Asymptomatic CMV infection at birth following maternal infection, valacyclovir treatment, and a subsequent negative amniocentesis. Case report
Recent evidence suggested that valacyclovir is currently the only pharmacological intervention able to reduce the risk of vertical CMV congenital infection in case of primary infection during pregnancy. So far, no data are available on the prognosis of children with congenital CMV infection diagnosed at birth after a negative amniocentesis whose mother were treated with valacyclovir during pregnancy, therefore it is essential to carry out a rigorous neurocognitive follow-up in these children in order to investigate the potential clinical consequence. Chronic recurrent urticaria in a patient with recurrent herpes labialis: complete remission after administration of aciclovir and antihistamines
We present a 41-year-old female patient with chronic recurrent urticaria associated with angioedema and bronchospasm, in whom her herpes genitalis was found to be concurrent within the history and clinical examination. According to the history, the herpes was also recurrent and dated back about a year. Chlamydia trachomatis infection was also found, with serological findings corresponding to vaginal discomfort. Treatment with acyclovir 400mg thrice daily for an initial period of 7 days in combination with desloratadine 5mg daily was started as we observed complete remission of the urticarial rash. Due to worsening vaginal ...

A case of viral meningitis due to Varicella Zoster virus infection in a young adult male
IDCases. 2023 Sep 28;34:e01899. doi: 10.1016/j.idcr.2023.e01899. eCollection 2023.ABSTRACTA 36-year-old male with headaches was empirically treated for herpes simplex virus meningitis; CSF PCR testing later confirmed varicella zoster virus meningitis. Valacyclovir was increased to 2 g QID for remaining duration of therapy with full recovery. This case highlights the importance of comprehensive testing and proper treatment adjustment for rarer etiologies of aseptic meningitis.PMID:37786647 | PMC:PMC10542008 | DOI:10.1016/j.idcr.2023.e01899 (Source: Herpes)

Herpetic esophagitis in healthy young adult
Clin J Gastroenterol. 2023 Sep 15. doi: 10.1007/s12328-023-01852-4. Online ahead of print.ABSTRACTHerpetic esophagitis (HE) is common in immunocompromised patients but rarely reported in healthy young adults. A 28-year-old healthy male patient visited our clinic with a chief complaint of chest pain. An esophagogastroduodenoscopy revealed widely spread superficial multiple ulcers in the middle to the distal esophagus, and repeated biopsies from the ulcer were performed but revealed inconsistent findings with HE. Thus, he was clinically diagnosed with HE based on endoscopic findings and serologic tests of immunoglobulin M (I...

Combined cytotoxic and immune-stimulatory gene therapy for primary adult high-grade glioma: a phase 1, first-in-human trial
This study aimed to evaluate the safety and activity of two adenoviral vectors expressing HSV1-TK (Ad-hCMV-TK) and Flt3L (Ad-hCMV-Flt3L) in patients with high-grade glioma.METHODS: In this dose-finding, first-in-human trial, treatment-naive adults aged 18-75 years with newly identified high-grade glioma that was evaluated per immunotherapy response assessment in neuro-oncology criteria, and a Karnofsky Performance Status score of 70 or more, underwent maximal safe resection followed by injections of adenoviral vectors expressing HSV1-TK and Flt3L into the tumour bed. The study was conducted at the University of Michigan Me...

A Phase 2 study of In Situ Oncolytic Virus Therapy and Stereotactic Body Radiation Therapy Followed by Pembrolizumab in Metastatic Non-Small Cell Lung Cancer
A phase 2 study of stereotactic body radiation therapy and in situoncolytic virus therapy in metastatic non-small cell lung cancer (mNSCLC) followed by Pembrolizumab (STOMP) was designed to explore the dual approach in enhancing single pembrolizumab with ADV/HSV-tk plus valacyclovir gene therapy and stereotactic radiotherapy (SBRT) in mNSCLC. Sensory and Motor Neurologic Presentations of Herpes Zoster (Shingles)
An elderly woman developed acute, severe, nontraumatic, and constant left knee pain. Three days later, she developed a herpetic rash along the L4 dermatome associated with allodynia. She was treated with valacyclovir, but pain continued with development of proximal leg weakness. Three weeks after onset, the rash was crusted (Figure  1), and neurologic examination demonstrated weakness in the L3/4 myotome with an absent patellar reflex (Supplemental Video, available online at http://www.mayoclinicproceedings.org). Disseminated primary herpes simplex infection imitating preterm prelabor rupture of membranes - a case report
CONCLUSION: Discharge from HSV cervicitis can present as nitrazine-positive pooling, imitating PPROM. A high index of suspicion is warranted, especially when the amniotic fluid volume is normal and arborization is not seen on microscopic exam.PMID:37503307 | PMC:PMC10368810 | DOI:10.1016/j.crwh.2023.e00528 (Source: Herpes)
