Facial Palsy in Post COVID-19 Invasive Fungal Sinusitis
This study aimed to study the various presentations of post COVID-19 invasive fungal sinusitis with 7th cranial nerve palsy and to correlate topodiagnostic tests and radiological findings to identify the site of lesion. A retrospective study was conducted at a tertiary care hospital where 11 patients with post COVID-19 invasive fungal sinusitis with facial palsy presented to our institute. Detailed history, clinical examination, radiological imaging were done for all patients. A total of 11 patients with post COVID-19 invasive fungal sinusitis with facial palsy were included in this study. The mean age of the patients was ...

Early-career general practitioners ’ antibiotic prescribing for acute infections: a systematic review
ConclusionsEarly-career GPs prescribe fewer antibiotics than later-career GPs. However, there are still significant improvements to be made for common acute conditions, as their prescribing is higher than recommended benchmarks. Addressing antimicrobial resistance requires an ongoing worldwide effort and early-career GPs should be the target for long-term change. (Source: Journal of Antimicrobial Chemotherapy)

Does acute isolated sphenoidal sinusitis meet the criteria of the recent acute sinusitis guidelines, EPOS2020?
ConclusionISS is an uncommon entity encountered in clinical practice with non-specific symptoms and a potential for complications. Therefore, the condition must be kept in mind by clinicians, and prompt diagnosis and treatment must be initiated. This kind of sinusitis does not fulfill the standard guidelines for acute sinusitis diagnosis criteria. (Source: European Archives of Oto-Rhino-Laryngology)

Amoxicillin-Clavulanate vs Amoxicillin for Pediatric Acute Sinusitis
In this study, use of amoxicillin-clavulanate was associated with more frequent gastrointestinal adverse events and yeast infections. We have some concerns about this study. (Source: JAMA)

Amoxicillin-Clavulanate vs Amoxicillin for Pediatric Acute Sinusitis —Reply
In Reply In their Letter regarding our recent study, Dr He and colleagues note that by excluding patients who received antibiotics in the prior 30 days, participants were at lower risk of an antibiotic-resistant infection, which may have masked a benefit of amoxicillin-clavulanate. We excluded patients with recent antibiotic exposure for the following reasons: (1) to ensure a new user design, which mitigates bias; and (2) to focus on the largest group of patients with acute sinusitis because most patients do not have recent antibiotic exposure (98% in our study). The comparative risk of treatment failure with amoxicillin-c...

The associations between different types of infections and circadian preference and shift work
In conclusion, neither circadian preference nor shift work seemed to be strongly associated with risk of infections, except for venereal disease (more common in evening types) and influenza-like illness (more common in night workers). Longitudinal studies are needed for causal inferences.PMID:38221779 | DOI:10.1080/07420528.2024.2303986 (Source: Chronobiology International)

Higher Risk of Rheumatoid Arthritis in Patients With Chronic Rhinosinusitis: Prospective Association in the U.K. Biobank and Genetic Evidence by Mendelian Randomization Analysis
CONCLUSION: CRS has a genetically causal effect on the incidence of RA, and the risk of RA is greatly higher in CRS at the individual level. This is the first study to reveal an association between CRS and RA. Due to the high incidence of CRS, it is recommended that additional attention should be paid to the increased RA risk in patients with CRS compared to that in common people.PMID:38225197 | DOI:10.1177/19458924231225488 (Source: American Journal of Rhinology and Allergy)

An enormous epidural abscess secondary to cranial metastasis of a lung tumor
Orv Hetil. 2024 Jan 14;165(2):69-73. doi: 10.1556/650.2024.32943. Print 2024 Jan 14.ABSTRACTEpiduralis abscessus a leggyakrabban otitis media, sinusitis és mastoiditis következtében alakul ki. Ezekben az esetekben a kórokozók a legtöbbször a vénás és artériás rendszeren keresztül jutnak az epiduralis térbe, míg nyílt törések vagy idegsebészeti műtétek során direkt módon is kerülhetnek baktériumok a koponyacsont és a dura közé. Esetismertetésünk irodalmi ritkaságnak tekinthető, mivel az enormis nagyságú epiduralis tályog kialakulásában szerepet játszott a tüdődaganat áttéte miatt ki...

Causal association of gastroesophageal reflux disease with chronic sinusitis and chronic disease of the tonsils and adenoids
ConclusionThere is a causal association between GERD and chronic disease of the tonsils and adenoids and chronic sinusitis, respectively, and the occurrence of GERD increases the risk of developing chronic disease of the tonsils and adenoids and chronic sinusitis. Mast Cells in Aspirin-Exacerbated Respiratory Disease
AbstractPurpose of ReviewAspirin-exacerbated respiratory disease (AERD) is a syndrome of high type 2 inflammation and is known to critically involve mast cell activation. The mast cell is an important cell in the baseline inflammatory processes in the upper and lower airway by maintaining and amplifying type 2 inflammation. But it also is prominent in the hypersensitivity reaction to COX-1 inhibition which defines this condition.Recent FindingsRecent work highlights the mast cell as a focal point in AERD pathogenesis. Using AERD as a specific model of both high type 2 asthma and chronic sinusitis, the role of mast cell act...

Reliability of large language models in managing odontogenic sinusitis clinical scenarios: a preliminary multidisciplinary evaluation
ConclusionsLLMs like ChatGPT, especially newer versions, showed potential for complimenting evidence-based clinical decision-making, but substantial disagreement was still demonstrated between LLMs and clinical specialists across most case examples, suggesting they are not yet optimal in aiding clinical management decisions. Future studies will be important to analyze LLMs ’ performance as they evolve over time. (Source: European Archives of Oto-Rhino-Laryngology)
