EE568 Tenecteplase or Alteplase for Acute Ischemic Stroke? A Cost-Effectiveness Analysis
Alteplase is widely used as intravenous thrombolytic medication in acute ischemic stroke (AIS). However, tenecteplase, a genetically modified form of alteplase, has recently been shown to result in higher recanalization rates, improved functional outcome and a similar safety profile in AIS patients with large vessel occlusion (LVO) compared to alteplase. Accordingly, this study aims to evaluate the cost-effectiveness of 0.25mg/kg tenecteplase versus 0.9mg/kg alteplase for thrombolysis in AIS patients due to LVO from the Dutch healthcare payer perspective.

Factors Associated With the Development of Tinnitus and With the Degree of Annoyance Caused by Newly Developed Tinnitus
Objectives: Tinnitus is highly prevalent, but only a few risk factors for developing tinnitus are known and little is known about factors associated with the degree of annoyance of new-onset tinnitus. Longitudinal analysis can reveal risk factors associated with the development of tinnitus and might lead to targeted prevention. The aim of this study is twofold. (1) To identify risk factors that are longitudinally associated with the odds of developing tinnitus 5 years later. (2) To identify factors that are cross-sectionally associated with tinnitus annoyance in adults with new-onset tinnitus. Methods: Baseline,...

Malnutrition risk and oropharyngeal dysphagia in the chronic post-stroke phase
ConclusionIn this specific sample of referred stroke patients, moderate to severe OD and moderate to high risk of malnutrition were common. Despite the use of clinical practice guidelines on stroke and a normal nutritional status at first sight, repeated screening for malnutrition and monitoring the severity and management of OD remain important elements in the care of patients with chronic post-stroke OD.

Trigger Factors for Stroke in Young Adults: A Case-Crossover Study
DISCUSSION: We identified cola consumption, vigorous physical exercise, sexual activity, illicit drug use, fever- and flu-like disease as potential trigger factors for stroke in the young, and found differences in type and number of trigger factors associated with different etiologic subgroups of ischemic stroke. These findings might help in better understanding the pathophysiological mechanisms of (cryptogenic) stroke in the young.PMID:36127143 | DOI:10.1212/WNL.0000000000201341

Safety during interhospital helicopter transfer of ventilated COVID-19 patients. No clinical relevant changes in vital signs including non-invasive cardiac output
CONCLUSIONS: Interhospital helicopter transfer of ventilated intensive care patients with COVID-19 can be performed safely and does not result in clinically relevant changes in vital signs.PMID:36123727 | DOI:10.1186/s12931-022-02177-5

Mortality and Morbidity Effects of Long-Term Exposure to Low-Level PM < sub > 2.5 < /sub > , BC, NO < sub > 2 < /sub > , and O < sub > 3 < /sub > : An Analysis of European Cohorts in the ELAPSE Project
CONCLUSIONS: Long-term exposure to PM2.5, NO2, and BC was positively associated with natural-cause and cause-specific mortality in the pooled cohort and the administrative cohorts. Associations were found well below current limit values and guidelines for PM2.5 and NO2. Associations tended to be supralinear, with steeper slopes at low exposures with no indication of a threshold. Two-pollutant models documented the importance of characterizing the ambient mixture with both NO2 and PM2.5. We mostly found negative associations with O3. In two-pollutant models with NO2, the negative associations with O3 were attenuated to esse...

IJERPH, Vol. 19, Pages 11110: Estimating the Burden of Stroke: Two-Year Societal Costs and Generic Health-Related Quality of Life of the Restore4Stroke Cohort
Conclusions: The non-healthcare costs have a substantial impact on the first- and second-year total societal costs post-stroke. Therefore, to obtain a complete picture of all the relevant costs related to a stroke, a societal perspective with a follow-up of at least two years is highly recommended. Additionally, more research is needed to investigate the decline in QoL found in stroke patients above the age of 75 years.

How is atrial fibrillation detected in everyday healthcare? Results of a  Dutch cohort study
CONCLUSION: Diagnosing AF is a multidisciplinary process. The irregular heartbeat was most often detected by the GP, but cardiologists diagnosed most cases. One-third of all newly diagnosed AF was silent.PMID:36048351 | DOI:10.1007/s12471-022-01719-2

High Blood Pressure and Diabetes Are Linked. Here ’ s How to Reduce Your Risk for Both
High blood pressure—also known as hypertension—and Type 2 diabetes are two of the most common medical conditions in the U.S. Unfortunately, they often occur together. Some research has found that 85% of middle-aged or older adults who have Type 2 diabetes also have hypertension, and both conditions elevate a person’s risk for heart disease, stroke, and kidney disease. These increased risks are significant, and in some cases grave. Researchers have found that people with Type 2 diabetes are up to four times more likely to develop cardiovascular disease than those who don’t have the conditio...

P64 NIMBUS geometric clot extractor for tough clots: SPERO study results and clot composition
ConclusionsIn challenging cases with tough clot, first pass of NIMBUS retrieved clot richer in fibrin and lower in RBC content than clot retrieved with the first pass of standard MT devices.ReferencesFennell VS, Setlur Nagesh SV, Meess KM, Gutierrez L, James RH, Springer ME, Siddiqui AH. Risk of bleeding complications per different perioperative antithrombotic regimes during carotid endarterectomy: a national registry analysis
CONCLUSION: The effectiveness and safety of DAPT did not differ from single antiplatelet therapy (SAPT) in patients undergoing CEA and needs further evaluation in prospective studies. Considering additional data from the literature and guideline recommendations DAPT should be started immediately after stroke until 30 days after CEA followed by SAPT, due to possible reduction in the risk of recurrency.PMID:36031046 | DOI:10.1016/j.ejvs.2022.08.020

Risk of Bleeding Complications With Different Peri-Operative Antithrombotic Regimens During Carotid Endarterectomy: a National Registry Analysis
Antithrombotic therapy is one of the cornerstones of the prevention of (recurrent) ocular or cerebral ischaemic events in patients with carotid artery stenosis. Randomised controlled trials on antithrombotic therapy for patients with minor ischaemic stroke and transient ischaemic attack (TIA) have recommended dual antiplatelet therapy (DAPT) in the three weeks following the index event. However, these trials excluded patients undergoing carotid revascularisation. To date, the optimal antithrombotic therapy during the peri-operative period of carotid endarterectomy (CEA) remains unclear.
