Duodenoscope combined with laparoscopy in treatment of biliary stones for a patient with situs inversus totalis: A case report
Rationale: Although endoscopic and laparoscopic techniques in situs inversus totalis (SIT) have been reported respectively, endo-laparoscopic combination therapy due to biliary lithiasis remains infrequent. We shared the experience regarding the operations with a video report and discussed the similarities and differences with the usual procedures, which proved to be challenging to some extent for SIT. Patient concerns: Herein we present a 72-year-old man with SIT who underwent endo-laparscopic combination therapy due to choledocholithiasis and gallbladder stone. Diagnosis: Choledocholithiasis; Gallbladder stone; S...

Prevalence and Predictors of Unnecessary Endoscopic Retrograde Cholangiopancreatography in the Two-Stage Endoscopic Stone Extraction Followed by Laparoscopic Cholecystectomy
ConclusionsAbout one fourth of the patients may have unnecessary ERCP in the two-stage procedure. Based on our nomogram  using preoperative factors, high-risk patients who are more likely to perform unnecessary ERCP could be considered for the one-stage procedure. The Southwestern Surgical Congress Multi-Center Trial on Suspected Common Duct Stones
ConclusionSignificant variation exists within the surgical community regarding suspected common duct stones. These results underscore the need for a protocol for common duct stones to minimize multiple, redundant interventions. (Source: The American Journal of Surgery)

Predictors of Biliary Strictures After Liver Transplantation Among Recipients of DCD (Donation After Cardiac Death) Grafts
ConclusionBiliary strictures occur frequently in DCD LT recipients and may be associated with older age of donor. Hyperbilirubinemia immediately after transplant and higher INR in the first 7  days after transplant may predict subsequent development of biliary strictures. Papillary thyroid carcinoma metastatic to the pancreas: Case report
We present the case of an 81‐year‐old man with past medical history of PTC status post total thyroidectomy with local recurrence treated with radioactive iodine and selective neck dissection. Ten years after his initial diagnosis, PET‐CT scan revealed a new hypermetabolic 1.1 cm × 0.9 cm left lower lobe lung nodule and hypermetabolism in the proximal body of the pancreas. Follow‐ up MRI cholangiogram showed a 1.0 × 0.8 cm T1 hypointense lesion in the proximal body of the pancreas. Endoscopic ultrasound‐guided fine‐needle aspiration biopsy of the pancreatic mass showed neoplastic epithelial cells arranged in p...

Efficacy of Endobiliary Radiofrequency Ablation using a Novel Endoluminal Radiofrequency Ablation Catheter in a Swine Model.
CONCLUSION: Endobiliary RFA using a novel RFA catheter resulted in controlled ablation with a linear relationship between microscopic ablation depth and ablation time in a swine model. Clinical studies are needed to validate the safe energy condition of endobiliary RFA in malignant biliary obstruction. Gallstone Pancreatitis and Choledocholithiasis: Using Imaging and Laboratory Trends to Predict the Likelihood of Persistent Stones at Cholangiography

Mirizzi Syndrome: Diagnosis and Management of a Challenging Biliary Disease.
Conclusions: Mirizzi syndrome requires being treated by an experienced biliary surgeon after a careful assessment of the local situation and anatomy. The preoperative placement of a stent via ERCP can simplify the surgical procedure. PMID: 30159303 [PubMed - in process] (Source: Canadian Journal of Gastroenterology)

Spontaneous bile duct perforation in a neonate
We present a case of spontaneous biliary perforation (SBP) in a previously healthy 3-week-old boy with worsening scleral icterus, abdominal distension, and acholic stools. Abdominal ultrasound demonstrated moderate ascites without extrahepatic ductal dilatation. The HIDA scan demonstrated accumulation of tracer within the right upper quadrant outside of the liver but not in the bowel, however there was a large amount of tracer activity seen diffusely within the abdomen that appeared to lie within the ascitic fluid and complicated the read of the scan. Intraoperative cholangiogram during an exploratory laparotomy confirmed ...

Comparison of diagnostic imaging modalities for the evaluation of pancreatic duct injury in children: a multi-institutional analysis from the Pancreatic Trauma Study Group
ConclusionsIn children with blunt pancreatic injury, MRCP is more useful than CT for identifying the pancreatic duct but may not be superior for confirmation of duct integrity. Endoscopic retrograde cholangiogram (ERCP) may be necessary to confirm duct disruption when considering pancreatic resection.Level of evidenceIII. (Source: Pediatric Surgery International)

Laparoendoscopic rendezvous in the treatment of cholecysto-choledocholitiasis: a single series of 200 patients
ConclusionsOur results confirm that LERV technique is a safe procedure with high success rates for the treatment of cholecysto-choledocholitiasis. The major advantages include the single-stage treatment, the shorter hospital stay, and the lower incidence of post-ERCP pancreatitis. (Source: Surgical Endoscopy)

Spontaneous biliary perforation in a 3-month old
Publication date: Available online 30 July 2018Source: Journal of Pediatric Surgery Case ReportsAuthor(s): Vallery Logedi, Erik HansenAbstractSpontaneous biliary perforation is a rare cause of jaundice in young children. A 3-month old girl presented to a referral facility with a history of jaundice and abdominal distension. Investigations suggested a hepatic cyst but on exploration, she was found to have bilious ascites and a sub-hepatic biloma consistent with spontaneous biliary perforation. Intraoperative cholangiogram demonstrated an intact biliary tree and normal duodenal drainage. Closed-suction drainage of the sub-he...
