Management of medical emergency patients in Danish emergency departments
CONCLUSION: Management of MEP varies greatly in Danish Hospitals. A systematic approach to MEP management in line with management of trauma and stroke patients may potentially serve to improve the quality of care for and outcome of this patient group, but further research is needed.FUNDING: None.TRIAL REGISTRATION: Not relevant.PMID:37622648

What Prohibition Can Teach Us About Drug and Alcohol Policy Today
It’s widely understood today that drinking while pregnant is harmful for the fetus. But the link between alcohol and the health of infants wasn’t as well known in the 1930s, when prohibition was repealed in the U.S. and all sorts of people, pregnant women included, began drinking again.Because prohibition was lifted on a piecemeal basis across the U.S., some counties continued to prohibit alcohol, or stay “dry,” while their neighboring counties were “wet.” Those conditions created what economists call a natural experiment, and made it possible to track the health impacts of maternal drin...

Ability of the DANCAMI to predict the risk ischemic stroke and mortality in patients with atrial fibrillation/flutter
Comparison of the danish comorbidity index for acute myocardial infarction (DANCAMI), the charlson comorbidity index (CCI), the elixhauser comorbidity index (ECI), and the CHA2DS2-VASc score to predict ischemic stroke, cardiovascular mortality, and all-cause mortality after atrial fibrillation/flutter.

Long-Term Imaging and Clinical Outcomes of Surgical Left Atrial Appendage Occlusion With AtriClip
Surgical left atrial appendage (LAA) occlusion with an AtriClip (AtriCure, West Chester, Ohio) is frequently performed for stroke prophylaxis in patients with atrial fibrillation (AF). We conducted a retrospective analysis of all patients with long-standing persistent AF who underwent hybrid convergent ablation and LAA clipping. Contrast-enhanced cardiac computed tomography was performed at 3 to 6  months after LAA clipping to assess the degree of complete closure and the residual LAA stump. A total of 78 patients (64 ± 10 years, 72% male) underwent LAA clipping as part of hybrid convergent AF ablation, from 2019 to 2020.

Associations between emergency call stroke triage and pre-hospital delay, primary hospital admission, and acute reperfusion treatment among early comers with acute ischemic stroke
AbstractTo investigate the association between the Emergency Medical Service dispatcher ’s initial stroke triage and prehospital stroke management, primary admission to hospitals offering revascularization treatment, prehospital time delay, and rate of acute revascularization. In an observational cohort study, patients with acute ischemic stroke (AIS) in Denmark (2017–2018) were in cluded if the emergency call to the Emergency Medical Dispatch Center (EMDC) was made within three hours after symptom onset. Among 3546 included AIS patients, the EMS dispatcher identified 74.6% (95% confidence interval (CI) 73.1–76.0) co...

Migraine and risk of premature myocardial infarction and stroke among men and women: A Danish population-based cohort study
by Cecilia Hvitfeldt Fuglsang, Lars Pedersen, Morten Schmidt, Jan P. Vandenbroucke, Hans Erik B øtker, Henrik Toft Sørensen BackgroundMigraine carries risk of myocardial infarction (MI) and stroke. The risk of premature MI (i.e., among young adults) and stroke differs between men and women; previous studies indicate that migraine is mainly associated with an increased risk of stroke among young women. The aim of this study was to examine impact of migraine on the risk of premature (age ≤60 years) MI and ischemic/hemorrhagic stroke among men and women. Methods and findingsUsing Danish medical registries, we conducted a ...

Trends in low-density lipoprotein cholesterol goal achievement and changes in lipid-lowering therapy after incident atherosclerotic cardiovascular disease: Danish cohort study
CONCLUSION: LDL-C goal achievement within the first 2 years after inclusion increased from 40.5% in 2010 to 50.6% in 2015. LLT initiation within the first year after inclusion increased, especially for intensive LLT, although only one third initiated intensive LLT in 2015. Despite trends show improvements in LDL-C goal achievement, 49.4% of individuals at very high risk of a CV event did not achieve the LDL-C goal within 2 years after ASCVD hospitalization.PMID:37256879 | PMC:PMC10231813 | DOI:10.1371/journal.pone.0286376

Dual elevated remnant cholesterol and C-reactive protein in myocardial infarction, atherosclerotic cardiovascular disease, and mortality
CONCLUSIONS: Dual elevated remnant cholesterol and C-reactive protein confers the highest risk of myocardial infarction, ASCVD, and all-cause mortality, that is, compared to either of these two factors individually.PMID:37217436 | DOI:10.1016/j.atherosclerosis.2023.05.010

Lb-456091-2 dynamic data driven management of atrial fibrillation using implantable cardiac monitors – the monitor af study
Management of atrial fibrillation (AF) is often complex and dynamic. Clinical decisions related to initiation of rate control, rhythm control strategies, stroke prophylaxis measures depend on the assessment of the presence and the burden of AF. Assessment of therapy outcomes and withdrawl of drugs and anticoagulation are guided by AF monitoring. Implantable cardiac monitors (ICM) are excellent tools to detect and monitor AF burden in a dynamic fashion. We prospectively studied if the presence of an ICM impacts the management of AF and there by clinical outcomes.

When does life end? New organ donation strategy fuels debate
On a chilly holiday Monday in January 2020, a medical milestone passed largely unnoticed. In a New York City operating room, surgeons gently removed the heart from a 43-year-old man who had died and shuttled it steps away to a patient in desperate need of a new one. More than 3500 people in the United States receive a new heart each year. But this case was different—the first of its kind in the country. “It took us 6 months to prepare,” says Nader Moazami, surgical head of heart transplantation at New York University (NYU) Langone Health, where the operation took place. The run-up included oversight from an ethi...

Atorvastatin and the influence on postoperative atrial fibrillation after surgical aortic valve replacement (STARC) in adults at Odense University Hospital, Denmark: study protocol for a randomised controlled trial
Introduction Atrial fibrillation (AF) is the most common postoperative complication after surgical aortic valve replacement (SAVR) and occurs in up to 50% of the patients. Development of postoperative AF (POAF) is associated with a 2–3 fold increased risk of adverse events, including stroke, myocardial infarction and death. Several studies have implied that prophylactic Atorvastatin therapy could prevent POAF in patients undergoing coronary artery bypass graft. These studies suggest that Atorvastatin has rapid and significant pleiotropic actions that reduce the risk of POAF. However, prophylactic treatment with stati...

Burden of disease due to transportation noise in the Nordic countries
CONCLUSION: Further harmonization of noise exposure data is required for between-country comparisons. Moreover, nationwide noise models indicate that DALY estimates based on END considerably underestimate national BoD due to transportation noise. The health-related burden of traffic noise was comparable to that of air pollution, an established risk factor for disease in the GBD framework. Inclusion of environmental noise as a risk factor in the GBD is strongly encouraged.PMID:37156356 | DOI:10.1016/j.envres.2023.116077
