Commentary on “Post-carotid Endarterectomy Hypertension. Part 2: Association with Peri-operative Clinical, Anaesthetic, and Transcranial Doppler Derived Parameters”
Guidance on peri-operative haemodynamics is crucial to prevent procedural stroke in carotid endarterectomy (CEA) by preserving cerebral perfusion.1,2 Newman et  al. suggest a one size fits all post-operative systolic blood pressure (BP) policy treating > 170 mmHg or > 160 mmHg in patients with symptoms.3 This policy causes significant overtreatment, as two in five CEA patients will undergo in hospital BP lowering treatment for several days, leading to a high workload, increased in hospital costs, and bed occupancy.

Thrombolytic therapy for pulmonary embolism.
CONCLUSIONS: Low-quality evidence suggests that thrombolytics reduce death following acute pulmonary embolism compared with heparin. The included studies used a variety of thrombolytic drugs. Thrombolytic therapy may be helpful in reducing the recurrence of pulmonary emboli but may cause major and minor haemorrhagic events and stroke. Effects of Normothermic Machine Perfusion Conditions on Mesenchymal Stromal Cells
In this study the effect of NMP perfusion fluid on survival, metabolism and function of thawed cryopreserved human (h)MSC and porcine (p)MSC in suspension conditions was studied. Suspension conditions reduced the viability of pMSC by 40% in both perfusion fluid and culture medium. Viability of hMSC was reduced by suspension conditions by 15% in perfusion fluid, whilst no differences were found in survival in culture medium. History of Hypertension Is Associated With MR Hypoperfusion in Chinese Inpatients With DWI-Negative TIA
Conclusion: Our results suggest that the prevalence of perfusion abnormality is high as assessed by RAPID using the parametric Tmax  4 s. History of hypertension is a strong predictor of focal perfusion abnormality as calculated by RAPID on Tmax map of TIA patients with negative DWI findings.

Effect of sildenafil on maternal hemodynamics in pregnancies complicated by severe early-onset fetal growth restriction: planned subgroup analysis from a multicenter randomized placebo-controlled double-blind trial.
CONCLUSIONS: Sildenafil in a dose of 25 mg three times daily increases HR, reduces BP and reduces arterial stiffness in pregnancies complicated by FGR. These changes are modest, consistent with the anticipated vasodilatory effect and their clinical impact on the mother and baby, in both the short- and long-term, remains uncertain. Impact of hypertension on cerebral microvascular structure in CPAP-treated obstructive sleep apnoea patients: a diffusion magnetic resonance imaging study
ConclusionBased on IVIM analysis, cerebral microvascular structure and function showed no difference between hypertensive and normotensive patients with moderate-to-severe OSA treated with CPAP. Postoperative hemorrhage during the acute phase after direct or combined revascularization for moyamoya disease: risk factors, prognosis, and literature review.
CONCLUSIONS: Preoperative hypertension, CTP stage > III, and posterior circulation involvement are independent risk factors for postoperative ICH after direct or combined revascularization for MMD. After appropriate perioperative management, postoperative ICH has no significant correlations with the postoperative short-term and long-term neurofunctional status. Potential applications of FDG-PET/MRI in detecting vascular dementia
Conclusions: FDG-PET/MRI may be a valuable tool in the diagnosis of VaD. However, the paucity of data in the field necessitates a further analysis of its similarity to FDG-PET/CT in results. Novel studies should be conducted to explore various scanners, patient populations, and MRI protocols to better inform the identification of VaD.
