Topical calcineurin inhibitors in eczema and cancer association: A cohort study.
CONCLUSIONS: In this first Asian study on the risk of TCIs and malignancies, we do not find an association between use of tacrolimus and pimecrolimus in atopic and endogenous eczema and the overall development of malignancies. However, the use of topical tacrolimus was found to be associated with the development of B-cell acute lymphoid leukemia in pediatric eczema patients; further studies are required to investigate if a true association indeed occurs. ETFAD/EADV Eczema task force 2015 position paper on diagnosis and treatment of atopic dermatitis in adult and paediatric patients
Abstract Atopic dermatitis (AD) is a clinically defined, highly pruritic, chronic inflammatory skin disease of children and adults. The diagnosis is made using evaluated clinical criteria. Disease activity is best measured with a composite score assessing both objective signs and subjective symptoms, such as SCORAD. The management of AD must consider the clinical and pathogenic variabilities of the disease and also target flare prevention. Basic therapy includes hydrating topical treatment, as well as avoidance of specific and unspecific provocation factors. Anti‐inflammatory treatment of visible skin lesions is based on...

Possible alternative therapies for oral lichen planus cases refractory to steroid therapies
Publication date: Available online 13 February 2016 Source:Oral Surgery, Oral Medicine, Oral Pathology and Oral Radiology Author(s): Huamei Yang, Yuanqin Wu, Hui Ma, Lu Jiang, Xin Zeng, Hongxia Dan, Yu Zhou, Qianming Chen Oral lichen planus (OLP) is a chronic inflammatory disorder with multifactorial etiopathogenesis. Immune dysregulation plays a critical role in the development and progression of this disease. Patients’ life may be affected by pain caused by atrophic-erosive lesions. Treatment is usually symptomatic given the obscure etiology. Topical steroids remain the mainstay of management. Howev...

Different Advanced Therapeutic Approaches to Treat Vitiligo.
This article reviews the published literature on various treatment modalities that might be effective in successfully treating patients with vitiligo, including phototherapies or some photochemotherapies, vitamin D analogs, topical and systemic corticosteroids, zinc treatment, anti-tumor necrosis factor agents, calcineurin inhibitors (tacrolimus, pimecrolimus), and surgical methods. This critical review also discusses a few herbal medications that may be worthy of future investigation because they have no significant side effects. Pimecrolimus micelle exhibits excellent therapeutic effect for Keratoconjunctivitis Sicca
Publication date: 1 April 2016 Source:Colloids and Surfaces B: Biointerfaces, Volume 140 Author(s): Fan Yingfang, Bo Zhuang, Cheng Wang, Xuelian Xu, Wei Xu, Zhihua Lv Poor corneal penetration and short residence time on the ocular surface are two major bottlenecks for conventional ophthalmic formulations. To overcome the foregoing dilemmas, we prepared two novel formulations of pimecrolimus nanomicelles (PNM) with particle size of 37.85±1.21nm and thermosensitive hydrogel (PTH) for treating Keratoconjunctivitis Sicca (KCS). PNM were investigated by transmission electron microscopy (TEM), Malvern laser part...

Association Between Leukemia and Topical Use of Pimecrolimus
In Reply We thank Drs Yew and Tey for their interest in our study. We are, however, concerned about their interpretation of our study results. Secondary analyses, as well as interpretations of analyses that were not part of the original study design, can result in spurious conclusions. Our study was not designed to evaluate only individuals of aged 1 to 4 years and was not designed to specifically study acute lymphoblastic leukemia (ALL). The investigation was designed to study all cancer and lymphoma in children (aged 2 to 18 years) as a group who were or will be followed for up to 10 years. (Source: JAMA Dermatology)

Association Between Leukemia and Topical Use of Pimecrolimus
To the Editor We read with interest the observational cohort study by Margolis and colleagues that evaluated the risk of cancer among patients in the Pediatric Eczema Elective Registry who were exposed to pimecrolimus. The authors reported that the overall incidence rates of all cancers were not significantly higher than expected rates based on age-standardized rates from the Surveillance, Epidemiology, and End Results database. While it is certainly heartening to note these reassuring results, we are concerned about the age-specified incidence rate of leukemias among the age group 0 to 4 years in this study. There were 2 ...

Food Allergy in Infants With Atopic Dermatitis: Limitations of Food-Specific IgE Measurements

Perioral dermatitis: still a therapeutic challenge.
PERIORAL DERMATITIS: STILL A THERAPEUTIC CHALLENGE. Acta Clin Croat. 2015 Jun;54(2):179-85 Authors: Mokos ZB, Kummer A, Mosler EL, Čeović R, Basta-Juzbašić A Abstract Perioral dermatitis is a common and often chronic dermatosis. In its classic form, it primarily affects women aged 15 to 45 years, but there are also variants including lupus-like and granulomatous perioral dermatitis, where granulomatous form is more common in childhood and affects mostly prepubescent boys. The etiopathogenesis of the disease remains unclear, but there is a frequent finding of prolonged use of t...

Topical glucocorticoid or pimecrolimus treatment suppresses thymic stromal lymphopoietin-related allergic inflammatory mechanism in an oxazolone-induced atopic dermatitis murine model
Abstract Congenitally or early impaired skin barrier as the first event starting the ‘atopic march’ in atopic dermatitis (AD) patients can increase allergen penetration that results in sensitization, even in the airways, followed by asthma and allergic rhinitis. Thymic stromal lymphopoietin (TSLP) is a cytokine existing in high levels in AD skin and is considered as a novel therapeutic target for atopic disease. We generated oxazolone (Ox)-induced AD-like (Ox-AD) hairless mice and divided them into four groups according to the therapeutic challenges: topical glucocorticoid, pimecrolimus, emollient, ...

Rosacea-like eruption due to topical pimecrolimus.
We report a case of a rosacea-like eruption in a 39-year-old woman occurring after the use of pimecrolimus cream for 12 months for atopic dermatitis. Withdrawal of pimecrolimus combined with treatment with oral lymecycline, topical metronidazole, and an emollient resulted in resolution of the eruption. There have been 5 previously reported cases of a topical pimecrolimus-induced rosacea-like eruption suggesting that this rare side-effect may be a class effect of all topical calcineurin inhibitors. Dermatologists prescribing these drugs should be aware of this uncommon complication and may wish to warn patients of its occur...

Efficacy of tacrolimus and clobetasol in the treatment of oral lichen planus: a systematic review and meta‐analysis
This study was carried out to assess and compare the efficacy of tacrolimus and clobetasol in the treatment of oral lichen planus (OLP). The Cochrane Central Register of Controlled Trials, PubMed, Scopus, Science Direct, Springer Journals, and Elsevier databases were searched using specific keywords relevant to the research question for articles published from 1998 to December 31, 2012. Finally, 15 articles that assessed the effects of tacrolimus, clobetasol, and pimecrolimus on improvements in OLP were reviewed. In addition, a meta‐analysis of odds ratios (ORs) was carried out for data in 10 of the 15 articles. The resu...
