Doxazosin improved COVID-19 associated nightmare in a patient with major depressive disorder: a case report with a positive rechallenge
This article reports on the treatment of a patient with nightmares who was treated with doxazosin of an alpha 1-adrenergic antagonists. A 71-year-old Japanese major depressive disorder (MDD) woman experienced nightmares after the coronavirus disease 2019 pandemic. She had nightmares about being chased by a coronavirus and catching the corona virus. After adding doxazosin 1 mg daily in the morning, her nightmares led to remission without side effects. We also had a rechallenge regimen with doxazosin. The nightmares ceased on the second night of the rechallenge and did not return with continued treatment. This case report ...

Parkinson disease among patients treated for benign prostatic hyperplasia with α1 adrenergic receptor antagonists
BACKGROUND Recently the α1 adrenergic receptor antagonist terazosin was shown to activate PGK1, a possible target for the mitochondrial deficits in Parkinson disease related to its function as the initial enzyme in ATP synthesis during glycolysis. An epidemiological study of terazosin users showed a lower incidence of Parkinson disease when compared with users of tamsulosin, an α1 adrenergic receptor antagonist of a different class that does not activate PGK1. However, prior research on tamsulosin has suggested that it may in fact potentiate neurodegeneration, raising the question of whether it is an appropriate control ...

Overexpressed XRCC2 as an independent risk factor for poor prognosis in glioma patients
CONCLUSIONS: This study analyzed the expression pattern of XRCC2 in gliomas and its relationship with prognosis using multiple datasets. This is the first study to show that XRCC2, a novel oncogene, is significantly overexpressed in glioma and can lead to poor prognosis in glioma patients. XRCC2 could serve as a new biomarker for glioma diagnosis, treatment, and prognosis evaluation, thus bringing new insight into the management of glioma.PMID:34051735 | DOI:10.1186/s10020-021-00316-0 (Source: Molecular Medicine)

Hypertension in Pheochromocytoma and Paraganglioma: Evaluation and Management in Pediatric Patients
AbstractPurpose of ReviewThe rare catecholamine-secreting tumors, pheochromocytomas and paragangliomas (PPGL), account for a minority of cases of secondary hypertension in pediatrics. As such, perioperative blood pressure (BP) management in pediatric patients presents a distinct challenge. This review will expand the practitioner ’s knowledge of antihypertensive treatment options for the pediatric patient with PPGL with a focus on literature in the past several years.Recent FindingsThere continue to be only small case series and single-center experiences to provide guidelines regarding BP management. While phenoxybenzami...

Prazosin for Alcohol Use Disorder: A Symptom-Driven Approach to the Choice of Intervention
J Clin Psychiatry. 2021 Mar 23;82(2):21f13980. doi: 10.4088/JCP.21f13980.ABSTRACTThe choice from among approved treatments for relapse prevention in alcohol use disorder (AUD) is not symptom-driven. It is reasonable to speculate that the discomfort and distress associated with the experience of alcohol withdrawal symptoms (AWS) discourage abstinence and prompt continuation of or relapse into drinking. Adrenergic mechanisms may underlie many of the commonly experienced AWS. This allows the further speculation that drugs with antiadrenergic properties may attenuate AWS and thereby improve treatment outcomes in patients with ...

69-Year-Old Man With Nasal Congestion and Pre-Syncope
A 69-year-old man with a past medical history consisting only of benign prostatic hyperplasia (BPH) presented to his primary care physician complaining of pre-syncopal episodes in the setting of a sinus infection lasting greater than 1 month. The patient was taking doxazosin for BPH and occasional ibuprofen for pain related to his sinus infection. He denied any new urinary symptoms such as dysuria or hematuria. He reported no changes in his dietary habits. Vital signs were blood pressure 124/76 mm Hg, pulse 84 beats/min, respiratory rate 14 breaths/min, temporal temperature 36.3 °C, height 173 cm, and weight 78.2 kg. (Sou...

Preoperative amlodipine is efficacious in preventing intraoperative HDI in pheochromocytoma- pilot RCT
J Clin Endocrinol Metab. 2021 Apr 11:dgab231. doi: 10.1210/clinem/dgab231. Online ahead of print.ABSTRACTBACKGROUND: Preoperative blockade with α-blockers is recommended in patients with pheochromocytoma/paraganglioma (PPGL). The data on calcium channel blockade (CCB) in PPGL is scarce. We aimed to compare the efficacy of CCB and α-blockers on intraoperative haemodynamic instability (HDI) in PPGL.METHODS: In the interim analysis of this monocentric, pilot, open-label, randomized controlled trial, patients with solitary, secretory, and nonmetastatic PPGL were randomized to oral prazosin GITS (maximum 30mg, n=9) or amlodip...
