Managing a difficult psych attending
I’ve gotten helpful responses before, so I thought I’d try again.
Client is mid-fourties’ and female. She was diagnosed 15 years ago by her psychiatrist as having bipolar 1, ocd, and ptsd. This client has taken medication for bipolar for 15 years with significant weight gain and sexual side effects as well as hair loss. Client was recently discharged for depression with suicidal ideation and intent after staying a week on the inpatient unit. This is when I began seeing her.

Need to do a post bac after failing out of my PhD program, suggestions?
So, I failed out of my PhD program a couple years ago (was depressed, didn't get help and spiraled), and afterwards I worked a research job and got a first author pub.
I'm currently considering a post-bac. I was going to do a Masters in a science/data analysis field so I have career prospects in case I'm too irredeemable/can't do med school due to family responsibilities later on, but do you guys have other suggestions? Bupropion and adult ADHD

Another neurotic new grad needing reassurance (inpatient discharges)
Seems there's been a spate of these posts lately - must be the time of year.
I've just started off on an inpatient unit and I'm feeling exactly like I did during intern year - just change afraid to order Tylenol with afraid to discharge patients. I got burned badly during my PGY3 year in which a seemingly low risk patient I advocated to discharge committed suicide within hours of leaving the hospital. Clearly I wasn't the one who officially signed off on that, but it still haunts me.

Involuntary commitment question
Hello. I have a conundrum and I’m trying to figure out if I’m simply misunderstanding the issue or if I’m witnessing wrongdoing.
I work in a government (US federal) hospital as an internist. We have psychiatry staff but they will only admit military patients who are actively suicidal or homicidal. They will not admit nonmilitary patients to their inpatient service for any reason.
What we get very often is that an unfunded/uninsured civilian patient will be brought to the ER by the police...

Old People With Unipolar Depression on Seroquel 800mg
I inherited a dump of a case in the outpatient setting. 74 yo on Seroquel 800 (14 years), Trazodone 200, Prazosin 1mg AM, 2mg HS, Ativan 0.5 bid prn. Has an obviously high tolerance to the sedating effects of her regimen. Unipolar depression per the last treating psychiatrist of years, oh and a and a touch of ptsd. How slowly would you shave off Seroquel? It’s 90% being used as a sleep aid only.
