MGMA Compensation/Collections Ratio Qs (data included)
I am middle career, partner (=minimal profit sharing, bought some equity) at medium-sized private practice in MCOL/HCOL. I am on eat-what-you-kill model. Based on this chart, I am >90%-tile in both compensation and collections, although maybe more like >90%-tile comp, >99%-tile coll. When I look at this 2016 MGMA data, my compensation/collections ratio seems low. I am historically taking home ~40% of gross collections. This dataset says comp/coll ratios for 25%-tile/median/75%-tile for All... Read more (Source: Student Doctor Network)

Hospital Employed, Making Sense of Financials
I’m hospital employed doc struggling to understand my net operating income. I have a good sense of revenue (both top line and after-insurance amount paid). I cannot discern my costs. My hospital can only produce for me a list of “charges.” Charges include straightforward things like lidocaine, dressings, etc. It also includes wildly inflated charges like SCS leads, up-charged at a pre-negotiated supply rate (2x-10x, depending on cost). It also charges my department for my time at a... Read more (Source: Student Doctor Network)

Electronic Intake Data Capture in Pain Clinic
We still have patinets fill out paper charts, which is time consuming and not useful since it has to get scanned into the EMR, so not searchable. The patient is giving you the HPI basically in writing. This should be going automatically into your note, IMO, without manual input. Is anyone here doing all electronic data capture on intake and follow up? Are you doing PROs (outcome measures) to see the efficacy of your treatments? One of the departments here hands every patient an ipad in the... Read more (Source: Student Doctor Network)

LSD panic attack public disturbance assaults, 8 years ago. Can I practice medicine?
Nearly 8 years ago, in the first few months of freshman year at 18 years old, I took LSD with dormmates. I had a panic attack. I went around the dorm buildings screaming that I was dying. I assaulted 3 people, a young man and two young women around the dorm buildings by going up to them, aggressively grabbing them, and telling them to call 911. Campus police arrived and restrained me. Over the years I've reflected on the distress, pain or potential trauma I caused the victims including... Read more (Source: Student Doctor Network)

Spine MRI without/with contrast denials
I recently moved out East and have been ordering post-op spine MRIs without and with contrast (as I always have). I am getting pushback from insurance companies and radiologists about this - they want a plain MRI or a different Dx code. First of all, am I missing something regarding ordering post-op spine MRIs with contrast? I though we wanted to discern scar tissue from HNP. Did something change and I don't know about it? Secondly, if it is still recommended to order with contrast... Read more (Source: Student Doctor Network)

Pain versus cardiac fellowship?
I like both pain and cardiac, but can’t decide between the two. I know they are vastly different, but I would just like some extra training after residency and was wondering what attendings/people with more experience than me would advise? Thank you (Source: Student Doctor Network)

TFESI via AP approach?
I'm pain trained. I know there is supra and infraneural and the whole Kambin's triangle bit. I use oblique to get in. However, I am wondering, why aren't TFESIs performed via the AP approach? The spine guys do kyphos and Intracept in AP view. Go down -> touch pedicle -> walk caudad a bit-> turn needle and go cephalad. Obviously one shouldn't go lateral b/c there is no backstop. And one of course shouldn't go medial. What am I missing? I'm just curious. (Source: Student Doctor Network)

SCS psych clearance delays
I've identified patients I'm considering for neuromodulation but have been getting held up by the psych clearance portion - long wait times, insurance non-coverage and patients having to pay out of pocket (even the telehealth options). I'm exploring the idea of hiring a liscensed psychologist as a contractor for the group to expedite the process on a per-eval basis. Anyone have any experience with this and Is a practice allowed cover the cost of the eval? (Source: Student Doctor Network)

OMM/OMT for AOBPMR Written Board
For those who have recently taken the AOBPMR written board, how much OMM/OMT was involved? Was it similar to having to study broad scope of OMM/OMT for COMLEX? I didn’t even think about it until now and realized because it’s the DO board it may have OMM questions, so I checked their website which groups OMM with pain and spine under the category of interventional therapeutics for 25-39% of the exam. What does that even mean :/ I’m wondering if I need to dive into the green Saverese OMT... Read more (Source: Student Doctor Network)

How do you work with non-physicians (NPs/PAs)?
How do you all effectively work with NPs and PAs in your practice? Do you have them see follow ups only? Or initials and follow ups, and have them send you procedures so you can spend most of your time on that? Something else? Benefits and disadvantages of any of the above options?
