Efficient synthesis of deuterium labeled hydroxyzine and aripiprazole
Hydroxyzine and aripiprazole are active pharmaceutical ingredients that have been largely acknowledged for their antipsychotic properties. Deuterium labeled isotopes of hydroxyzine and aripiprazole are internal standards that can aid in the further research of non‐isotopic forms via quantification analysis using HPLC‐MS/MS. The synthesis of hydroxyzine‐d8 was accomplished by coupling piperazine‐d8 with 4‐chlorobenzhydryl chloride followed by the reaction of the first intermediate with 2‐(2‐chloroethoxy) ethanol to afford 11.7% of hydroxyzine‐d8 with 99.5% purity. The synthesis of aripiprazole‐d8 was also ...

Potentially Inappropriate Anticholinergic Medication Use in Community-Dwelling Older Adults: A National Cross-Sectional Study
Conclusion The study found that approximately one in ten older adults used potentially inappropriate anticholinergic medications. Several predisposing, enabling and need factors were associated with the use of potentially inappropriate anticholinergic medications. Concerted efforts are needed to optimize potentially inappropriate anticholinergic medication use in older adults. (Source: Drugs and Aging)

Effect of a combination of duloxetine with hydroxyzine on experimental models of anxiety in mice
Conclusion: Combination of duloxetine with hydroxyzine is not beneficial in anxiolytic treatment than the respective monotherapies. There is a need to study the pharmacokinetic drug-drug interactions to understand the present study outcomes. (Source: Indian Journal of Pharmacology)

Successful control of radicular pain in a pediatric patient with Guillain–Barré syndrome
A 10-year-old boy was diagnosed as having the axonal form of Guillain–Barré syndrome (GBS). The patient noticed progressive weakness of the lower legs on the 1st day of illness. Intravenous immunoglobulin therapy was immediately started on the 2nd day of illness. Despite the favorable recovery of muscle weakness, he complained of severe needle-like pain in the thighs and buttocks and also painful numbness over the gastrocnemius regions. Acetaminophen and hydroxyzine therapy was ineffective for the pain control. (Source: Brain and Development)

Successful treatment of hydroxyzine and dexclorfeniramine maleate in combination with electroconvulsive therapy in a neuroleptic malignant catatonia: a case report
Malign neuroleptic syndrome is a potentially life-threatening condition that is normally treated with electroconvulsive therapy (ECT). In this case report, we discuss a severely agitated and catatonic bipolar I patient who developed a neuroleptic malignant syndrome and did not improve with benzodiazepines and ECT. On the basis of anecdotal reports of the positive effects of antihistamines in psychosis and ECT, we treated our case with a combination of two antihistamines, hydroxyzine and dexclorfeniramine maleate, and ECT, which improved the clinical picture to the point of clinical remission. C3-C4 shingles post haematopoietic stem-cell transplantation
A 12-year-old boy, 3 months following matched unrelated haematopoietic stem-cell transplantation (HSCT) for relapsed acute myeloid leukaemia with inv(16)(p13.1q22) developed a rapidly spreading, severely pruritic right sided C3-C4 shingles. Despite early intravenous aciclovir (10 mg/kg, three times a day), he developed full C3-C4 dermatome skin erosion to the dermis with secondary hair loss extending to the occiput (figure 1). Following 3 weeks of intravenous morphine sulfate and broad spectrum antibiotics, oral hydroxyzine and cetirizine, together with topical fusidic acid, paraffin gel and miconazole he be...

Evaluation of Treatments for Pruritus in Epidermolysis Bullosa
The objective of the current study was to determine quantitatively which treatments individuals with EB have used for pruritus and to evaluate the perceived effectiveness of these treatments in pruritus relief. A questionnaire was developed to evaluate the treatments and therapies used for pruritus in patients of all ages and for all types of EB. Questions about bathing products, moisturizers, topical products, oral medications, dressings, and alternative therapies were included. A 5‐point Likert scale (−2 = relieves itch a lot, −1 = relieves itch a little, 0 = no change, 1 = increases itch a little, 2 = in...

Sleep measurements in women with dysthymic disorder and insomnia
Abstract To determine the effects of trazodone, mirtazapine and hydroxyzine on sleep profiles of dysthymic disorder (DD) women with selective serotonin reuptake inhibitor (SSRI) treatment and compare their polysomnographic measurements with controls. Among 36 patients trazodone receiving (n = 9), mirtazapine receiving (n = 8), hydroxyzine receiving (n = 8) and only SSRI receiving (n = 11) patients were compared to each other and to controls in terms of polysomnographic (PSG) findings. Rapid eye movements (REM) ratio and sleep onset time (SO) were higher; slow wave sleep (non‐REM3) ratio and total sleep du...

H1-antihistamines for chronic spontaneous urticaria.
CONCLUSIONS: Although the results of our review indicate that at standard doses of treatment, several antihistamines are effective when compared with placebo, all results were gathered from a few studies or, in some cases, from single-study estimates. The quality of the evidence was affected by the small number of studies in each comparison and the small sample size for many of the outcomes, prompting us to downgrade the quality of evidence for imprecision (unless stated for each comparison, the quality of the evidence was low).No single H1-antihistamine stands out as most effective. Cetirizine at 10 mg once daily in the s...

Cutaneous drug eruption induced by antihistamines
We report a 70‐year‐old woman whose medical history included lichen planus, and who was referred by the dermatology department of our hospital for suspected allergy to corticosteroids. The reason for referral was that on the fourth day of treatment with prednisone and hydroxyzine, the patient presented a bilateral highly pruritic palmar erythema that evolved to a generalized morbilliform rash with subsequent complete desquamation. At a later time, she took cetirizine for a cold, and developed palmar erythema and desquamation. Skin tests (prick and intradermal tests) were performed with steroids, and patch tests (read a...

Bilastine vs. hydroxyzine: occupation of brain histamine H1‐receptors evaluated by positron emission tomography in healthy volunteers
ConclusionsA single oral dose of bilastine 20 mg had minimal H1RO, was not associated with subjective sedation or objective impairment of psychomotor performance and was devoid of treatment‐related sedative AEs, thus satisfying relevant subjective, objective and PET criteria as a non‐sedating antihistamine. Drug-induced adverse reactions via breastfeeding: a descriptive study in the French Pharmacovigilance Database
Conclusion This study shows that ADR via breastfeeding are rarely reported due to low awareness or low occurrence of ADR via breast milk. These results highlight the need for additional pharmacokinetic, clinical and epidemiological studies, given the paucity of published data. They also demonstrate the need to improve information for the general public about drugs and self-medication during breastfeeding. (Source: European Journal of Clinical Pharmacology)
