Comparison of gadoxetic acid to gadobenate dimeglumine for assessment of biliary anatomy of potential liver donors
Conclusion Equivalent image quality was found for T1 MRC obtained with Gd-BOPTA or Gd-EOB-DTPA and T2 MRC. T1 MRC is equivalent to T2 MRC for detection of variant biliary anatomy, and the combination of sequences may have added value. (Source: Abdominal Imaging)

A rare gallbladder ciliated foregut cyst in chronic cholecystitis
CONCLUSION Despite the rarity of CFCs and their potential to mimic malignancy, we propose awareness and understanding of the management for them – being excision and hopefully not cause any confusion or devastatingly allow it to become malignant. Choosing the correct path: the double orifice papilla
A 74-year-old woman with rheumatoid arthritis experienced cholestatic liver function. Abdominal US demonstrated intrahepatic and extrahepatic duct dilatation and gallstones in an enlarged gallbladder. Magnetic resonance cholangiography confirmed echogenic foci within a dilated common bile duct (CBD). ERCP revealed a double orifice major papilla (A). After careful assessment, the cranial orifice at 11 o ’clock was intubated, and a biliary cholangiogram confirmed gallstones in a dilated CBD (B). Sphincterotomy was performed, and a plastic stent was inserted for drainage (C), and the patient was given postprocedure intraven...

Extrahepatic biliary cystadenoma: a rare cause of biliary obstruction.
Authors: Metussin A, Telisinghe P, Kok K, Chong V Abstract Biliary cystadenoma is a rare tumor of the biliary tree and a rare cause of obstructive jaundice. Most are intrahepatic, and pure extrahepatic biliary cystadenoma is less common. Cases are more common in women. Unless suspected, diagnosis of extrahepatic biliary cystadenoma is often delayed. Here, we report the case of a young woman with extrahepatic biliary cystadenoma who presented at Raja Isteri Pengiran Anak Saleha Hospital with obstructive jaundice initially thought to be due to a large biliary stone based on the endoscopic cholangiogram image...

The effect of residents as teaching assistants on operative time in laparoscopic cholecystectomy
Conclusions Residents' operative time improves as experience with LC increases. These improvements become more profound after adjusting for the seniority of the TA. (Source: The American Journal of Surgery)

Adenomas involving the extrahepatic biliary tree are rare but have an aggressive clinical course
We present a case series of extrahepatic bile duct adenomas at our institution, along with a review of the literature. All three patients with extrahepatic biliary adenomas (two in the common bile ducts, one in the hepatic duct) were female with a mean age of 74 years. On initial presentation, none of the patients had obstructive jaundice but two of the three patients had symptoms of biliary origin. Case 1 is an 85-year-old woman with an incidental biliary dilation seen on chest imaging; endoscopic ultrasound revealed a sessile adenomatous polyp in the distal bile duct. The patient refused surgery and presented with occlus...

Endoscopic management of radio-opaque bile duct stones
Conclusions RO bile duct stones provide unique challenges for endoscopic management with success of conventional techniques in only about half of them (46 %). RO stones detected on fluoroscopy are extremely hard and difficult to crush with lithotripsy basket probably due to high calcium content. Cholangioscopy guided LL provides an excellent alternative management strategy. (Source: Indian Journal of Gastroenterology)

Training vs practice: A tale of opposition in acute cholecystitis.
Authors: Patel PP, Daly SC, Velasco JM Abstract Acute cholecystitis is one of the most common surgical diagnoses encountered by general surgeons. Despite its high incidence there remains a range of treatment of approaches. Current practices in biliary surgery vary as to timing, intraoperative utilization of biliary imaging, and management of bile duct stones despite growing evidence in the literature defining best practice. Management of patients with acute cholecystitis with early laparoscopic cholecystectomy (LC) results in better patient outcomes when compared with delayed surgical management techniques...

Single Incision Laparoscopic Cholecystectomy Performed Via the "Marionette" Technique Shows Equivalence in Outcome and Cost to Standard Four Port Laparoscopic Cholecystectomy in a Selected Patient Population.
Abstract The aim of our study is to compare single incision laparoscopic cholecystectomy (SILC) performed using the "marionette" technique (m-SILC), to the standard four-port technique [four-port laparoscopic cholecystectomy (4PLC)]. Patient information was extracted from a prospectively maintained database (n = 188). Our primary endpoint was operative costs (determined by operating time and instruments used). Secondary endpoints were length of stay, operative time, blood loss, and postoperative complication rates. Univariate and adjusted multivariate analysis was used to compare the outcomes. There were a...
