Predicting independence of gait by assessing sitting balance through sitting posturography in patients with subacute hemiplegic stroke.
CONCLUSIONS: Sitting posturography parameters is clinically useful because they can quantitatively assess post-stroke balance and neurological impairment and predict post-stroke independence of gait even when patients cannot reach their arms forward or stand upright. Cognition assessments to predict inpatient falls in a subacute stroke rehabilitation setting.
Conclusions: Cognition assessments, i.e., one FIM item, one RAVLT item, or a one-minute verbal fluency test are significant falls predictors. PMID: 32431244 [PubMed - as supplied by publisher]

Clinical and psychosocial predictors of exceeding target length of stay during inpatient stroke rehabilitation.
CONCLUSIONS: Findings suggest that social and stroke-specific factors may be helpful to adjust LOS expectations and promote efficient resource allocation. This exploratory study was limited to findings from one inpatient rehabilitation hospital. Cross-validation of results using data-sets from multiple rehabilitation hospitals across Ontario is recommended. PMID: 28486861 [PubMed - as supplied by publisher]

Analysis of Run Sheets of EMS Serving a Metropolitan Comprehensive Stroke Center (P1.268)
Conclusions:Patients with acute stroke transported by EMS have better quality metrics and higher chance of receiving acute treatment. EMS documented assessment allows for the diagnosis or stroke in general but cannot identify LVO because the assessment of cortical signs is very limited.Disclosure: Dr. Hussein has nothing to disclose. Remote Assessment of Stroke Using the iPhone 4
This study demonstrates the efficacy of using the relatively inexpensive iPhone 4 in telestroke management. Twenty patients with stroke were assessed at the bedside using an iPhone 4, and each examination was directed remotely on another iPhone 4. Both the physician performing the bedside exam and the remote physician calculated a National Institutes of Health Stroke Scale (NIHSS) score for each patient. Each physician was blinded to the other’s NIHSS score. In the 20 patients assessed, NIHSS scores ranged from 0 to 22. Interrater reliability assessed using the κ statistic demonstrated excellent agreement in 10 items (l...

To Treat or Not to Treat?: Pilot Survey for Minor and Rapidly Improving Stroke Brief Reports
Conclusions— This pilot survey provides the first quantitative evidence that National Institutes of Health Stroke Scale score is not the only determinant of treatment decision. A National Institutes of Health Stroke Scale score of 2 is the potential equipoise point, with the least consensus on treatment decision. These preliminary findings require validation in larger population surveys.

A rare cause of stroke in young: PHACE syndrome (P1.253)
Conclusions:Neurocutaneous syndromes are typically diagnosed in childhood and can be a rare cause of stroke in young. Our patient suffered an ischemic stroke secondary to sequelae of undiagnosed PHACE syndrome. Early identification of neurocutaneous syndromes allows for proper surveillance, evaluation, and preventative education for development of complications.Disclosure: Dr. Fitzgerald has nothing to disclose. Stroke and nonstroke brain attacks in children
Conclusions: Brain attack etiologies differ from adults, with stroke being the fourth most common diagnosis. These findings will inform development of ED clinical pathways for pediatric brain attacks.

Stroke Scale Items Associated with Neurologic Deterioration within 24 Hours after Recombinant Tissue Plasminogen Activator Therapy
In conclusion, ND of ischemic stroke patients treated with intravenous rt-PA therapy was frequently seen within 120 minutes after therapy. Items such as loss of consciousness and motor limbs total may be considered indices for monitoring neurologic deficits after therapy.

A Case of Creutzfeldt-Jakob Disease with Stroke-Like Onset
Creutzfeldt-Jakob disease (CJD) is a prion disease characterized by rapidly progressive dementia that is often followed by behavioral disturbances, ataxia, myoclonus, and akinetic mutism. The initial symptoms of CJD reportedly vary, but the onset is usually gradual. Here, we report a case of CJD with a sudden, stroke-like onset of right hemiparesis to alert readers that CJD can mimic a stroke during its early stage.

Teaching Video NeuroImages: Thalamic infarct with pseudo-abducens and vertical gaze palsies and an unusual stroke mechanism
A 32-year-old man presented with confusion and diplopia. His examination showed complete vertical gaze palsy with relatively preserved vertical vestibulo-ocular reflexes, convergence nystagmus on attempted upgaze, alternating adducting hypertrophic skew deviation, limited right eye abduction with esotropia, and right limb ataxia (video on the Neurology® Web site at Neurology.org). MRI showed a left paramedian thalamic infarct (figure 1). Vertical gaze palsy results from damage to the rostral interstitial nucleus of the medial longitudinal fasciculus. The contralateral abduction limitation is consistent with pseudo-abdu...

Intravenous Thrombolysis in Posterior Circulation Stroke
Conclusions: Up to date, no data about PCIS and IVT are available from RTCs. Based on limited results from retrospective clinical studies and case series, IVT is safer for use in PCIS than in ACIS. Patients with brainstem ischemia, vertebral artery occlusion, and absence of basilar or posterior cerebral artery occlusion could be considered for treatment with IVT even in borderline cases. Time to IVT in PCIS seems to be a less crucial factor than in ACIS. IVT for PCIS may be beneficial even after 4.5 h from symptom onset. Risk Factors and Risk Model Construction of Stroke in Patients with Vertigo in Emergency Department
CONCLUSIONS: Age ≥ 60 years old, diabetes, atrial fibrillation, previous history of vertigo or inner ear disease, focal muscle weakness, dysphagia, or ataxia were associated with a higher risk of stroke. The risk model constructed based on our findings may help to assess the risk of stroke in patients with vertigo in the emergency department.PMID:35633924 | PMC:PMC9142295 | DOI:10.1155/2022/2968044
