Cemented vs cementless stems for revision arthroplasties due to Vancouver B2 periprosthetic hip fracture
ConclusionCemented fixation in stem revision due to Vancouver B2 periprosthetic hip fracture could be an optimal option with faster recovery which could decrease the rate of complications and reintervention, without compromising the fracture healing and patient mortality. Thus, this option can be considered when an anatomical reduction can be obtained, especially in elderly patients with multiple comorbidities in which a less aggressive surgical option should be considered. Morbidity and Mortality Following Total Hip and Knee Arthroplasty with Spinal versus General Anesthesia: A Retrospective Analysis
Despite the longstanding debate surrounding anesthesia in total joint arthroplasty, an optimal anesthetic mode has yet to be clearly identified. Patients undergoing total joint arthroplasty may be offered spinal anesthesia (SA) or general anesthesia (GA). While the arthroplasty literature has reported differences in postoperative morbidity, the hip fracture literature does not demonstrate the superiority of SA or GA. The purpose of this study was to further investigate this relationship and determine if there is a significant difference in morbidity and mortality between patients undergoing SA compared with those undergoin...

Worry as Important "Feelers" in Clinical Anesthesia Practice: A Mixed-Methods Study
CONCLUSIONS: While a clear definition of worry could not be given, it is a valuable element of anesthesia practice as it serves as a catalyst for critical thinking, problem-solving, clinical reasoning, and decision-making. Use of the feeling of worry alongside technological systems to make an informed decision is crucial. Technology has significantly improved the ability of health care providers to detect and respond to patient deterioration promptly, but it is crucial for nurse anesthetists to use their feeling of worry or intuition alongside technological systems and evidence-based practice to ensure quick assessments or...

Development of Efficient Synthetic Reactions Using Enamines and Enamides Carrying Oxygen Atom Substituent on Nitrogen Atom
Chem Pharm Bull (Tokyo). 2024;72(5):432-453. doi: 10.1248/cpb.c23-00915.ABSTRACTWe have developed efficient synthetic reactions using enamines and enamides carrying oxygen atom substituent on nitrogen, such as N-alkoxyenamines, N,α-dialkoxyenamines, N-alkoxyanamides, and N-(benzoyloxy)enamides. The umpolung reaction by polarity inversion at the β-position of N-alkoxyenamines afforded α-alkyl-, α-aryl-, α-alkenyl-, and α-heteroarylketones by using aluminum reagent as nucleophiles. Furthermore, one-pot umpolung α-phenylation of ketones has been also developed. We applied this method to umpolung reaction of N,α-dialko...

Qualitative transformations of street-seized ecstasy over a decade: A case study in Rio de Janeiro (Brazil)
This study presents the chemical fingerprinting of ecstasy tablets seized in the state of Rio de Janeiro (Brazil) between 2012 and 2021. The tablet samples were weighed, extracted, diluted with methanol, and acidified before analysis using gas chromatography high-resolution mass spectrometry and attenuated total reflection Fourier transform infrared spectroscopy. The major constituents found were MDMA and clobenzorex, with fewer occurrences of MDA, MDEA, and 2C-B. The results also indicate that the occurrence of mega-events in the study location impacted the chemical fingerprints of ecstasy. A total of 27 combinations of c...

Ultrasound-guided erector spinae nerve block for relief of chest pain from pneumonia in the emergency department
We present the case of a 58-year-old male presenting to the ED with chest pain from pneumonia which remained unmanageable after acetaminophen, NSAID, and opioid administration. An ultrasound-guided erector spinae plane block was performed in the ED and the patient had a significant reduction in his chest pain.PMID:38693022 | DOI:10.1016/j.ajem.2024.04.026 (Source: Herpes)
