The Effect of Cerebellar rTMS on Modulating Motor Dysfunction in Neurological Disorders: a Systematic Review
Cerebellum. 2022 Aug 26. doi: 10.1007/s12311-022-01465-6. Online ahead of print.ABSTRACTThe effectiveness of cerebellar repetitive transcranial magnetic stimulation (rTMS) on motor dysfunction in patients with neurological disorders has received increasing attention because of its potential for neuromodulation. However, studies on the neuromodulatory effects, parameters, and safety of rTMS implementation in the cerebellum to alleviate motor dysfunction are limited. This systematic review aimed to evaluate the effectiveness and safety of cerebellar rTMS treatment for motor dysfunction caused by neurological disorders and to...

Case report: Acute vestibular syndrome and cerebellitis in anti-Yo paraneoplastic syndrome
We report three patients evaluated over the past decade with an acute AVS along with subtle downbeat nystagmus (DBN), followed by dysarthria and progressive truncal and limb ataxia, as well as increasing DBN intensity.MethodsAll patients underwent neurologic examination, video-oculography, MRI, serum cancer markers, spinal fluid examination, paraneoplastic panel testing, and oncologic workup. With a consolidated diagnosis of cancer/paraneoplastic syndrome, we treated with plasma exchange (PLEX), high-dose steroids, surgery, and oncologic investigation. We additionally provided oncotherapy in one out of three patients.Resul...

Posterior circulation stroke due to intracranial artery disease in the Chinese population
ConclusionsPCS due to INVAD was more common in the Chinese population. However, PCS due to IVAD had more serious clinical-radiologic patterns and worse outcomes.

Atypical clinical presentations are common in TIA and minor stroke patients with DWI-MRI confirmed ischaemia
Conclusions Over one-third with DWI-confirmed TIA/minor stroke present atypically. The value of atypical symptoms in excluding ischaemia, particularly in patients with vascular risk factors, appears limited.

Role of incubitus truncal ataxia, and equivalent standing grade 3 ataxia in the diagnosis of central acute vestibular syndrome
acute vestibular syndrome is a diagnostic challenge, requiring a rapid and precise diagnosis to take therapeutic actions. Truncal ataxia, inability to sit still, and Babinski flexor dysergy were evaluated. Material anf methods: 52 patients with central pathology (stroke in aica and pica territory) and vestibular neuritis were prospectively studied. MRI of the brain was used as the gold standard.

E-248 Postpartum vertebral artery dissections: a report of 7 cases and literature review
ConclusionA limited cohort of 33 ppVADs exist in the literature. This study contributes 7 additional cases and suggests that, despite heterogenous management of ppVAD, the resulting outcomes were favorable.Disclosures V. Lazarov: None. A. Monteiro: None. F. Infratentorial developmental venous anomaly concurrent with a cavernoma and dural arteriovenous fistula
A 32-year-old woman underwent a head computed tomography scan after a traffic accident. A calcification in the left cerebellar hemisphere was disclosed. She did not complain of any symptoms, but neurological examination revealed cerebellar ataxia. She did not have any preceding episodes since birth, such as acute-onset headache. Magnetic resonance imaging showed past hemorrhage of the calcification lesion suggestive of a cerebellar cavernoma. A dilated vascular structure was also observed (Fig.  1A, B).

Risk Factors and Risk Model Construction of Stroke in Patients with Vertigo in Emergency Department
CONCLUSIONS: Age ≥ 60 years old, diabetes, atrial fibrillation, previous history of vertigo or inner ear disease, focal muscle weakness, dysphagia, or ataxia were associated with a higher risk of stroke. The risk model constructed based on our findings may help to assess the risk of stroke in patients with vertigo in the emergency department.PMID:35633924 | PMC:PMC9142295 | DOI:10.1155/2022/2968044

Post-varicella neurological complications: A preliminary observation from a tertiary care centre of Eastern India
Conclusion: Chickenpox is a common viral disease and most patients recover without any complication. Although rare, neurological complications following acute varicella infection may have myriad presentations ranging from lower motor neuron facial palsy to life-threatening encephalitis. Compared to other studies, varicella encephalitis and ataxia were not so common in our study group. Response to therapy was uniformly good except in the patients presenting with ataxia. Response was particularly good to central and peripheral demyelinating disorders.

Thioredoxin 1 regulates the pentose phosphate pathway via ATM phosphorylation after experimental subarachnoid hemorrhage in rats
Brain Res Bull. 2022 May 16;185:162-173. doi: 10.1016/j.brainresbull.2022.05.008. Online ahead of print.ABSTRACTSubarachnoid hemorrhage (SAH), a type of hemorrhagic stroke, is a neurological emergency with high morbidity and mortality. Early brain injury (EBI) after SAH is the leading cause of poor prognosis in SAH patients. TRX system is a NADPH-dependent antioxidant system which is composed of thioredoxin reductase (TRXR), thioredoxin (TRX). The pentose phosphate pathway (PPP), a pathway through which glucose can be metabolized, is a major source of NADPH. Thioredoxin 1 (TRX1) is a member of thioredoxin system mainly loc...
