Patient-reported satisfaction with thyroid hormone replacement therapy for subclinical hypothyroidism in older adults: A pooled analysis of individual participant data from two randomized controlled trials
Conclusion These pooled data from two RCTs, showed no major differences in treatment satisfaction between older adults receiving levothyroxine or placebo. This finding has important implications for decision-making regarding initiating levothyroxine treatment for SCH. Our findings generally support refraining from routinely prescribing levothyroxine in older adults with SCH. Cases: Decompensated Heart Failure Secondary to Hypopituitarism in an Elderly Patient
Intern Med. 2024 Apr 23. doi: 10.2169/internalmedicine.3491-24. Online ahead of print.ABSTRACTHypopituitarism is difficult to diagnose because of its non-specific symptoms, especially in the presence of comorbidities. A 77-year-old woman with worsening anorexia and exertional dyspnea was initially diagnosed with decompensated dry cold-type heart failure. Hormonal laboratory tests indicated secondary hypothyroidism as a part of the evaluation of heart failure. Furthermore, pituitary magnetic resonance imaging revealed thickening of the pituitary stalk and a loss of signal intensity in the posterior pituitary, thus suggestin...

Growth impairment in children with atrophic autoimmune thyroiditis and pituitary hyperplasia
ConclusionsThis is the largest pediatric cohort with severe autoimmune primary hypothyroidism without goiter, but with pituitary hyperplasia in which significant growth impairment was the most evident presenting sign. AAT phenotype might be correlated with this specific clinical presentation. In youths with growth impairment, hypothyroidism should always be excluded even in the absence of clear clinical signs of dysthyroidism. (Source: Italian Journal of Pediatrics)

Risks of suboptimal and excessive thyroid hormone replacement across ages
ConclusionSuboptimal and excessive LT4 replacement of the preventable non-communicable disease hypothyroidism requires more focus from the healthcare system and from the global political systems to prevent the personally devastating and socioeconomically challenging consequences. Bone mineral density, turnover, and microarchitecture assessed by second-generation high-resolution peripheral quantitative computed tomography in patients with Sheehan ’s syndrome
ConclusionLow areal BMD (aBMD) is highly prevalent in SS as compared to age- and BMI-matched controls. However, there were no significant differences in bone microarchitectural measurements, except for tibial cortical vBMD, which was lower in adequately treated SS patients. Impact of carbimazole combined with vitamin E on testicular injury induced by experimental hyperthyroidism in adult albino rats: oxidative/inflammatory/apoptotic pathways
This study aimed to evaluate the impact of combining carbimazole (CBZ) with vitamin E (VE) on testicular injury induced by experimental hyperthyroidism in adult albino rats, focusing on oxidative, inflammatory, and apoptotic pathways. In this experimental study, 64 adult male albino Wistar rats were divided into eight groups: Group I (control-untreated), Group II (CBZ-control), Group III (VE-control), Group IV (CBZ + VE-control), Group V (levothyroxine-induced testicular injury), Group VI (levothyroxine + CBZ-treated), Group VII (levothyroxine + VE-treated), and Group VIII (levothyroxine + CBZ + VE-treated). The study was ...

One sip of water with LT-4 supplementation —a key to euthyroidism in Hashimoto’s thyroiditis
ConclusionsThe reduction of water ingestion to a maximum of two sips, which is<50  mL, combined with LT-4 supplementation helps to achieve euthyroidism in HT. In addition, it reduces the L-T4 medication dosage needed to lower TSH serum levels and decreases TPO antibodies in HT. (Source: Endocrine)
