The C/L Bashing Thread
I've posted variably in the depths of this forum my disdain for C/L. Applause for Glaucomflecken again. This skit encapsulates the pain of C/L, and when a resident the attendings telling you to just please the primary services and just go do the consult. CT or MRI
I sometimes am not sure whether to get a CT or MRI. I not infrequently see post traumatic acute neck pain patients. I know CTs are more sensitive for acute bony injury but less sensitive for stenosis or cord changes. Often times I’m not sure the best modality to get in these situations Also in any of my post fusion patients who fall and come in with worsening pain, I debate whether to get a CT for better visualization of the hardware and bones or MRI for the soft tissue structures. VA occlusion
Have a patient with moderate cervical stenosis and clear radicular pain. Had planned to do a CESI. His c spine MRI and later CT angiogram revealed a major blockage of the entire left VA as it passes through the neck. He didn’t have any CN deficits when I saw him last month. Thoughts on him ? No acute neurological deficits so I was thinking of sending him to neurology but they have a 2 month wait. Maybe just send him to ER in major city so he can get a same day neurology consult? Leg cramps
Have a patient with moderate lumbosacral stenosis that I did bilateral S1 tfesi about 6 weeks ago with good relief of radic symptoms but consistent cramping throughout legs after the procedure. Encouraged hydration and after two weeks symptoms persisted. EMG was normal, started low dose neurontin without much benefit. Any thoughts on this? More time? Referral to neurology? No thoracic facet procedures per Carelon guidelines
First time I’ve run into this I’m fairly certain the patient has thoracic facet joint pain. However all MBB/facet joint procedures are considered experimental at the thoracic level per the guidelines his insurance uses. 80 y/o s/p cabg 20 years ago. Redo sternotomy for severe Mitral regurgitaiton 15 years ago. Other hx: Bil Fem-pop bypass, h/o stroke, severe COPD and severe pulm htn on ATC O2. Culture of medicine and time off
I’m wondering if this is just something personally engrained in me or if others can relate. I’m in my last year of psychiatry residency, recently had abdominal pain, vomiting and went to the ED. Had elevated LFTs, bili, lipase, WBC. Ended up with a 3 day hospital stay, ERCP with 2 stents placed and a cholecystectomy. Post Cervical mbnb HA/dizziness
I have a patient who I did bilateral c3-5 medial branch blocks approximately 2 weeks ago. Procedure was relatively uncomplicated. He had some dizziness/nausea afterwards which got better by the time I saw him a few days later. 2 days after this visit (6 days ago) he is endorsing intermittent mild to moderate fronto-occipital headaches worse in posterior upper neck with occasional nausea.
