Piriformis Botox?
Does anyone do this? If so, what's your technique? I have a young patient with history of SI fusion with terrible piriformis issues. She gets great relief with piriformis local/steroid injections, but they only last about 3 months and I'm looking for something that will last longer.

Medicare si joint requirements
This probably applies to some commercial as well, as i have gotten information from aetna and certain BCBS out of state plans
For medicare part a & b, do we need to do 2 diagnostic blocks (aka without any steroid usage)? before we can do a therapeutic injection? Or can we actually use steroid in the diagnostic block (the LCD isn't clear)
Thoughts?

Cirrhosis and Local Anesthetics
What considerations do you take when doing injections on cirrhotic patients? Use lido with epinephrine? Any precautions? Tried looking in literature but couldn't find much except to pay careful attention to dosing.

Board certified
I’m almost scared to ask this question.

Hand held ultrasound?
Does anyone have a handheld ultrasound system they recommend? My main use case for ultrasound is essentially piriformis, knee/shoulder, and carpal tunnel injection (in that order of frequency). Ideally I’d need a linear and curved probe to cover superficial and deep structures…even better if unit has both.
I’ve looked online at Clarius, Butterfly IQ, Eagle View, Vue scan but no real world usage. I hate subscriptions which a lot of the companies use to lock in useful features like needle...

intrathecal pump question
one of my few remaining pump patient story.. i need some input from the board
80 something year old i inherited few years ago, i tried weaning from pump but she realized pump was helping her pain significantly.
