Opening for Child Fellowship in my same city - Conflicted
Hey everyone, I'm a PGY3 and there is an opening for a child psych spot at a local university, and I am conflicted. I do enjoy my child outpatient days, particularly teenagers struggling with trauma, eating disorders, depression and anxiety; but I also find it exhausting. And I am not a big fan of autism. Negligence accusation story.

Late onset bipolar
Patient in their 70s. Chronic cannabis user. Chronic depression, insomnia, anxiety, possible narcissistic personality traits. Why is trazodone rarely used for depression?
Why is trazodone rarely utilized as an early-on monotherapy option for depression? You would think it's a solid option with it's low incidence of weight gain and sexual side effects, combined with it's benefits on sleep. Maybe you run into tolerability issues when you dose it above the lower doses used for sleep? I'd be unaware personally, because I self-admittedly rarely use it like this as well. How to avoid scam in GP jobs
Hello this thread is for GPs to raise awareness among them against scam jobs; this thread will be obviously severely attacked by some DSOs, owners and specialists who try to make the GP a cheap, ugly, limited and by all means not rewarding job. This behavior in my opinion has been linked to some of the suicidal cases that I have heard about and knew myself and so please GPs this is a big deal. Billing question
Regarding outpatient codes 99214, 99215. For a 99214 lets say I have a patient with only GAD. Their anxiety is worse from previous visit. I am managing them with an SSRI. Would the fact their anxiety is worse since previous visit be considered 1 or more chronic illness with "exacerbation or progression"? For a 99215 lets say I have a patient with Bipolar I. MDM in 2023 E/M guidelines
In the 2023 guidelines for E+M codes on inpatient, it seems that it's both easier and more difficult to justify a high complexity code. It does however seem that one can justify this on the basis of discussing a medication that requires monitoring, even if it isn't started. In my view, this would mean that discussing, say, the option of an atypical antipsychotic which requires lipid/glucose monitoring in a patient who is manic, psychotic or suicidal, would meet criteria in terms of MDM. I've applied to ~33 schools this year and think I'll need to reapply next year considering how little II I've gotten so far. I'm honestly extremely depressed even though I know I shouldn't take rejections personally. Right now, I'd like to focus on how I can improve my chances for next year. I was just curious how confident you guys feel with the bipolar 2 diagnosis in the outpatient setting. I ask, but usually when they present the history is unclear and often fairly vague with some BPD sx thrown in the mix.
