Anesthetic management of airway stent placement by rigid bronchoscopy with superior laryngeal nerve block while preserving spontaneous breathing: A case report
Clin Case Rep. 2023 Nov 19;11(11):e8232. doi: 10.1002/ccr3.8232. eCollection 2023 Nov.ABSTRACTKEY CLINICAL MESSAGE: The combination of superior laryngeal nerve block can reduce the respiratory depression that occurs during management under total intravenous anesthesia.ABSTRACT: Anesthetic management of endobronchial stent placement by rigid bronchoscopy requires the maintenance of spontaneous breathing while suppressing upper airway reflexes. The combination of superior laryngeal nerve block (SLNB) can reduce the respiratory depression that occurs during management under total intravenous anesthesia. The patient was diagno...

Functional and nonfunctional parathyroid carcinoma: two case reports and literature review
AbstractParathyroid carcinoma (PC) is a rare malignant endocrine tumor. It can be divided into functional and non-functional types according to the tumor ’s ability to secrete parathyroid hormone. Herein, we present a case each of functional and nonfunctional PC. Case 1: Functional PC. The main clinical symptoms were high parathyroid hormone and hypercalcemia with bone injury and other complications. The mass was large, capsulated, and showed vascu lar invasion. The tumor was surgically removed, along with a part of the tracheal wall and recurrent laryngeal nerve that were invaded by the tumor. The ipsilateral and isthmu...

Man with progressive dyspnoea
Clinical introduction A 74-year-old man presented to the ED with progressively worsening dyspnoea for 10 days. The patient was diagnosed with small cell lung carcinoma (stage IVb) 2 months prior to presentation but declined intervention. He experienced intermittent barking cough, hoarseness and haemoptysis without fever, and later developed dyspnoea. Physical examination revealed mild expiratory stridor, and laryngoscopy showed right vocal cord palsy without upper airway obstruction. Laboratory test results were unremarkable except for anaemia. Chest radiography was performed (figure 1). Question What is the most likely di...

Ibuprofen inhibits anaplastic thyroid cells in vivo and in vitro by triggering NLRP3-ASC-GSDMD-dependent pyroptosis
In this study, we demonstrated in vitro and in vivo that ibuprofen can induce ATC pyroptosis. Hence, we treated C643 and OCUT-2C ATC cells with ibuprofen and found that several dying cells presented the characteristic morphological features of pyroptosis, such as bubble-like swell ing and membrane rupture, accompanied by activation of ASC and NLRP3 and cleavage of GSDMD. Along with the increased release of LDH, ibuprofen treatment promoted apoptosis and inhibited viability, invasion, and migration. However, overexpression of GSDMD significantly inhibited ibuprofen-induced pyr optosis. In vivo, research has demonstrated tha...

Long ‐term follow‐up of unresectable adenoid cystic carcinoma of the trachea and bronchus treated with high‐dose proton beam therapy: A report of two cases
This report showed two cases of unresectable adenoid cystic carcinoma of the trachea and bronchus that received definitive proton beam therapy with concurrent chemotherapy. Proton beam therapy can deliver high doses (74  Gy [relative biological effectiveness]) safely to unresectable adenoid cystic carcinoma tumors and may contribute to long-term local control and overall survival. AbstractAdenoid cystic carcinoma (ACC) of the trachea is a rare disease that is slow growing and has a risk of distant metastasis. The standard treatment for ACC of the trachea is surgery, but this tumor is often unresectable. In definitive rad...

Reply: Reported experience of full-circumferential tracheal replacement with cartilage-reinforced forearm free flaps
We thank Sarsam and colleagues1 for their comments about our article2 reporting our experience of extended resection for bronchial and tracheal adenoid cystic carcinoma (ACC). In this correspondence addressing our article, they were concerned by the outcomes after full-circumferential tracheal replacement (FCTR) with cartilage-reinforced forearm free flaps. Targeting PD-1/PD-L1 inhibits rejection in a heterotopic tracheal allograft model of lung transplantation
Immune checkpoint molecules such as programmed death-1 (PD-1) and programmed death ligand-1 (PD-L1) have revolutionized the field of lung cancer treatment. As part of our study, we examined the role of these proteins in acute rejection in a mouse model of heterotopic tracheal transplantation. Recipient mice were untreated (Allo group) or treated with anti-PD-L1 (aPDL1 group) or PD-L1 Fc recombinant protein (PD-L1 Fc group). A further group of C57BL/6 mice received isografts (Iso group). The occlusion rate was significantly higher in the Allo group than in the Iso group (p = 0.0075), and also higher in the aPD-L1 group (p =...

CT-based quantification of trachea shape to detect invasion by thyroid cancer
ConclusionsGeometric moments and bounding shape features can quantify the tracheal shape and are reliable for identifying DTC tracheal invasion. The selected features quantified the extent of tracheal deformity in DTC patients with and without tracheal invasion.Clinical relevance statementSix geometric features provide a non-invasive, semi-automated evaluation of the tracheal invasion status of thyroid cancer.Key Points• A novel method for quantifying tracheal shape using 56 geometric moments and 60 bounding shape features was developed.• Six features identify tracheal invasion by thyroid carcinoma.• The selected fea...

Clinicopathological characteristics and survival outcomes in patients with advanced esophageal squamous cell carcinoma who were intraoperatively diagnosed non-curative
CONCLUSIONS: T4b tumor located in aortobronchial constriction or trachea/bronchus makes it difficult to diagnose whether it is resectable or unresectable. Moreover, surgical procedures and perioperative treatment were deeply associated with the clinical outcomes.PMID:37926097 | DOI:10.1159/000533772 (Source: Oncology)
