Amoxicillin-Clavulanate vs Amoxicillin for Pediatric Acute Sinusitis —Reply

In Reply In their Letter regarding our recent study, Dr He and colleagues note that by excluding patients who received antibiotics in the prior 30 days, participants were at lower risk of an antibiotic-resistant infection, which may have masked a benefit of amoxicillin-clavulanate. We excluded patients with recent antibiotic exposure for the following reasons: (1) to ensure a new user design, which mitigates bias; and (2) to focus on the largest group of patients with acute sinusitis because most patients do not have recent antibiotic exposure (98% in our study). The comparative risk of treatment failure with amoxicillin-clavulanate or amoxicillin among patients with recent antibiotic exposure could be an area for future research.
