Remnant cholesterol is associated with the progression and regression of metabolic dysfunction-associated steatotic liver disease in Chinese adults
The prevalence of nonalcoholic fatty liver disease (NAFLD) has progressively increased over recent decades with increased rates of obesity and the rapid growth of the ageing population, affecting approximately 29% of the Chinese population (1). In addition to intrahepatic diseases such as liver cirrhosis and hepatocellular carcinoma, NAFLD has been demonstrated to increase the risks of multiple extrahepatic complications, including type 2 diabetes (T2DM), chronic kidney disease, cardiovascular disease (CVD), and even extrahepatic tumours (2,3). Revisiting the association between vitamin D deficiency and active tuberculosis: A prospective case-control study in Taiwan
CONCLUSIONS: VDD is an independent risk factor for incident active TB. Future randomized trials examining the effect of vitamin D supplementation on TB incidence should focus on people with a low BMI or other risk factors to maximize the statistical power.PMID:38594108 | DOI:10.1016/j.jmii.2024.03.005 (Source: Journal of Microbiology, Immunology, and Infection)

Emerging therapies targeting growth factors in hepatocellular carcinoma
Expert Opin Pharmacother. 2024 Apr 9:1-8. doi: 10.1080/14656566.2024.2340714. Online ahead of print.ABSTRACTINTRODUCTION: Hepatocellular carcinoma (HCC) is a primary liver cancer that commonly arises in the background of chronic liver inflammation and/or cirrhosis. Chronic liver inflammation results in the production of different growth factors, remodeling of the microenvironment architecture into fibrosis, and eventually carcinogenesis. Overexpression of some growth factors has been associated with worse prognosis in patients with HCC. Targeted therapies against growth factors may disrupt cell signaling and the mechanisms...

Liver and cardiovascular disease outcomes in metabolic syndrome and diabetic populations: Bi-directional opportunities to multiply preventive strategies
The incidence and prevalence of metabolic syndrome (MetS) and type 2 diabetes mellitus (T2DM) are rising globally. MetS and T2DM are associated with significant morbidity and mortality, which is partly related to liver and cardiovascular disease. Insulin resistance is central to MetS and T2DM pathophysiology, and drives ectopic fat deposition in the liver, also known as metabolic dysfunction-associated steatotic liver disease (MASLD). MetS and T2DM are not only risk factors for developing MASLD but are also independently associated with disease progression to steatohepatitis, cirrhosis, and hepatocellular carcinoma. (Sourc...

Indications to LDLT: a changing pathway?
AbstractLiver transplantation (LT) revolutionized the outlook for cirrhotic patients, offering a potential cure with over 80% life expectancy after 5  years. Cirrhosis, with or without hepatocellular carcinoma (HCC), is the primary LT indication. Living donor LT (LDLT) initially explored as an alternative, declined due to poorer outcomes. Studies on LDLT improved outcomes through precise recipient selection, emphasizing the importance of careful donor/recipient matching. Emerging concepts like left lobe preference and minimally invasive donor approaches enhance LDLT outcomes. The RAPID technique shows promise in both cirr...

Phenotypes of MASLD-associated hepatocellular carcinoma
Hepatocellular carcinoma (HCC) typically develops as consequence of liver cirrhosis, but HCC epidemiology has evolved drastically in recent years. Metabolic dysfunction-associated steatotic liver disease (MASLD), including metabolic dysfunction-associated steatohepatitis (MASH), has emerged as the most common chronic liver disease worldwide and a leading cause of HCC. A substantial proportion of MASLD-associated HCC (MASLD-HCC) can also develop in patients without cirrhosis. The specific pathways that trigger carcinogenesis in this context are not completely elucidated, and recommendations for HCC surveillance in MASLD pat...
