dragon
Hi any of you all in PP using something besides drgon. I like that i can pair with me phone and i can have 'smart phrases'. It's always not the best at picking things up correctly. I am paying like $69-99 or so . I just kept with it since i started 2 years. With all this AI talk etc, anyone using something that's better and more cost efficient? (Source: Student Doctor Network)

SCS trials in office (oral sedation/antibiotics)
Due to limited OR availability, will be doing SCS trials in office. Outside of localizing the tract with a 25g spinal needle (0.25% marcaine : 1% lidocaine mix) prior to the Tuohy needle, what PO meds do you guys use for sedation? Was thinking potentially 5-10mg valium, and a 5mg percocet. For antibiotics, doing an IV won't be logistically smooth. Heard about potentially being able to do PO antibiotics before/during trial, instead of the IV ancef. Any recs there? (Source: Student Doctor Network)

May Have Carpal Tunnel...
Hey All! So I finished up with my Chemistry/Biology degrees and I am working on an MBA while working in a Dental office. I’ve been there almost one year. A month ago I had my yearly health screening and I mentioned to my doctor some wrist pain I’ve been having. Not to severe but enough to cause me discomfort while assisting on longer procedures. After some initial testing by my PCP she thinks I may have carpal tunnel... I am going to a specialist next month. I’m scared, nervous, and... Read more (Source: Student Doctor Network)

Cannabis
Georgia has 5% THC tincture for those with cannabis cards. This was rolled out summer '23. I've had...I think 3...pts show up and disclose to me they got a card from their PCP and they've tried it. None have benefitted, and one actually ended up having to be physically carried out of a Cracker Barrel and put into his car by the restaurant staff bc he couldn't move. Literally stuck to the dinner table (80 yo man BTW). I just realized today that cannabis is not on the state's PDMP site... Read more (Source: Student Doctor Network)

PCP Notes
We get a lot of crappy notes from PCPs who have no idea what is going on and what should be done. And then we sometimes (rarely) get remarkable notes. I appreciate the heck out of a doctor who takes the time to T up a patient like this for me. Kudos to Dr. B (in house referral). Cervical spondylosis (M47.812) Current Plans Started traMADoL 50 mg tablet, 1 Tablet every day at bedtime, #30, 01/15/2024, Ref. x1. ASC vs. HOPD - CMS rules
Question: With the Medicare changes regarding prior auth requirements and frequency for basic procedures such as MBB, RFA, ESI, etc., quick question... Is there any difference in performing these procedures in an ASC vs. HOPD facility? The reason I ask is that I currently practice in HOPD facility, but have and opportunity to move my procedures to an nearby ASC. If that would help to avoid some of the CMS LCD requirement nonsense, it would be worth it! Thoughts? Thanks, (Source: Student Doctor Network)

Question about nerve glides for sciatica
Hi all, I've been an aide for two DPTs. The first didn't perform nerve glides at all for patients with sciatica, citing that he found it often made the patient worse. The second DPT is at the opposite end of the spectrum. I've seen him perform nerve glides with all the sciatica patients. All three are in extreme pain when he does them, even the guy who has no sciatica pain, only numbness. One woman literally begs him to stop, another cries, and the guy curses a storm and apologizes... Read more (Source: Student Doctor Network)
