Magnetic resonance cholangiogram patterns and clinical profiles of ketamine-related cholangiopathy in drug users
Recreational use of inhalational ketamine is emerging as a major global social and health issue [1,2]. While ketamine, a N-methyl D-aspartate receptor antagonist, has medical uses in anesthesia and chronic pain control, its highly addictive nature has led to a massive increase in recreational consumption worldwide. Due to the ease of production and low cost, the non-medical use of ketamine is especially increasing in East and South East Asia, with its lifetime prevalence in the general population ranging from 0.3% to 2.0% [3], comprising up to 39.7% of total recreational drugs users in these regions [4]. Variability in Case Durations for Common Surgical Procedures.
CONCLUSIONS: There was wide variation in surgery case duration for patients undergoing common general surgical and orthopedic surgeries. This variability in care delivery may represent an important opportunity to promote more efficient use of health care resources. Laparoscopic cholangiogram in biliary atresia: a refinement in the gallbladder hitch technique
ConclusionThe findings of laparoscopic cholangiogram were confirmed in all six children who underwent laparotomy for Kasai procedure. The laparoscopic cholangiogram using gallbladder hitch reliably demonstrates a patent biliary system (11/11) and was valuable in avoiding further invasive procedures in 70% (14/20) of babies. Analysis of clinical characteristics and treatment of immunoglobulin G4-associated cholangitis: A retrospective cohort study of 39 IAC patients
Immunoglobulin (Ig)G4-associated cholangitis (IAC) is one of the common organ manifestations of IgG4-related systemic disease (ISD). IAC and autoimmune pancreatitis (AIP) may mimic sclerosing cholangitis, cholangiocarcinoma, or pancreatic carcinoma. Diagnosis is based on a combination of clinical, biochemical, radiological, and histological findings. To study the clinical presentation of and treatment strategy for IAC, we reviewed clinical, serologic, and imaging characteristics, as well as treatment response, in 39 patients with IAC. The majority of patients were men (82%). Clinical features on presentation included obst...

Endoscopic diagnosis and treatment of biliary obstruction due to acute cholangitis and acute pancreatitis secondary to Fasciola hepatica infection.
Abstract In the differential diagnosis of biliary obstruction with unknown etiology, biliary fascioliasis should be considered in endemic and nonendemic regions. After diagnostic evaluation, endoscopic retrograde cholangiopancreatography (ERCP) was performed for etiological evaluation and/or treatment of biliary obstruction in five patients with a mean age of 55.8 years. Endoscopic sphincterotomy and cholangiogram revealed linear filling defects in the biliary system. Fasciola hepatica parasites were extracted using balloon and basket catheters in two and three patients, respectively. No morbidity or morta...

Guidewire cannulation in ERCP: from zero to hero!
For decades biliary cannulation during ERCP was done by lining up and advancing a catheter or a sphincterotome into the ampulla at what one believed was a “good” biliary angle and then gently (or not so gently) injecting contrast. This either resulted in a cholangiogram or a pancreatogram. If the bile duct was accessed, a wire was then advanced through the catheter into the biliary tree, and the procedure moved forward. If the pancreas was accesse d, the catheter was pulled out and the maneuver was attempted again (and again and again) until ultimately successful or the biliary tree was not accessed. Selective intraoperative cholangiography during laparoscopic cholecystectomy in children is justified
Intraoperative cholangiograms (IOC) during laparoscopic cholecystectomy have been advocated to detect biliary anomalies and intraductal calculi. However, IOC increases operative time and patient irradiation, and therefore is not universally used. We hypothesise selective IOC may be a valuable tool in children. A Retrospective Review of the Diagnostic and Management Challenges of Mirizzi Syndrome at the Singapore General Hospital
Conclusion: MS is a challenging condition and multimodal diagnostic approach has the greatest yield in achieving accurate pre-operative diagnosis. If suspicion is high, a trial of laparoscopic dissection with low threshold for open conversion is recommended.Dig Surg (Source: Digestive Surgery)

Side-by-side partially covered self-expandable metal stent placement for malignant hilar biliary obstruction
Conclusions Endoscopic SBS placement of a PCSEMS was feasible in patients with unresectable MHBO. Additionally, reinterventional stent removal was possible in the absence of tumor ingrowth. Uncomplicated common bile duct stone removal guided by cholangioscopy versus conventional endoscopic retrograde cholangiopancreatography
ConclusionsIn the management of uncomplicated CBDS, our data confirmed the feasibility of DCS for CBDS clearance as it showed efficacy and safety comparable to those of cERCP. Although certain conditions may limit its effectiveness, DCS offers the ability to perform CBDS clearance without the need for fluoroscopy unit and can avoid radiation exposure while ERCP under fluoroscopy remains the current standard of care in patients with CBDS. IRCAD recommendation on safe laparoscopic cholecystectomy
Abstract An expert recommendation conference was conducted to identify factors associated with adverse events during laparoscopic cholecystectomy (LC) with the goal of deriving expert recommendations for the reduction of biliary and vascular injury. Nineteen hepato‐pancreato‐biliary (HPB) surgeons from high‐volume surgery centers in six countries comprised the Research Institute Against Cancer of the Digestive System (IRCAD) Recommendations Group. Systematic search of PubMed, Cochrane, and Embase was conducted. Using nominal group technique, structured group meetings were held to identify key items for safer LC. Cons...

Hybrid Single-Port Cholecystectomy Vs Four-Port Cholecystectomy in Children.
CONCLUSION: Hybrid single-port laparoscopic cholecystectomy in children with uncomplicated gallbladder disease is feasible and equally safe, with similar intra- and postoperative outcomes compared with the conventional 4-port approach. It can contribute to global cost reduction because of lower operative and total hospitalization costs.
