Edible glitter?
Hi all, I moderate over in the pain forums and am scheduled Friday for my 3 year colonoscopy. Hoping to brighten the day of my GI doc and was wondering if edible glitter (maybe a teaspoon) would ruin the scope? It will be our third scope together and I see him regularly in my office (as a colleague) as we have a GI ASC down the hall from my procedure suite. Thanks. (Source: Student Doctor Network)

Dr. Death (again)
Nothing new here. But I just watched the re-enactment of the podcast staring Alec Baldwin and Christian Slater. It was very good and I encourage you all to watch. It is such a fascinating story that leaves the listener/watcher with so many questions. The podcast was not fresh on my mind since I had listened to it years ago. But my own take - it was the drugs couple with narcissism, sprinkled with Dunning- Kruger. Interesting but in the show, a co-medical student said he was the... How do you manage patient expectations?
Every so often, I may get a referral from a PCP as their patient gradually became a chronic opioid user for various conditions I wouldn't necessarily prescribe opioids for. The expectation that I run into often times revolves around the patient coming with the idea they will be getting opioids from me (at equivalent dosing or higher), but unfortunately what they are met with is optimizing non-opioid options along with the offering to down-titrate opioids. Understandably, this is often met... Read more (Source: Student Doctor Network)

Full time position - Norman, OK
Looking to bring on a full time associate. Office based, private practice. You can choose to participate in a local ASC or physician owned hospital in the future if that is of interest to you. You will hit the ground running as I am overbooked. I am personally seeing over 1000 encounters per month. I do medical management (reasonable and this is a shrinking part of the practice) as well as all advanced and basic interventions with the exception of pumps. Income potential is unlimited if you... National Spine and Pain
Looked through the old posts at not much since 2021. Talked to one of the docs in the group about a job opening with NS and P. 20-25 patients a day. Base 300 but you get that or 20% of gross collections, facility fees, pro fees, UDS and DME. Does this make any sense? Anyone currently working with them? (Source: Student Doctor Network)

Why the hell does Medicare not cover bupe?
Got this lovely message today, in regards to an 86 year old woman, whose 2 tramadol per day are not helping anymore: Buprenorphine 5mcg/hr patch - Drug not on formulary - Plan does not cover the medication - We cannot do PA. She provided the preferred alternative: Morphine Sulfate ER tablets and Oxymorphone HCL ER tablets, and she submitted a test claim for both medications, stating that it was paid and that no PA was required. In the midst of the "opioid epidemic" that everyone is... Read more (Source: Student Doctor Network)

Workers compensation
I am currently an employed physician (interventional pain medicine ) at a hospital setting in California. A friend of mine (is a lawyer) does a lot of workers compensation. He sees the patients and sends them out to the doctors he knows. He told me if I started my own practice he could easily refer 75 (if not closer to 100) patients a month. I am trying to understand the sort of compensation I will get from just 75 workers comp patients a month till the rest of the practice picks up... Read more (Source: Student Doctor Network)

RFA frequency
Straight Medicare insurance. I did a medial branch RFA for a patient one year ago at THREE levels (billing told me I could) that worked well, >50% pain relief for over a year. Later came to find out that we did not get reimbursed for the third level. Fast forward a year, I repeat the RFA but only at two levels given our prior denial. Patient reports unsatisfactory pain relief. I looked over the previous and current fluoro images and the needle placement is nearly identical. My question is... Experience from the other side
Today I had a glimpse of what urgent care is like and, maybe, what the PAs/NPs I used to supervise in the ED were up to. (There are some good ones out there, I know. But many are not.) I'm an emergency physician. Retired a few years ago, moved to a new state, so no doctor friends here that I could bug for a quick x-ray. I had a dumb hand injury this morning while trying to get some things done around the house, and by mid-day, there was enough pain and swelling that I gave in and decided to... Recommended RFA setup?
I have an old Stryker Multigen RF generator (the original version) that's dying. I inherited the machine from the previous setup and really hate the interface lag / non-responsiveness. Wondering what is the recommended vendor/RFA setup now? I'm in private practice (paying for this machine) so expense is definitely one of my priorities (no cool RF since it's so costly for supplies). What do you guys use and do you like your setup? (Source: Student Doctor Network)

Buying into ASC
Have a job offer to work with an Ortho group where they are offering buy in to their surgery center. What are the appropriate questions to ask? Never dealt with anything like this before so I appreciate any input! Thanks! (Source: Student Doctor Network)

Regional variation in reimbursement
Working up in the Northeast. A frequent complaint I've heard about the area, particularly the state I am in, is that insurance reimbursement for all medical services (not just pain) is significantly lower here than in other adjacent states, or even other parts of the country. If this is true, is there any resource or way to determine which regions may offer the best rates for service, or is this just a matter of contacting insurance companies directly. While I have a job I can tolerate for... Read more (Source: Student Doctor Network)
