Co-regulation of biofilm formation and antimicrobial resistance in Acinetobacter baumannii: from mechanisms to therapeutic strategies
AbstractIn recent years, multidrug-resistantAcinetobacter baumannii has emerged globally as a major threat to the healthcare system. It is now listed by the World Health Organization as a priority one for the need of new therapeutic agents.A.baumannii has the capacity to develop robust biofilms on biotic and abiotic surfaces. Biofilm development allows these bacteria to resist various environmental stressors, including antibiotics and lack of nutrients or water, which in turn allows the persistence ofA.baumannii in the hospital environment and further outbreaks. Investigation into therapeutic alternatives that will act on ...

Production of OXA-48 carbapenemase acts as an independent risk factor for poor outcome in Klebsiella pneumoniae infection
ConclusionOur matched analysis suggests that the production of OXA-48 carbapenemase acts as an independent risk factor for poor outcome inK. pneumoniae infection as compared to episodes due to non-carbapenemase-producing strains. Increased recognition of Q fever aortitis as a chronic manifestation of Q fever in tropical North Queensland, Australia
We report a series of 5 patients with Q fever aortitis who have presented to our hospital in tropical Australia since 2019. All diagnoses were confirmed with polymerase chain reaction (PCR) testing of aortic tissue. Only one had a previous diagnosis of acute Q fever, and none had classical high-risk exposures that might increase clinical suspicion for the infection. All patients underwent surgery: one died and 3 had significant complications. Q fever aortitis may be underdiagnosed; clinicians should consider testing forCoxiella burnetii in people with aortic pathology in endemic areas. (Source: European Journal of Clinical...

Multicenter interlaboratory study of routine systems for the susceptibility testing of temocillin using a challenge panel of multidrug-resistant strains
AbstractAccurate susceptibility result of temocillin (TMO) is important for treating infections caused by multidrug-resistantEnterobacterales. This multicenter study aimed to investigate the performance of routine temocillin testing assays againstEnterobacterales challenging strains. Forty-seven selected clinical isolates were blindly analyzed by 12 Belgian laboratories using VITEK® 2 (n = 5) and BD Phoenix™ (n = 3) automated systems, ETEST® gradient strip (n = 3), and disk (3 brands) diffusion method (DD;n = 6) for temocillin susceptibility using standardized methodology. Results were interpreted usi...

Core genome multilocus sequence typing of Clostridioides difficile to investigate transmission in the hospital setting
ConclusionAmong clusters determined by cgMLST analysis, roughly 40% included unexplained HA-CDI acquisitions, which may be explained by unidentified epidemiological links, asymptomatic colonization, and/or shared common community reservoirs. The use of DLST, followed by whole genome sequencing analysis, is a promising and cost-effective stepwise approach for the investigation of CDI transmission in the hospital setting. Severe acute herpes virus type 2 primo-infection and its association with anti-type 1 interferon autoantibodies
AbstractHerpes simplex virus type 2 (HSV-2) is a common cause of infection, which is usually self-limited and asymptomatic. A 71-year-old patient with HSV-2 primo-infection developed acute hepatitis and secondary hemophagocytic lymphohistiocytosis. The patient had high levels of autoantibodies against type I interferon (IFN) (> 1000 ng/mL), neutralizing high concentration (10 ng/mL) of both IFN-α and IFN-ω but not IFN-β. Anti-IFN-I auto-antibodies are rarely observed in healthy individuals; however, their prevalence increases in individuals over 70 years of age and have been identified as a cause of some severe vi...

High risk of intestinal colonization with ESBL-producing Escherichia coli among soldiers of military contingents in specific geographic regions
AbstractOne-hundred Polish soldiers of a contingent in Afghanistan in 2019 were screened forEnterobacterales resistant to newer-generationβ-lactams at their departure and return. Seventeen percent were colonized in the gut at the departure, whereas 70% acquired carriage in Afghanistan. The commonest organisms were extended-spectrumβ-lactamase (ESBL)-producingEscherichia coli (ESBL-Ec; 96.6%). All isolates were sequenced and were clonally diverse overall, even within the same sequence type, indicating that independent acquisitions mainly. ESBL-Ec were often multi-drug-resistant. Soldiers stationing in certain regions are ...

Effectiveness of a multimodal intervention to improve blood culture collection in an adult emergency department
AbstractWe investigated the impact of a multimodal intervention to improve the compliance of BC collections as a composite outcome, taking into account both blood volume collected and absence of solitary BC. We performed a quasi-experimental study using a before-after design (5 months for pre- and post-intervention evaluation) in an adult emergency department at a tertiary care hospital that showed that a multimodal intervention was associated with a dramatic increase in the proportion of blood cultures that were collected as recommended per national guidelines, from 17. 3% (328/1896) to 68.9% (744/1080),P < 0.00...

Country-wide expansion of a VIM-1 carbapenemase-producing Klebsiella oxytoca ST145 lineage in Poland, 2009 –2019
ConclusionKoSC has been one of the major VIM producers in Poland, owing largely to clonal expansion of the specificK. oxytoca–In237-like lineage. Its apparently enhanced epidemic potential may create a danger on international scale. (Source: European Journal of Clinical Microbiology and Infectious Diseases)

Extreme resistance to the novel siderophore-cephalosporin cefiderocol in an extensively drug-resistant Klebsiella pneumoniae strain causing fatal pneumonia with sepsis: genomic analysis and synergistic combinations for resistance reversal
AbstractCefiderocol (CFDC) is the first-in-class siderophore-cephalosporin.Klebsiella pneumoniae strain that is extremely resistant to CFDC (MIC: 256 µg/ml) was isolated for the first time in the United Arab Emirates from a patient with pneumonia and sepsis. It belonged to sequence-type 14 (ST14), with a novel core genome ST. Resistance was driven by the co-expression of β-lactamases (blaNDM-1,blaOXA-232 andblaCTX-M-15) and a mutation in catecholate-siderophore receptor, utilized by CFDC to enter the bacterial cell. Synergistic combinations ( β-lactamase inhibitors, aztreonam plus CFDC) re-sensitized the bacteria to CFD...
