Man with periumbilical redness and umbilical drainage
Clinical introduction A 42-year-old man presented with periumbilical pain and a draining umbilicus. A day prior to admission, he experienced periumbilical redness and pain. His vital signs were normal. On examination, periumbilical redness, tenderness and a positive Carnett’s sign were observed (figure 1). Despite inflammation (C reactive protein, 2.5 mg/L), laboratory investigations and urinalysis yielded no notable findings. Contrast-enhanced CT was performed. He was prescribed cefaclor. Question What is the diagnosis? Diverticulitis Cellulitis Metastatic carcinoma Urachal remnant abscess Answer: D Contrast-enhance...

Concurrent urachal abscess and florid cystitis glandularis masquerading as malignancy: a case report and literature review
The urachus is the embryological remnant of the cloaca and allantois. Failure of its regression can cause diseases any time after birth. It is difficult to differentiate an abscess from urachal adenocarcinoma ... (Source: BMC Surgery)

Surgery of the Equine Urinary Tract
Urinary surgery in the horse may be challenging. More straightforward procedures, such as urinary bladder or urachal defects, do not usually require specialized equipment or imaging, although laboratory work is helpful. Congenital or acquired conditions of the ureters or kidneys may necessitate advanced diagnostic work-ups including advanced imaging /or and minimally invasive procedures. Some surgery of the lower urinary tract is done in the sedated, standing adult horse. Surgery involving the kidney typically requires general anesthesia. Laparoscopy and associated tools are frequently used. Although many of the surgical p...

Robot-Assisted Partial Cystectomy versus Open Partial Cystectomy for Patients with Urachal Cancer
Conclusion: Both RAPC and OPC seem feasible surgical modalities to treat localized UrC with comparable survival. The PSR rate of 25% after RAPC should prompt us to be cautious to recommend RAPC as no such recurrences were seen using OPC.Urol Int (Source: Urologia Internationalis)

Cancers, Vol. 14, Pages 615: Immune Microenvironment Characteristics of Urachal Carcinoma and Its Implications for Prognosis and Immunotherapy
g Muyan Cai Urachal carcinoma (UrC) is an exceedingly rare tumor and lacks effective treatment. Herein, we characterized an immune microenvironment characteristic of UrC in detail and identified its implications for prognosis and immunotherapy. In total, 37 resections of UrC were stained for CD20, CD3, CD4, CD8, FOXP3, CD68, HLA-DR, CD163, PD1, and PD-L1, as well as mismatch repair protein including MSH2, MSH6, MLH1, and PMS2 by immunohistochemistry. Intratumoral and peritumoral immune cell densities or the proportion of PD1 and PD-L1 expression alongside MSH2, MSH6, MLH1, and PMS2 status were manually evaluated usin...

A 67 éves férfi anamnézisében ismétlődő húgyúti fertőzés szerepelt ciprofloxacinterápiával. Cisztoszkópia során a húgyhólyag felső részének másodlagos érintettségére utaló bullosus nyálkahártya képe mutatkozott. Az ezt követő CT-felvételek alapján az alsó medián hasfallal, húgyhólyaggal, belekkel összefüggő tumor iránydiagnózisát állították fel. Urachal mucinous cystic tumor of low malignant potential in a polymorbid female: a case report and review of the literature
We present a 43  year old female with a nuanced constellation of comorbidities and confirmed MCTLMP following a workup for abdominal pain and irritative lower urinary tract symptoms. Notably, this tumor did not change in size over a 3-year course of serial imaging prior to surgical excision. This urachal MCTLMP re presents roughly the 26th and one of the smallest of its subtype reported in the literature. This case illustrates the diagnosis and management of this rare urachal MCTLMP. Individual patient medical history, clinical considerations, and neoplasm characteristics are examined. Although rare, the pot ential for i...

Omphalitis with Umbilical Abscess in an Adult with a Urachal Remnant
Omphalitis is an inflammation of the umbilicus and is seldom diagnosed in adults. It is even rarer when it results from an infection of the urachus, an embryological remnant that connects the umbilicus to the dome of the bladder. Patients with omphalitis present with erythema, edema, tenderness, and purulent discharge form the umbilical stump. Workup includes ultrasonography and CT scan of the abdomen and pelvis. Management consists of antibiotics and incision and drainage of the umbilical abscess, followed by surgical resection of the urachal remnant. In this article, we report a case of omphalitis complicated by umbilica...

A rare case of urachal carcinoma with multiple lung metastasis that required differentiation from primary lung carcinoma
We describe a case of multiple metastatic lung cancer from urachal carcinoma that required differentiation from primary lung cancer. AbstractUrachal carcinoma is a rare malignancy of all bladder carcinomas. Metastatic lung tumours showing multiple nodules are rare without a local recurrence. We describe a case of multiple metastatic lung cancer from urachal carcinoma that required differentiation from primary lung cancer. (Source: Respirology Case Reports)

Case of Infected Urachal Cyst in an Adult Presenting as an Incarcerated Umbilical Hernia
Am Surg. 2021 Dec 6:31348211060434. doi: 10.1177/00031348211060434. Online ahead of print.ABSTRACTThe urachus is a remnant of the allantois. Failure to obliterate can result in one of four anomalies, urachal cyst being most common. Urachal cysts are relatively rare, especially in adults. This paper presents a patient with an umbilical hernia and a ruptured urachal cyst. A 39-year-old male presented with concern for umbilical hernia, but he also noted drainage. Computed tomography scan showed a urachal cyst and umbilical hernia. The urachal cyst was excised and umbilical hernia closed primarily. The incidence of an urachal ...
