Cerebral autoregulation is heterogeneous in different stroke mechanism of ischemic stroke caused by intracranial atherosclerotic stenosis
ConclusionIn IS patients, CA was impaired heterogeneously and was correlated with different risk factors in varied stroke mechanism. CA can be as an informative determinant of stroke risk in patients with ICAS and to help improving individualized treatment strategies in the presence of ischemic stroke caused by ICAS.

Intraoperative hypotension and perioperative acute ischemic stroke in patients having major elective non-cardiovascular non-neurological surgery
ConclusionOur analysis suggests that when MAP is less than 60  mmHg for more than 20 min, there is increased odds of acute ischemic stroke. Further studies are needed to determine what MAP should be targeted during surgery to optimize cerebral perfusion and limit ischemic stroke risk.

Characteristics of cerebral ischemic stroke based on moyamoya disease and atherosclerosis-associated intracranial arterial stenosis
ConclusionsDiffering from LAS stroke, MMD stroke mainly presents with WSI and does not feature with platelet hyper-aggregation and fragmentation of ulcer plaque. Whereby, focusing on perfusion improvement rather than antiplatelets and statins may be the predominant step in MMD-stroke correction.

Identification of embolic stroke in patients with large vessel occlusion: The Chinese embolic stroke score, CHESS
CONCLUSION: CHESS could reliably and independently identify embolic stroke as the cause of large vessel occlusion.PMID:34559949 | DOI:10.1111/cns.13729

Insights into a personalized management of blood pressure in acute stroke
Purpose of review Blood pressure management in acute stroke has long been a matter of debate. Epidemiological data show that high blood pressure is associated with death, disability and early stroke recurrence, whereas the pathophysiological rationale for ensuring elevated blood pressure in order maintain adequate cerebral perfusion remains a pertinent argument, especially in ischaemic stroke. Recent findings The European Stroke Organisation Guidelines on blood pressure management in acute ischaemic stroke and intracerebral haemorrhage provide recommendations for the appropriate management of blood pressure in v...

Hematocrit and Stroke: A Forgotten and Neglected Link?
Semin Thromb Hemost DOI: 10.1055/s-0037-1602663Stroke is considered among the most common causes of mortality and disability, leading to dramatic socioeconomic consequences. From a pathophysiologic perspective, enhanced blood viscosity due to increased hematocrit might be associated with stroke through impaired cerebral blood perfusion. This association has remained rather neglected during previous decades, but newly emerged as an epicenter of scientific interest due to the unexpected elevation of stroke rates with sodium-glucose cotransporter-2 inhibitors, a new class of hypoglycemic drugs with otherwise dramatic cardiova...

Combined Arterial Hypertension and Ischemic Stroke Exaggerate Anesthesia-Related Hypotension and Cerebral Oxygenation Deficits: A Preclinical Study
CONCLUSIONS: Our findings are the first demonstration that ischemic stroke per se increases the severity of IOH, particularly when combined with a prior history of hypertension; this combination appears to compromise penumbral perfusion.PMID:36730724 | DOI:10.1213/ANE.0000000000006263

Areas of Cerebral Ischemia Have Increased Propensity to Convert to Infarct among Diabetics (P7.155)
CONCLUSIONS: PCT evaluation suggests that a lower CBV could explain worse outcomes among diabetics with stroke. Microvascular disease and endothelial dysfunction should be investigated as potential mechanisms for lower CBV and worse outcomes among diabeticsDisclosure: Dr. Mohamed has nothing to disclose. Endovascular Therapy and Ethnic Disparities in Stroke Outcomes
Conclusions: Despite unique baseline characteristics, African-Americans treated with ET for large vessel occlusion strokes have similar outcomes as Caucasians. Greater availability of ET may diminish the ethnic/racial disparities in stroke outcomes.Intervent Neurol 2018;7:389 –398
