Predictors of success for double balloon assisted endoscopic retrograde cholangiopancreatography in patients with Roux ‐en‐Y anastomosis
ConclusionThe indication for REYA appears to impact on successful biliary cannulation in patients undergoing DBE‐ERCP. The procedure is most successful in non‐liver transplant adult surgery and post‐transplant patients without a second operation. It is least successful in patients with surgically corrected biliary atresia and post‐transplant patients who had a second operation. Single-stage laparoscopic management of acute gallstone pancreatitis: outcomes at different timings
Conclusions This study demonstrated the feasibility and reproducibility of single-stage laparoscopic management of acute gallstone pancreatitis, which has a low complication rate at any stage. Patients undergoing early treatment have a higher incidence of choledocholithiasis and multiple stones than those treated after 30 days, supporting the passage of stones with time. Biliary tree variations as viewed by intra-operative cholangiography – Comparing Egyptian versus international data
Conclusion In our small scale study (n =248); the more common typical biliary anatomy is observed here in Egypt at percentages nearly similar to that reported in the international literature. On the other hand, the less common variation types, show prevalence here in Egypt that are different from those reported in the international literature; a finding that could cause a higher number of bile duct injuries in laparoscopic cholecystectomies if not recognized. Outcomes after discontinuation of routine use of transanastomotic biliary stents in pediatric liver transplantation at a single site
This study is a retrospective review of 99 patients who underwent first LT (2005–2014). In 2011, RTBS was discontinued at our center. This study describes biliary complications following LT with and without RTBS. 56 (56%) patients had RTBS. Median age at LT was 1.9 yr (IQR 0.7, 8.6); 55% were female. Use of an esophageal stent for successful treatment of significant choledocholithiasis to facilitate direct cholangioscopy

Intraoperative cholangiogram during laparoscopic cholecystectomy: A clinical trial in rural setting
ConclusionThe majority of patients treated in our centre were women, middle‐aged patients booked for elective laparoscopic cholecystectomy. Although only 6.7% cases were suspicious for CBD stones pre‐operatively, a total of 17% of patients with routine IOC suggested CBD stones. IOC was found to be safe, taking only an additional 28 min of total theatre time. Routine rather than selective use of IOC could be considered to improve patient safety and long‐term results. Quadrate lobe: a reliable landmark for bile duct anatomy during laparoscopic cholecystectomy

Virtual Intraoperative Cholangiogram Using WebCL.

A Prospective Blinded Study Evaluating the Role of Endoscopic Ultrasound before Endoscopic Retrograde Cholangiopancreatography in the Setting of "Positive" Intraoperative Cholangiogram during Cholecystectomy.
This study sought to assess the role of EUS in predicting the likelihood of choledocholithiasis at ERCP in patients found to have a positive IOC. This was a prospective blinded study of EUS before ERCP in patients with a positive IOC. Recruited subjects who underwent cholecystectomy and had an IOC with suspicion for obstruction were referred for ERCP within one month of their procedure. Use of digital cholangioscopy in a dilated bile duct for detection of small symptomatic bile duct stones
A 47-year-old woman recently underwent an ERCP with mechanical lithotripsy for a symptomatic stone. The stone was cleared per the endoscopist, but because of ampullary edema a stent was placed. The patient continued to experience right upper-quadrant abdominal pain, and thus the clinical picture suggested a retained stone. A subsequent ERCP was performed for stent removal and re-evaluation. The stent was removed, and multiple balloon sweeps that started at the hilum yielded no stones. An occlusion cholangiogram showed a bile duct dilated to 15  mm and no filling defects (Fig. 1A).
