Cash Procedures at Non-Profit = Holy Grail?
Any experience doing cash procedures at a tax-exempt charity hospital and having the patient and hospital characterize the financial transaction as a "donation" and not a professional service? The patient gets a tax write-off, the hospital receives a tax-free donation, and the doctor gets a piece of the action. Win-win-win. (Source: Student Doctor Network)

Continuing narcotics via telemedicine
I have a patient treated with oral narcotics for cancer pain. They are apparently severely immunocompromised and requesting telemedicine visits. As I understand if I have seen patient in person initially this is an option however the Ryan Haight act is not crystal clear to me. I was planning on seeing in office every three months and telemedicine the months in between. I live in Texas. Is this legal? Should I call the DEA to double check this? Stem cell injections- regenexx vs ROW
I only offer PRP at this time not stem cell. However, I noticed that the regenexx people don’t just perform a single stem cell injection they do a series, so some kind of growth factors/nutrients a week or two before the actual stem cell injection. Is there any significant evidence of benefit for the other injections beside the actual stem cell injections? (I realize that the stem cells themselves are not completely “proven” either for peripheral joint OA) (Source: Student Doctor Network)

ITP for generalized cancer pain?
Correct me if I’m wrong. I do not see how an ITP would likely cover generalized cancer pain. Im getting pressure from higher-ups (I’m employed) to utilize this modality. Does anyone have wild success with this for cancer pain? I’ve had a few cancer patients with ITPs and we’re still taking oral narcotic medications. Appreciate any advise or good literature sources. TIA! (Source: Student Doctor Network)

Review Book for Woke Questions on MOC Exam
I miss the "woke" questions on the MOC multiple-choice exam. Does anyone know if there's a resource like "Woke-ism Made Ridiculously Simple" or something for MOC? My training was before those topics were covered. (Source: Student Doctor Network)

Strike?
Please tell me one reason that doctors can't / don't strike? this is why nurses consistently get paid more for their training than we do. we could bring the system to its knees if doctors took a single day off.... Kaiser Permanente workers say deal unlikely to avert strike The coalition of unions representing healthcare workers at Kaiser Permanente said late on Saturday it is unlikely there will be a new agreement with the healthcare provider, as their current contract expires and the unions prepare for strike action. ...

E/M with procedure
Following scenario: A referral to perform an injection (ESI/TFESI/SIJ etc). from surgeon or external provider was created and the injection was approved/verification of benefits was completed. Patient comes to your office on a separate day for the procedure, can you bill E/M code with modifier 25 on day of procedure along with the procedure code? You are a different speciality (pain/anesthesia) from the surgeon or other referring provider. (Source: Student Doctor Network)

Trials to send referrings
I thought it might be good to have a running thread of recent trials or papers you’ve sent or will send to your referrings to create new referral patterns. I’ll start: Prophylactic Radiation Therapy Versus Standard of Care for Patients With High-Risk Asymptomatic Bone Metastases: A Multicenter, Randomized Phase II Clinical Trial - PubMed Radiation delivered prophylactically to asymptomatic, high-risk bone metastases reduced SRE and hospitalizations. We also observed an improvement in OS with prophylactic radiation, although a confirmat...

Onboarding Curriculum For APCs
Hi all, I’m currently in my third year of practice in an Interventional pain group. We are looking to hire more APCs to our group. I was wondering if anyone out there has any recommendations of resources that we could use that would be a standardized way of teaching/training APCs new to interventional pain. Their responsibility will be entirely clinic (Assessing pts for injections, fuv, and med mgmt). (Source: Student Doctor Network)

Repeat Surgery vs SCS with post surgical foraminal stenosis
I see this a lot Someone has had a back surgery, still has radicular pain Formaminal stenosis on MRI (moderate or severe lets say), at a different level Does not want anymore back surgery, period. No weakness Has failed injections, antineuropathic meds. I typically do not offer SCS. It just seems like the indicated procedure is a nerve decompression, so that is what they should get, not an SCS which is just going to mask the pain. And I dont even know how good SCS is at masking pain... Read more (Source: Student Doctor Network)
