Cervical cancer is the most common cancer treated with radiation in Botswana. There is a single linear accelerator and a single HDR brachytherapy unit in the country, treating a median of 200 cervical cancer cases year. About 80% of these are treated with curative intent, using concurrent chemoradiation with brachytherapy boost. Patients in Botswana often present with advanced disease and very bulky tumours. Cervical cancer is the most common cancer treated at Gaborone Private Hospital (GPH), the only facility offering radiotherapy services in Botswana. For curative intent, patients receive concurrent chemoradiation followed by brachytherapy boost. Brachytherapy services commenced in Botswana in 2012, and treatment began by using traditional A points for planning. Extended balloon labour induction: A single arm proof of concept trial
CONCLUSION: This proof-of-concept study suggests that use of side-by-side balloons in participants with favourable cervix is effective in achieving vaginal birth in most participants without uterotonics or amniotomy. There were no adverse outcomes, though the study was too small to exclude the possibility of rare adverse outcomes. This offers a novel option for labour induction/augmentation, particularly where uterotonics or amniotomy are best avoided such as prior caesarean birth, vertical infection transmission risk and in settings with limited capacity for fetal surveillance.
