Chronic inducible urticaria: classification and prominent features of physical and non-physical types.
Authors: Pozderac I, Lugović-Mihić L, Artuković M, Stipić-Marković A, Kuna M, Ferček I Abstract Chronic inducible urticaria (CIndU) is a common inflammatory skin condition characterized by the recurrence of itchy wheals and/or angioedema that lasts more than 6 weeks and is induced by specific physical or environmental stimuli (cold, heat, exercise, pressure, sunlight, vibration, water, etc.). According to the current international classification, it includes physical urticarias (dermographism, delayed-pressure urticaria, exercise-induced urticaria, cold urticaria, heat urticaria, solar urticaria, and...

[Panic and pandemic: Review of the literature on the links between panic disorder and the SARS-CoV-2 epidemic].
Authors: Javelot H, Weiner L Abstract Although the "panic" word has been abundantly linked to the SARS-CoV-2 (severe acute respiratory syndrome coronavirus 2) pandemic in the press, in the scientific literature very few studies have considered whether the current epidemic could predispose to the onset or the aggravation of panic attacks or panic disorder. Indeed, most studies thus far have focused on the risk of increase and aggravation of other psychiatric disorders as a consequence of the SARS-CoV-2 epidemic, such as obsessive-compulsive disorder (OCD), post-traumatic stress disorder (PTSD), and generali...

Intravenous Cetirizine (Quzyttir) for Acute Urticaria
Date: April 6, 2020 Issue #:  1595Summary: Quzyttir (TerSera), an IV formulation of the second-generation H1-antihistamine cetirizine, has been approved by the FDA for treatment of acute urticaria in patients≥6 months old. Oral formulations of cetirizine (Zyrtec, and others) have been for sale over the counter for years.Quzyttir is the first parenteral formulation of a second-generation H1-antihistamine to become available in the US; parenteral formulations of two first-generation H1-antihistamines, diphenhydramine (Benadryl, and others) and hydroxyzine (Vistaril, and others), have been a...

Efficacy and Safety of Midazolam Oral Solution for Sedative Hypnosis and Anti-anxiety in Children: A Systematic Review and Meta-Analysis
Conclusions: Available evidence suggests that midazolam oral solution is as good as midazolam injection and dexmedetomidine and is better than ketamine. Based on efficacy and safety results, an oral midazolam solution dose of 0.5–1 mg/kg is recommended for children. (Source: Frontiers in Pharmacology)

Pitfalls of toxicological investigations in hair, bones, and nails in extensively decomposed bodies: illustration with two cases
AbstractIt is difficult to carry out toxicological investigations in biological samples collected from extensively decomposed bodies and to interpret obtained results as several pitfalls should be considered: redistribution phenomena, degradation of xenobiotics during thepostmortem period, contamination by putrefaction fluids, and external contamination. This work aims to present two cases in order to illustrate and discuss these difficulties in this tricky situation. Case#1: the body of a 30-year-old woman was found in a wooded area (1  month after she has been reported missing by her family): hair and a femur section we...

Cetirizine use in childhood: an update of a friendly 30-year drug
Cetirizine is a second-generation antihistamine, derived from the metabolism of hydroxyzine, highly specific for the H1 receptors, and with marked antiallergic properties. Although its history began more than ... (Source: Clinical and Molecular Allergy)

Pre-operative anxiolysis in children through a combined pharmacological therapy with hydroxyzine and a non-pharmacological distraction technique with a clown (SONRISA): study protocol for randomised double-blind clinical trial
Surgery can generate significant stress and anxiety in up to 70% of the paediatric population. There are several pharmacological and non-pharmacological strategies to reduce pre-operative anxiety in children, ... (Source: Trials)

Interventions to reduce bruxism in children and adolescents: a systematic scoping review and critical reflection
Conclusions: Several intervention options are available to inhibit or reduce bruxism activity. The respective indication, contraindications, and side effects of each treatment option must be assessed individually and carefully, taking into account that bruxism is not considered a disorder in otherwise healthy individuals. What is known•Biological and psychological factors have been strongly correlated to the development of bruxism• Bruxism prevalence ranging from 6 to 50% in childrenWhat is new• Reduction in self-reported bruxism and headaches associated with bruxism were observed in studies that used medication (Hydr...

Temporal Trends in the Use of Anticholinergic Drugs Among Older People Living in Long-Term Care Facilities in Helsinki
ConclusionsDespite increased knowledge of the harms of DAPs, they remain widely used. Physicians seem to be aware of the harms of DAPs among people with dementia, and some other favorable trends in prescribing were also observed. Clinicians should especially consider the indications behind the use of DAP antidepressants and antipsychotics, and carefully weigh their potential benefits and harms. (Source: Drugs and Aging)

CSACI position statement: Newer generation H1-antihistamines are safer than first-generation H1-antihistamines and should be the first-line antihistamines for the treatment of allergic rhinitis and urticaria
Oral H1-antihistamines (AHs) are the most commonly used therapy to treat allergic rhinitis and chronic urticaria. Older, first-generation AHs (e.g. diphenhydramine, hydroxyzine) have significant and common side e... (Source: Allergy, Asthma and Clinical Immunology)

Base-Mediated 1,6-Aza-Michael Addition of Heterocyclic Amines and Amides to para-Quinone Methides Leading to Meclizine-, Hydroxyzine- and Cetirizine-like Architectures
Synthesis DOI: 10.1055/s-0039-1690677 An expeditious, cost-effective synthetic methodology for a wide range of nitrogen-containing unsymmetrical trisubstituted methanes (TRSMs) is reported. The synthesis involves base-mediated 1,6-conjugate addition of heterocyclic amines and amides to substituted para-quinone methides, giving the unsymmetrical TRSMs in moderate to very good yields (up to 83%) in one pot. The low cost, mild temperature, high atom economy and yields, easy scale-up and broad substrate scope are some of the salient features of this protocol. Further, the methodology could be extended for the synthesis of mecl...

The Case | Unexplained stupor with a high osmolal and high anion gap metabolic acidosis
A 51-year-old male with a psychiatric history was admitted to the hospital for evaluation of altered mental status. He had been suffering from an intense itching involving his head and torso for the past 2 years. A thorough evaluation by a dermatologist was negative for any organic skin pathology. Several medications, including cetirizine, hydroxyzine, risperidone, gabapentin, did not alleviate his itching. The patient started applying hand sanitizer diffusely to his skin several times a day. In a desperate attempt to achieve relief, he began scrubbing the sanitizer with a hard brush to his skin, which led to peeling of th...

Prescription Medications for the Treatment of Insomnia and Risk of Suicide Attempt: a Comparative Safety Study
Conclusions and RelevanceCompared to zolpidem, hazard of suicide attempt was 61% higher with trazodone (<  200 mg). No significant differences in suicide attempt risk were identified between benzodiazepines or sedating antihistamines and zolpidem, respectively. These findings provide the first comparative effectiveness evidence against the use of trazodone for insomnia. (Source: Journal of General Internal Medicine)
