An unremarkable midesophageal 4-chamber TEE view in a hemodynamically unstable patient: what are we missing?
A 78-year-old, 109-kg, 160-cm woman with aortic valve stenosis and severe peripheral vascular disease presented to the authors ’ institution for increasing shortness of breath. The physical examination indicated that a previously noted systolic murmur was more prominent. Transthoracic echocardiography revealed a peak velocity of 4.4 m/sec across the aortic valve, consistent with severe stenosis. Evaluation of the efficacy of a novel dermatotomy device for central venous cannulation.
Central venous cannulation (CVC) is one of the most common medical procedures performed in the United States.1 The Seldinger technique, described in 1953, has been widely used for CVC.2 To accomplish CVC using the Seldinger technique, a small dermatotomy is required at the wire entry site to facilitate skin dilatation and subsequent catheter insertion over the guidewire. A precise dermatotomy incision facilitates successful cannulation and minimizes bleeding at the insertion site. Collateral-Induced Hypoxemia After Bidirectional Glenn Procedure
A 5-year-old boy [weight:15.6kg; height: 112cm] presented to the author's institution with progressive shortness of breath and central cyanosis [arterial oxygen saturation of 76% on room air]. As a neonate, the child was diagnosed with normal situs, pulmonary atresia, and multiple ventricular septal defects. A stent was deployed into the ductus arteriosus as an emergent procedure. At 15 months of age, the child underwent a bidirectional Glenn procedure with azygos vein ligation and division of the distal part of the ductus arteriosus stent. Surgical capacity is disaster preparedness: A scoping review of how surgery and anesthesiology departments responded to COVID-19
CONCLUSION: Our review reflects the mechanisms by which hospital surgical systems responded to the initial stress of the COVID-19 pandemic and reinforced the many changes to hospital policy that occurred in the pandemic. Healthcare systems with robust surgical systems were better able to cope with the initial stress of the COVID-19 pandemic. The well-resourced health systems of HICs reported rapid and dynamic changes by providers to assist in and ultimately improve the care of patients during the pandemic.
