Feasibility of tracking invasive Escherichia coli disease among older adults in a community setting: A prospective observational study
ConclusionDeployment of multiple tracking methods can optimise detection of IED among community-dwelling older adults. Older adults with a history of UTI could be feasibly targeted for a phase 3 vaccine efficacy trial through a network of GPs. Molecular analysis of metallo-beta-lactamase-producing Pseudomonas aeruginosa in Switzerland 2022 –2023
ConclusionsThis study showed that the MBL-PA in Switzerland could be attributed to the wide dissemination of high-risk clones that accounted for most isolates in this study. Although FDC resistance was only found in 8 isolates, MBL carriage was shown to be a major contributor to this phenotype. (Source: European Journal of Clinical Microbiology and Infectious Diseases)

Emergence of an ST1934:KL121 hypervirulent Klebsiella pneumoniae carrying a novel virulence-resistance hybrid plasmid with chromosomal integration of ICEKp1
AbstractTo identify the phenotypic and genomic characteristics ofK. pneumoniae KP43 from bloodstream infection. KP43 was resistant to ticarcillin and tetracycline and was hypervirulent in theGalleria mellonella larvae infection model, positive for string test, and possessed high-level macrophage killing resistance. The hypervirulence phenotype was associated with the chromosome integration of ICEKp1 carryingiroBCDN-iroP,rmpADC, andpeg-344, and a novel plasmid pKP43_vir_amr harboringiutAiucABCD. pKP43_vir_amr was an IncFIB κ/FII virulence-resistance hybrid conjugative plasmid which also carried antibiotic resistance genes....

An unusual case of endotipsitis: persistent infection and migration to the right atrium of a transjugular intrahepatic portosystemic shunt (TIPS)
AbstractEndotipsitis is an underdiagnosed entity mainly because it requires a high initial level of suspicion. It should be considered in the differential diagnosis of persistent bacteremia in the cirrhotic patient with TIPS. Most cases are treated conservatively with a long-term antibiotherapy, due to the impossibility of surgical removal of the TIPS, except in a liver transplant or autopsy. The patient we present had endotipsitis that manifested as persistent bacteremia with thrombosis of the TIPS. Initially, conservative management with intravenous antibiotherapy was performed; however, due to mechanical complications c...

Prevalence of astrovirus and sapovirus among adult oncology patients with acute gastroenteritis using a multiplexed gastrointestinal pathogen PCR panel
ConclusionIn oncology patients with viral gastroenteritis, astrovirus and sapovirus were the causative agents in 18% of the cases. Both viruses were associated with mild disease. Prolonged diarrhea and viral shedding were observed in a few transplant patients. (Source: European Journal of Clinical Microbiology and Infectious Diseases)

Inoculum effect of CTX-M-15, OXA-48, and KPC-2 producing Klebsiella pneumoniae on meropenem and ceftazidime-avibactam efficacy
In this study, 20Klebsiella pneumoniae isolates were tested with different inocula (1 –5 × 105 and 1 –5 × 107 cfu/ml) using broth microdilution methods. The inoculum effect was observed in meropenem against OXA-48 + CTX-M-15- and KPC-2-producing isolates but not with ceftazidime/avibactam. Notably, meropenem exhibited inoculum effect against carbapenemase-producing strains, whereas ceftazidime-avibact am remained effective. We conclude that ceftazidime-avibactam is recommended for high-inoculum infections. (Source: European Journal of Clinical Microbiology and Infectious Diseases)

Preliminary reading of antibiogram by microdilution for clinical isolates in urine culture
ConclusionsA preliminary antibiogram reading may be useful in urine cultures to reduce the delay before targeted antibiotherapy, especially againstEnterobacteriaceae and Gram-positive cocci, but not in cases of carbapenemase-producing NFGNB. Further local studies are warranted to evaluate the usefulness of this approach in relation to resistance rates. (Source: European Journal of Clinical Microbiology and Infectious Diseases)

Population-based genomic surveillance of Pseudomonas aeruginosa causing bloodstream infections in a large Canadian health region
ConclusionAn effective multivalent vaccine consisting of five serotypes (O1, O3, O5, O6, O11) would confer protection to  >  70% of Calgary residents withP. aeruginosa BSIs. This study has provided a unique perspective of the population dynamics over time ofP. aeruginosa STs, clades, and serotypes responsible for BSIs. Clinical outcomes and complications of S53P4 bioactive glass in chronic osteomyelitis and septic non-unions: a retrospective single-center study
ConclusionS53P4 bioactive glass is an effective and safe therapeutic option in the treatment of chronic osteomyelitis and septic non-unions because of its unique antibacterial properties, but also for its ability to generate a growth response in the remaining healthy bone at the bone-glass interface. Accuracy of gastric nodule combined with rapid urease test prediction in diagnosing Helicobacter pylori infection in children
ConclusionsThe findings of this study suggest that gastric nodules, particularly when combined with RUT, can be a valuable predictor ofH. pylori infection in children, offering a simple and feasible alternative to other invasive methods. HPV positivity status in males is related to the acquisition of HPV infection in females in heterosexual couples
ConclusionHusbands ’ HPV positivity increases the burden of non-HR-HPV infection and increases the risk of cervical lesions developing in wives. It is hoped to provide a reference value for cervical cancer prevention in females and HPV vaccination in males. (Source: European Journal of Clinical Microbiology and Infectious Diseases)

Mortality risk factors and fulminant sub-phenotype in anaerobic bacteremia: a 10-year retrospective, multicenter, observational cohort study
ConclusionsThis study identified acute circulatory dysfunction and performance of drainage as independent predictive factors for 30-day AB-related mortality and revealed the existence of a fulminant AB sub-phenotype. Our findings could serve as a practical guide to predict the clinical outcomes of AB. (Source: European Journal of Clinical Microbiology and Infectious Diseases)

Differences in PPD- and mitogen-induced T-cell activation marker expression characterize immunopathology in acute tuberculosis patients
AbstractImpaired T-cell responses to mitogens and high T-cell activation marker (TAM) expression onMycobacterium tuberculosis–specific T-cells characterize immunopathology in patients with tuberculosis (TB). In a study of patients with TB (n = 60) and asymptomatic contacts (controls,n = 37), we found that TB patients had higher CD38+ T-cell proportions specific forM. tuberculosis protein (PPDMtb), yet total proportions of PPDMtb-specific T-cells were comparable. Notably, both activated (CD38+) and total IFN- γ+ T-cells from TB patients had lower mitogen (phytohemagglutinin, PHA)-induced responses. This impaired mitogen ...
