bipolar affective disorder terminology
Can someone please help? This is driving me nuts. Ocassionally on charts I will see "bipolar affective disorder" or "manic depression." Is this an old term from DSM 3 or something? Or a current one from ICD-10? Does it more closely resemble schizoaffective disorder or is it just bipolar? Does this term just mean "bipolar" with a few extra words thrown in before and after? Or is like here is "bipolar" and "bipolar affective disorder" like how we use bipolar 1 and bipolar 2? (Source: Student Doctor Network)

Phq-9 ?
I've noticed, and maybe this isn't anything too new, a trend towards more of a push to use the PHQ-9 as a standardized screener and quantitative way of measuring patient improvement/outcomes. Or diagnosing depression in patients. But what concerns me is seeing it used in facilities and settings that serve populations where a number of the PHQ-9 symptom items can easily be symptoms of the populations' common medical conditions. For example, why use the PHQ-9 used in facilities where... Read more (Source: Student Doctor Network)

Therapeutic dilemma in a female 72years old patient with Schizophrenia
I was referred a female schizophrenic 72 years old patient. She does not have any positive symptoms right now. But she has been having mixed depressive/anxiety symptoms with anxiety being the prevalent one - starting with the covid19 pandemic. She is afraid to leave the house (she used to every day) and she is afraid to do anything without the presence of his son, who lives with her (she is scared she might fall - she complains of dizziness, or something bad happens to her... Cerebral but make it ketamine??
I just got an ad for this company on Instagram with tons of clients raving about their experience in the comments: Mindbloom | Psychedelic Medicine is Here Mindbloom helps you heal and grow through legal, safe, and clinician-guided ketamine therapy for depression and anxiety. See if you're a candidate today. www.mindbloom.com I had no idea that at-home ketamine therapy was even legal but I looked it up and was really surprised by how little regulation there is. This service is...

Geri psych turf
So, I'm in my last year of psych residency, and deciding on doing an additional geri psych year of training (had a very weak rotation on geri psych, but was deeply interested by it). I know what most people say about fellowships in psych: the only truly necessary seem to be child and forensic, but I'm very interested in the topics covered by geri psych - cognition and BPDS, depression, end-of-life and all the existential problems of old age, the overlap with neuro/IM that sometimes come into... Remimazolam vs Propofol
https://bmcanesthesiol.biomedcentral.com/track/pdf/10.1186/s12871-022-01713-6.pdf Interesting trial. In my opinion, as long as respiratory depression is not 0, you still need an anesthesiologist administering. Could be a useful drug, but the lack of time difference when compared to propofol means it'll just be an expensive analogue. Data from 77 patients were analysed. The success rate of sedation in both groups was 100%. The time to LOC (MOAA/S score ≤ 1) in the RT group was... Click to expand... Read more (Source: Student Doctor Network)

Seeking Advice For Next Cycle
Hello All, I'm looking to apply next cycle, since I am in the process of fixing myself from my undergrad grind. I'm still not done, but I plan on taking my last 2 semesters slowly ( part-time ). I got a MCAT score that I somewhat expected, since I picked a poor time to take it. I am considering a retake of my MCAT vs just applying to DO schools, but I would like to avoid taking CASPER and other tests. My main deficiency during my MCAT prep was caused by depression from loneliness, which... Read more (Source: Student Doctor Network)

Psychotherapy doesn ’t improve depression outcomes?
Hi all. I’m wondering what you guys make of the research i’ve linked below. To quote the medscape article: “Results of a cross-sectional, naturalistic, multicenter European study showed there were no significant differences in response rates between patients with major depressive disorder (MDD) who received combination treatment with psychotherapy and antidepressant medication in comparison with those who received antidepressant monotherapy” Medscape article: No Added Benefit of... Adversity/Challenge secondary prompt idea help !
Hi SDN, I just wanted to know your opinions on my potential adversity/challenge essay topics. Which of these would be a good topic for the prompt? Adversity/Challenge: - severe depression in Junior Fall: had suicidal ideation, decided to take a 0 on a genetics exam and left for home to recover, realized the importance of self-care and mental health. I am concerned that Adcoms might see me as irresponsible for not caring about grades or weak for not being able to tough it out (which I... Read more (Source: Student Doctor Network)

How treatable, in general, is high-functioning autism spectrum disorder?
At least compared to psych conditions usually considered "treatable," such as ADHD or depression, and psych conditions usually considered "untreatable," such as any personality disorder besides borderline personality disorder. (Source: Student Doctor Network)

Work set up to "retire" from day 1
So this is probably a bit philosophical, but I've been thinking a lot lately. What is the purpose of life? Is it to wake up each morning and go to work, deal with all the hassles there, then come home and engage in the daily routine of chores only to repeat it day after day? Seems we generally fill our time with things we'd rather not be doing with the occasional pleasurable bits interspersed. Fwiw I'm not depressed, I enjoy living, I'm just becoming increasingly disillusioned by society. I... Read more (Source: Student Doctor Network)

Antidepressants when not clinically depressed
I recently came an old book, Listening to Prozac by Peter Kramer, and found it interesting and wanted to get some opinions from practicing psychiatrists. Do you think that, hypothetically, if someone who does not meet criteria for MDD, GAD, or any other psychiatric disorder started an SSRI, could this confer some benefit to them such as resilience to everyday stresses, increased sociability/confidence, decreased overall neuroticism, or "feeling better than well" as Kramer puts it? Are there... Read more (Source: Student Doctor Network)

Hope and perseverance
Hi everyone! I have no issues but a brief story about hope and perseverance which might help those who are in a similar situation either AMG or IMG. I am a Non-US IMG who graduated in 2013. During a government required 1 year internship, I started to prep for my Step 1. I studied hard for it but ended up failing. I was depressed and angry at the results but determined. I started the process again, doubling the study hours, putting myself under more stress and completely isolating myself... Read more (Source: Student Doctor Network)
