Worth applying to dental school?
Would appreciate some honest input. Bullet points might be a bit more digestible: - Finished bachelors degree in 2013 and matriculated to a US MD school in 2014. - Passed first block of courses on pass/fail curriculum. - Dropped out mid second block, transcript shows "withdrawal." - Dropping out motivated by depression, not coping well with moving away from home, recent break-up, and dreading the 7+ year path ahead of me. - Took off 2 years to work and then went back to school for a... panic disorder, major depression, anxiety disorder, brief psychotic disorder, tbi, ptsd?
I think this might qualify for the /awfuleverything subreddit. The circumstances sound terrible for everyone, obviously the soldiers killing and being killed, their families and communities, the folks trying to help. Long distance remote artillery fire can't be far away. A Secret War, Strange New Wounds and Silence From the Pentagon Many U.S. troops who fired vast numbers of artillery rounds against the Islamic State developed mysterious, life-shattering mental and physical problems. But the military struggled to understand what was wrong...

Weird case. Treatment Resistant Depression but after a snake bite
Got a patient that was already seen by a great clinician and she couldn't get this patient better. Was tried on all the conventional meds. A few SSRIs, SNRIs, Lamotrigine, Lithium, atypical antipsychotics (and yes the better ones for depression such as Aripiprazole, Cariprazine) , Bupropion, augmentation agents such as Buspirone, Auvelity, Esketamine, TMS (not ECT...yet). After several meetings she finally said something that hit me like a ton of bricks. She told me she never had mental... Read more (Source: Student Doctor Network)

What psychiatric illness/disorder would you say has the worst overall prognosis/outcomes?
The research on this seems to show that it largely depends on the metric you use to measure outcomes (e.g., suicide rate, overall mortality, self-related QOL, functioning [and what domain of functioning], disability-adjusted life years [I have serious issues with that concept, but that's another topic], time without hospitalization, etc), so I'm curious, based on the overall gestalt of "outcomes" what disorder(s) would you consider to have the worst outcomes and why? Expert witness question about life span prediction and lawsuit valuation
I had a recent call with a plaintiff attorney representing family of mid 20s female with schizophrenia who committed suicide while inpatient. They asked if I would be able to refute defense expert's claims that the patient was high risk and had a significantly reduced life expectancy on the order of less than 5 years due schizophrenia and history of suicide attempts. Plaintiff attorney said this prediction was based on a sophisticated predictive model of people with severe mental illness... Read more (Source: Student Doctor Network)

I don ’t do anything outside of school really, am I screwed when it’s time to apply for residencies?
TL;DR—end of M2 (core clinical year) at a T20 without much productivity outside of the classroom. Interested in Neuro or IM or FM. I’m most interested in Neuro, IM, FM, or MAYBE anesthesia but I’ve only spent 4 days on that so idk. Anyway, outside of school, I generally just go home and chill. I don’t mean to make this too personal, but at the end of M1 I came out to my mom as gay and it caused a huge stir. She threatened suicide, told me I am a disgrace, and I had to move out pretty... The challenge of managing chronically depressed patients
I have a few patients, usually females in their 40s or 50s, whom I inherited from past psychiatrists with a history of persistant and VERY impactful depressive symptoms (HAMD usually >30), with chronic suicidal ideation and almost no functionality in the real world. The way I'm trying to approach these cases is: 1) Review the diagnosis, looking for secondary causes that may have been missed, and other psychiatric diagnosis such as personality disorders, OCD, PTSD, etc...no luck thus far...
