Suppression of wheal and flare in histamine test by the main H1 antihistamines commercialized in Brazil

Treatment with omalizumab or cyclosporine for resistant chronic spontaneous urticaria?
Koski and Kennedy1 review omalizumab and cyclosporine for the treatment of antihistamine-resistant chronic spontaneous (idiopathic) urticaria. Although appropriately indicating cetirizine to be used in up to 4 times the maximum recommended dose, they indicate adding a leukotriene receptor antagonist, diphenhydramine, or hydroxyzine as a second step. However, a systematic review concluded that leukotriene receptor antagonists were not effective as monotherapy or in combination with antihistamines. Treating Pediatric Anxiety: Initial Use of SSRIs and Other Antianxiety Prescription Medications.
CONCLUSIONS: SSRIs are the most commonly used first-line medication for pediatric anxiety disorders, with about half of SSRI initiators continuing treatment for 6 months. Still, a third began therapy on a non-SSRI medication, for which there is limited evidence of effectiveness for pediatric anxiety, and a notable proportion of children initiated with 2 antianxiety medication classes. Chloral hydrate as a sedating agent for neurodiagnostic procedures in children.
CONCLUSIONS: The quality of evidence for the comparisons of oral chloral hydrate against several other methods of sedation was very variable. Oral chloral hydrate appears to have a lower sedation failure rate when compared with oral promethazine for children undergoing paediatric neurodiagnostic procedures. Extrapyramidal Side Effects in a Blue and Gold Macaw (Ara ararauna) Treated with Amitriptyline
Extrapyramidal side effects were observed in a 17-year-old female blue and gold macaw (Ara ararauna) that presented with an acute onset of dystonic reactions (repetitive and abnormal motions, head bobbing) and akathisia (restlessness, pacing) a few hours following a single partial dose of amitriptyline (5 mg/kg) orally. The patient was concurrently being treated with meloxicam (1 mg/kg PO q12h) and had previously been treated with hydroxyzine, prednisolone, leuprolide acetate, and deslorelin acetate, for chronic feather damaging behaviour.
