagnosed as desmoid tumour in a 36-year-old female patient with a one-year history of lower abdominal pain. Endometriosis is defined by the presence of endometrial tissue outside the uterine cavity, associated with fibrosis and inflammatory reaction. Although the abdominal wall is one of the most frequent sites of extra pelvic endometriosis, the localization in the anterior rectus abdominis muscle is unusual and associated with previous cesarean section. In most cases, the preoperative diagnosis is erroneous because the different imagi...

Hyperthyroidism with thyroid cancer: more common than expected?
DISCUSSION: Prevalence of cancer on a background of hyperthyroidism has gradually increased in recent years. It is not realistic to determine thyroid cancer prevalence only based on autopsy examinations. This rate significantly increases in the presence of nodule in clinical studies. Most of the carcinomas are microcarcinomas that do not have lymphovascular invasion and have very low lymph node metastasis. Thus, their treatment is still under debate. Endoscopy-assisted, single trans-gastric trocar, laparoscopic pseudocysto-gastrostomy A minimally invasive alternative for drainage of large pseudocysts.
CONCLUSIONS: Internal drainage of pancreatic pseudocyst inside the stomach, by the technique described above, is facile, giving the patient the advantages of endoscopy and minimally invasive surgery. In these circumstances, we consider it superior to endoscopic drainage, by providing a broad communication between the pseudocyst and stomach cavity, reducing the risk of relapse and abscess formation. Moreover, it offers the possibility of exploring the pseudocyst cavity and of removing any necrotic tissues. The endoscopic assistance makes possible the single trans-gastric trocar approach, limiting the injury of the gastric w...
