In conclusion, mTORC1 signaling contributes to the ISC fate decision, enabling regional control of intestinal cell differentiation in response to nutrition. « Back to Top Reviewing the Development of Senotherapeutics to Treat Aging https://www.fightaging.org/archives/2024/02/reviewing-the-development-of-senotherapeutics-to-treat-aging/ Senescent cells accumulate with age and contribute meaningfully to chronic inflammation and degenerative aging. Destroying these cells produces rapid and sizable reversal of age-related diseases in mice, demonstrating that the presence of senescence cells ...

Phenotypes in Arrhythmogenic Cardiomyopathy
Arrhythmogenic Cardiomyopathy was better known as Arrhythmogenic Right Ventricular Dysplasia or ARVD and sometimes as Arrhythmogenic Right Ventricular Cardiomyopathy or ARVC earlier. When left ventricular [1] and biventricular involvement became increasingly recognized, the terminology has been revised to Arrhythmogenic Cardiomyopathy. A study published in JACC has compared the phenotypic expression and clinical outcomes in patients with arrhythmogenic cardiomyopathy [2]. Of the 446 patients, 44% had arrhythmogenic right ventricular cardiomyopathy, while 23% had arrhythmogenic left ventricular cardiomyopathy. 33% had bive...

Fontan Fenestration: To do or not to do?
Leaving a fenestration in the interatrial septum during a Fontan repair is useful in relieving the central venous congestion when pulmonary blood flow is driven by venous pressure in Fontan repair. Data from a multicenter Pediatric Heart Network has been published in JACC: Advances [1]. Subjects were between 2-6 years at Fontan surgery done between 2010 and 2020 with cardiac catheterization done within 1 year prior to the surgery. Fenestration was done in 465 of the 702 patients. Interestingly placement of fenestration was associated with center and Fontan type – whether it was lateral tunnel or extra cardiac. Shorte...

Fetal aortic valvuloplasty
is considered for fetuses with severe valvar aortic stenosis and echocardiographic features suggesting a risk of progression to hypoplastic left heart syndrome. Though surgical options are available for infants with hypoplastic left heart syndrome, morbidity and mortality are high. So if progression to hypoplastic left heart syndrome can be prevented by fetal aortic valvuloplasty, that would be theoretically a great boon. But centers which offer fetal aortic valvuloplasty are very few and cases suitable for fetal aortic valvuloplasty are quite rare. Hence there is difficulty in attaining good case volumes for optimal proc...

Transforming Chronic Care Management: A Comprehensive, Technology-Driven Approach
The following is a guest article by Kevin Riley, CEO and President at Zyter|TruCare In an era dominated by the complex challenges of chronic diseases, which account for approximately 90% of healthcare costs, a strategic approach to chronic care management (CCM) is essential. This approach should integrate advanced technology with a deep comprehension of healthcare’s fundamental principles, effectively tackling the current challenges and proactively shaping the future of healthcare. Emphasizing patient-centered solutions, this methodology is set to revolutionize the management of chronic diseases, thereby impacting pa...

When conferences become an “ Edutainment Industry ” the specialty of Cardiology leads from front !
Recently , I received two e-mail invite for two major conferences one in India other in Europe. Once upon a time, in 1990s we as fellows used to attend these conference for Rs 1000 (15 Euros/Dollors). Now it is 50-75(5000-7500%times increased) Even few years ago it was affordable. I don’t know how many of us can think to attend such conferences. Definitely not me. What prevents these guys to keep the cost nominal. Certainly, Inflation is not the reason. Air tickets and hotel room tariffs has just raised 50-100%.over the same period if you look closely into the above menu card, Indian tariffs would te...

MTTP as a Mediator of the Benefits of Exercise
MTTP is a longevity-associated gene involved in lipid metabolism and correlated with cardiovascular function. Here, researchers use flies to demonstrate that the fly version of MTTP, called mtp, is involved in the mechanisms by which exercise improves long-term cardiac health. It isn't clear as to how exactly MTTP or mtp is involved in the known set of mechanisms important to the pace of aging and cardiovascular health. That sort of deep dive into establishing connections between cellular processes occurs only after numerous studies have demonstrated an interesting correlation, and even then it is a slow and incremental pr...

Signaling Between Cell Types is Vital to Heart Regeneration
A fair amount of the research and development work aimed at spurring greater regeneration of an injured heart is focused on cardiomyocytes, either by delivering new cells, or by encouraging existing cells to replicate or otherwise better resist the hostile environment following injury. As researchers here point out, regeneration is known to be an intricate dance between multiple different cell populations. Thus the signaling that facilitates coordination between those cell types may prove to be a better target for intervention than any single cell population, and single cell population approaches that have shown promise in...

Digital Health, Menopause, And The $150 Billion Ignorance
The idea of this story came from personal experience. During the past year, I have spent countless hours and a bucketload of money trying to figure out what the heck is wrong with my health. I was feeling worse and worse, having various symptoms, totally inexplicable with my impeccable test results. During this journey, not a single doctor asked or suggested that my symptoms may come from entering perimenopause – the stage of life of women preceding menopause.  I am 47 years old, and as I have learned since then, extremely average in starting to have perimenopause symptoms at the age of 47. Also very average ...

Role of ultrahigh-spatial-resolution photon-counting CT in assessment of coronary artery disease
I had posted about photon counting CT which  converts X-rays directly to electronic signals unlike conventional CT which converts X-rays first to light and then to electronic signals. Direct  conversion allows low energy photons to be differentiated from electronic noise. When data is acquired directly from photons of different energy levels, better contrast-to-noise ratio can be realized. Photon-counting-detector CT also improves spatial resolution. A study published in Radiology, the flagship journal of the  Radiological Society of North America evaluated both in vivo and in vitro coronary stenosis using ultrahigh-sp...

How will you save this critically ill patient? A fundamental and lifesaving ECG interpretation that everyone must recognize instantly.
Written by Pendell MeyersA woman in her 30s called EMS for acute symptoms including near-syncope, nausea, diaphoresis, and abdominal pain. EMS arrived and found her to appear altered, critically ill, and hypotensive. An ECG was performed:What do you think?Extremely wide complex monomorphic rhythm just over 100 bpm. The QRS is so wide andsinusoidal that the only real possibilities left are hyperkalemia or Na channel blockade. Hyperkalemia is by far more common.Indeed, further history revealed two missed dialysis sessions. And of course on exam she has a dialysis fistula.EMS reportedly gave 4 grams of calcium (unknown w...

Whole Roasted Squash With Tomato-Ginger Chickpeas & Za ’atar
I read myself the riot act about 6 months ago, when my cholesterol level reached a new high. My doctor seemed nonplussed, perhaps because my cardiac calcium score was a perfect zero. But I was not happy. Yes, I had lost weight and was exercising, but to be honest, my heart belonged to cheese. And eggs. And ice cream. Something had to change. Breakfast was easy. The whole eggs (which I had been eating almost daily) were replaced by Starbucks Sous Vide Egg whites or oatmeal served with a side of chicken sausage. Lunches were yogurt or soup or vegan bean burritos or salad or tuna or peanut butter. I started snacking on nu...
