A rare case of ileo-ileal intussusception due to a bleeding lipomatous mass treated by laparoscopic ileal resection.
CONCLUSION: Ileal intussusception caused by lipoma is a rare condition, which can be diagnosed with endoscopy, barium enema, and abdominal ultrasound or CT scanning, but preoperative diagnosis may be difficult. The treatment of choice is the reduction of the intussusception and the resection (laparoscopic or open) of the involved tract. KEY WORDS: Intussusception, Lipoma, Ileum, Laparoscopy, Bleeding. Evidence of Different IL-1 β Activation Pathways in Innate Immune Cells From Indeterminate and Cardiac Patients With Chronic Chagas Disease
Conclusions: We suggest some distinct molecular mechanisms for production of IL-1β in innate immune cells from patients with different clinical forms of Chagas disease. MMP-2 and MMP-9 gelatinases are associated with distinct disease outcomes and IL-1β production. Introduction Chagas disease, also known as American trypanosomiasis, is a neglected parasitic disease caused by the protozoan Trypanosoma cruzi (1), that affects millions of people in the world (2). During the acute phase, a diffuse and intense inflammation in the cardiomyocytes is observed, which is composed mainly of neutrophils, monocytes...

Clinical and manometric investigation in constipated Chagasic patients with and without megacolon
ConclusionThe absence of RAIR confirms the diagnosis of Chagasic colopathy and endorses surgical treatment whenever clinical treatment fails. The presence of the RAIR in patients with positive serology for Chagas disease without megacolon/megarectum might not be due chagasic colopathy and other causes should be considered.

Effectiveness of therapeutic standard concentration barium enema for colonic diverticular bleeding: Preliminary results
ConclusionAlthough we had no statistical evidence, a therapeutic barium enema with a standard concentration as a diagnostic examination may be effective for both the initial hemostasis and preventing the recurrence of colonic diverticular bleeding without complications. Barium Enema Revisited in the Workup for the Diagnosis of Hirschsprung's Disease
Conclusion: Therefore, the high diagnostic sensitivity of TZ identification combined to RSI ≤1.0 reinforces the usefulness of these BE parameters in the screening for Hirschsprung's disease. (Source: Journal of Pediatric Gastroenterology and Nutrition)

Crohn ’s disease in a developing African mission hospital: a case report
ConclusionsCrohn ’s disease is uncommon in Africa, and this entity is encountered sparingly. The signs and symptoms of Crohn’s disease overlap with many other abdominal disorders, such as tuberculosis, ulcerative colitis, irritable bowel syndrome, and others. Several publications in the literature describe that it is difficult to make an accurate diagnosis of this disease, despite the fact that many diagnostic armamentaria are available to suggest its presence. Most of the patients with Crohn’s disease are treated conservatively, and a few may require surgical intervention, especially those presenting wi th complicat...

A Case of Barium Enema –Induced Anal Fistula in a Long-term Care Resident: An Imperative to Avoid Barium Enema as a Screening and Diagnostic Tool in the Elderly
The use of barium enema study has decreased as lower GI endoscopy becomes more prevalent. However, it is still used in several clinical settings given benefits of sedation avoidance, less invasiveness and low cost. Even though barium enema is a relatively safe study, complications may result in rare cases. Colonic perforation is the most frequent serious complication, occurring in 0.02 to 0.04% of patients. Extra peritoneal perforation is usually less catastrophic than intra peritoneal perforation, but may result in pain, abscess, rectal stricture or fistula. Diagnostic Reference Levels for conventional radiography and fluoroscopy in Austria: Results and updated National Diagnostic Reference Levels derived from a nationwide survey
ConclusionThird quartiles derived from the survey are substantially lower than the old Austrian NDRLs (valid till early 2018). Since 3rd quartiles correspond well to European NDRLs, the update would be in accordance with European DRL harmonisation efforts. (Source: European Journal of Radiology)

When Milk isn't the Problem —Carrageenan as a Trigger for Allergic Reactions to Dairy Products
Carrageenan is a thickening or emulsifying agent in food and medicinal products that is derived from red seaweed algae. Adverse reactions to carrageenan have been reported sparingly with barium enema solution, but may also be associated with adverse reactions to food. [Colorectal cancer: role of imaging in screening].
Abstract CLINICAL PROBLEM: Colorectal cancer (CRC) is a major cause of cancer-related morbidity and mortality. Most colorectal cancers derive from benign precursor lesions, so-called adenomatous polyps, over a long period of time. Colorectal cancer screening is based on the detection of precancerous polyps and early stage CRC in asymptomatic individuals to reduce CRC incidence and mortality. The protective effect of screening programs can be improved by increasing the screening rates. PRACTICAL RECOMMENDATIONS: Apart from the established examinations, CT colonography (CTC) has been proposed as an...

A case of carrageenan allergy in a pediatric patient
Carrageenan is a common food additive obtained from Eucheuma, Chrondrus, and Gigartina species of seaweed. It is used in dairy, processed meat, sauces, pharmaceuticals and other food products as a thickener and stabilizer. It's soluble fiber properties are used to alter the consistency, appearance, and nutritional properties of food. Although it has been reported to cause adverse gastrointestinal effects, reports of IgE-mediated allergy appear limited to a case of anaphylaxis to carrageenan-containing barium enema. The usefulness of “Finger-assist” during the laparoscopically assisted anorectal pull through
Publication date: Available online 16 October 2018Source: Journal of Pediatric Surgery Case ReportsAuthor(s): Keigo Yada, Hiroki Mori, Hiroki IshibashiAbstractLaparoscopically assisted anorectal pull-through (LAARP) is one of the surgical procedures for anorectal malformations (ARM) with rectourethral fistula. However, it is sometimes difficult to obtain favorable laparoscopic view and adequate dissection of fistula. To obtain the tactile sensation of surgeons, ‘Finger-assisted’ laparoscopic surgery has been introduced for small numbers of pediatric patients. We developed the ‘Finger-assisted’ technique during LAAR...

Atypical onset of total colonic Hirschsprung disease in a small female infant: A case report
We report the case of a 7-month-old female infant admitted in our clinic for constipation and failure to thrive. At the age of 6 months, she was examined in our clinic for the same reasons, and we recommended symptomatic treatment without improvements. The clinical examination revealed pallor of the skin and mucosa, distended abdomen, and abdominal tenderness at palpation. Diagnoses: The abdominal ultrasound showed abdominal bloating, and the barium enema was normal. The patient's evolution worsened progressively within the following 3 days after admission associating sings of toxic megacolon. Interventions: She unde...
