Will pain come back around?
Hi All, Currently resident looking into pain and have been seeing a lot of posts about the current state of pain- slashed reimbursement, saturation in desirable cities, etc. I also have seen a lot about anesthesiologists flocking back to the OR give the insanity of their job prospects atm. As many of these things work sort of sinusoidally, my question is- do we think pain will come back around? Is there a future where older pain docs get fed up with slashed reimbursements and retire en... Read more (Source: Student Doctor Network)

Anesthesia residents that are planning on doing a pain fellowship, what made you want to?
M2 here just considering going into anesthesia and potentially pain, and was wondering what made you want to go into pain since I know the day to day is quite different from general anesthesia. Also, did you always plan on going into pain and you just had to get through anesthesia residency? Or was it something you realized after starting residency. If you always knew you wanted to do pain, was there a reason you went the anesthesia instead of PM&R or other route? Thanks so much for your... Read more (Source: Student Doctor Network)

Oral ketamine
Why isn’t oral ketamine commercially available? I use it, although quite sparingly mostly because it seems experimental at this point. I’m using it primarily for opioid resistant cancer pain, CRPS, neuropathic pain resistant to standard anti-epileptics, etc. I use a compounding pharmacy that makes it into sublingual troche or lozenge. Usually using 25 mg up to 100 mg BID. What are your experiences with oral ketamine? (Source: Student Doctor Network)

2023 Oral Boards
I took the oral boards today. Did solid on 5 cases. I missed the diagnosis on a structured interview case, and the other I stumbled and missed a critical action likely. Also missed minor things like forgetting to give pain meds in one case and stating IV, O2, monitor. I feel like I could have failed. Should I start studying again now? This feels defeating. Post RFA Neuritis
Long time SDN lurker and I appreciate the wisdom of the docs in this forum. I've been out of fellowship about 1 year. Just wanted to get everyones thoughts on Post RFA neuritis (have had a few over the past year). Prescribed some neuropathic agens such as gabapentin vs lyrica vs duloxetine with modest benefit. Haven't found a whole lot of literature on any treatment algorithm. Have spoken to a few attendings about this in the past other options po steroid dose pack, vs doing mbbs at... Expandible probes and lumbar RFA.
Do any of you do lumbar RFA with a direct (MBB approach), using an expandible probe such as venom, sidekick, cooled RFA? If so 1- how well does it work and how long does it last compared to standard SIS technique? 2-other than cooled RFA being the largest, is there any major lesion size and efficacy differences between other expandable probes? 3- where do you aim if you do RFA with an MBB approach. Mostly I want to know if you aim directly at the center of the SP/TP junction or if your... Read more (Source: Student Doctor Network)

Fluoro: flat panel detector vs image intensifier?
Does anyone see a significant advantage with using flat panel detector C-arm vs the old image intensifier unit? Apparently there is an 18k price differential based on what I was quoted, so trying to figure out if the added cost is worth it. The flat panel C-arm has better orbital range and higher resolution, but not sure how this translate to in real life application. I've only used an image intensifier setup, which the only complaint I have is the intensifier unit getting in my face/field... Read more (Source: Student Doctor Network)
