Thyroid Storm In Heart Failure: A Case Report
Thyroid storm (TS) is a rare presentation of hyperthyroidism. Heart failure (HF) is the leading cause of death. Hyperthyroidism is usually related to high-output HF and rarely to low-output HF secondary to dilated thyrotoxic cardiomyopathy (DTC). Burch-Wartofsky criteria is used to diagnose TS. Management includes anti-thyroid drugs, b blockers, iodine and steroids. The use of beta blockers proves challenging given their negative chronotropic and inotropic effects. Maternal Thyroid Dysfunction During Pregnancy as an Etiologic Factor in Autism Spectrum Disorder: Challenges and Opportunities for Research
CONCLUSIONS: Overall, evidence supported a positive association between maternal thyroid dysfunction during pregnancy and ASD outcomes in the child, especially for hypothyroidism. Future studies could reduce misclassification bias by using laboratory measures instead of medical records to ascertain thyroid dysfunction and evaluating children for ASD at an age when it can be reliably detected. Total Thyroidectomy while on Extracorporeal Membrane Oxygenation for Thyroid Storm
We present the first case reported of total thyroidectomy successfully performed while on ECMO for thyroid storm in a 32-year-old male. This case highlights the challenges of managing refractory thyroid storm with multi-organ failure. We demonstrate that total thyroidectomy may still be safely performed while on ECMO for thyroid storm, if steps are taken to optimise the patient perioperatively, with careful surgical and anaesthesia planning.
