Sending clients to the ER
I have a client with bipolar1 disorder who went through a depressive phase with suicidal ideation, intent, and behavior. I was gone for two weeks and when I returned, she didn’t tell me even though I always ask. At the beginning of January, she began taking ever increasing doses of a medication that can be lethal. At our appointment Friday, she confessed she had been doing this. Two days prior, she took the highest dose she ever has. She lost the ability to walk and use her legs. Somehow... Thoughts on invol hosp for SI
Hey there guys. We were having this discussion on another thread, so I decided to create this one. What are your real thoughts about the subject? This is much more common in the US than in other countries, and I do believe it is mostly CYA than actual treatment. I have seen some very absurd cases of people being locked up due to "SI" when it was actually not. I actually think it's quite harmful to just throw people in a little jail for a few days when they say they have suicide... Read more (Source: Student Doctor Network)

Redefining Psychotic Depression: Advocating for a Separate Diagnostic Category?

College Psychiatry
What are some pluses and minuses of a gig being a psychiatrist for a college campus? Other than low pay likely. Minuses off the top of my head (and I have a bias against the outpatient world): 1. Likely a fair amount of reading up on adhd and psychostimulant seeking (this is actually in line with my exposure to this cohort w/in a more general adult population and outpatient settings). 2. Higher risk age group for suicide (Source: Student Doctor Network)

I did nothing - can it be made up for?
Title. For over a year after my graduation in 2020 (philosophy BA), I did essentially nothing. I can cite environmental factors, like COVID-19 and depression, or not having a car (or meaningful public transportation where I live), or the fact that I hadn't yet decided to switch tracks, but that doesn't change the fact that I did nothing. Furthermore, after starting a DIY postbacc, I moved at a glacial pace and studied only part-time for a few semesters without major extracurriculars on the... SCS for depression being studied
The Spinal Cord Could Provide a Radical New Way to Treat Depression With depression affecting around 1 in 10 of us at some point during our lives, the need for new and improved treatments is a top priority for researchers – and it appears that spinal cord stimulation could be one route for experts to investigate. www.sciencealert.com (Source: Student Doctor Network)

Unfit for med school???
My MCAT is next week. I can't seem to break a 27 on my FLs, and I am at my wit's end...I have studied so long and hard for this, and I'm just really depressed and discouraged right now. I often think, "How will I EVER make it through med school, much less residency, if I can't even score a measly 30 on the MCAT? Med school is way harder than this..." I know it's unhealthy to think that way, but it's hard not to sometimes. I have a decent GPA (3.7), but this MCAT is killing me, and I feel... Read more (Source: Student Doctor Network)

Totally Demoralized
MCAT is in a few weeks. I'm considering backing out. I'm totally lost in my life right now and none of this seems worth it. Talk about return on investment, I've been in school forever, the whole time I've been depressed, lonely, no friends, nothing. I have no money and I'm trapped. This whole country is a scam. I don't think I have a future. (Source: Student Doctor Network)

AI workflow design thread
What's up muppets. The desire to collaboratively jam on AI-powered workflow automations and design has awoken me from an sdn slumber. I'd read through the seminal private practice threads which are quite inspiring and useful. I've scanned recent AI thread topics and found some lively interesting commentary. But besides @SmallBird 's idea about creating behavioral activation handout for depression and maybe a few ideas about clinical uses, I didn't see too many specific use case... Auvelity, stuff not in the textbooks
I've been using it now on patients for months with shockingly good results. The manufacturer alleges 70-80% efficacy with 70-80% going into complete remission with benefits possibly starting day 1. I've seen this happen in front of my eyes with patients taking it and literally 1-2 hours later feeling tremendously better from even severe depression. I'm posting here because several doctors will not prescribe a brand new med because there may be October surprises with the med with it being... Read more (Source: Student Doctor Network)

How do you manage anxiety about malpractice?
I have a colleague who just had a bad outcome with a patient and it just makes me so nervous. I work in a high litigation state with no protections. I have multiple borderline patients. How do you guys deal with it? Suicide rates going up. I'm barely out of training. No one really talks about this. Everyone seems terrified. Everyone tells you to document well, but what happens if you get sued? Do you go broke? Is your career over? No one wants to talk about this. How do you guys deal with it?
