Antithrombotic therapy in high-risk patients  after percutaneous coronary intervention; study design, cohort profile and incidence of adverse events
CONCLUSION: The high risk for both bleeding and ischaemic events in this cohort of patients with multiple clinical risk factors illustrates the challenges that the cardiologist faces to make a balanced decision on the optimal treatment strategy. This cohort will serve to answer several future research questions about the optimal management of these patients on dual/triple antithrombotic therapy, and the possible value of a wide range of laboratory tests to guide these decisions.PMID:34468944 | DOI:10.1007/s12471-021-01606-2

Screening and follow-up care for cognitive and emotional problems after transient ischaemic attack and ischaemic stroke: a national, cross-sectional, online survey among neurologists in the Netherlands
Conclusions This study demonstrates that clinical practice at neurology departments is highly variable regarding screening, information provision and follow-up care for cognitive and emotional problems in patients after TIA or ischaemic stroke. Approximately half of the participating neurologists reported that screening was performed only sometimes or never for cognitive and emotional problems after TIA and ischaemic stroke.

Report on outcomes of valve-in-valve transcatheter aortic valve implantation and redo surgical aortic valve replacement in the Netherlands
CONCLUSION: Patients with degenerated aortic bioprostheses treated with ViV-TAVI or redo-SAVR have similar mortality and morbidity.PMID:34373997 | DOI:10.1007/s12471-021-01608-0

Validity of the Utrecht scale for evaluation of rehabilitation-participation restrictions scale in a hospital-based stroke population 3 months after stroke
Conclusions:The USER-P-R shows good measurement properties and provides additional patient-reported information, proving its usefulness as an instrument to evaluate participation after 3 months in a hospital-based stroke population.PMID:34315356 | DOI:10.1080/10749357.2021.1956047

E-081 Susceptibility - Weighted imaging findings in experimental acute ischemic stroke model
ConclusionIn the hyperacute stages of MCAO, SWI images display a consistent and significant signal drop relative to the unaffected side. This signal drop is suspected to result from the paramagnetic effect of deoxyhemoglobin and venodilation and thus an indirect means of visualizing tissue hypoxia.Disclosures N. Saadat: None. G. City Heat is Worse if You ’re Not Rich or White. The World’s First Heat Officer Wants to Change That
Jane Gilbert knows she doesn’t get the worst of the sticky heat and humidity that stifles Miami each summer. She lives in Morningside, a coastal suburb of historically preserved art deco and Mediterranean-style single-family homes. Abundant trees shade the streets and a bay breeze cools residents when they leave their air conditioned cars and homes. “I live in a place of privilege and it’s a beautiful area,” says Gilbert, 58, over Zoom in early June, shortly after beginning her job as the world’s first chief heat officer, in Miami Dade county. “But you don’t have to go far to see t...

Influence of Onset to Imaging Time on Radiological Thrombus Characteristics in Acute Ischemic Stroke
Conclusion: In our population of acute ischemic stroke patients, we found no clear association between onset-to-imaging time and radiological thrombus characteristics. This suggests that elapsed time from stroke onset plays a limited role in the interpretation of radiological thrombus characteristics and their effect on treatment results, at least in the early time window.

Call Characteristics of Patients Suspected of Transient Ischemic Attack (TIA) or Stroke During Out-of-Hours Service: A Comparison Between Men and Women
Conclusion: Overall, patient and call characteristics are mostly comparable between men and women, and these only modestly assist in identifying TIA/stroke. There were no gender differences in allocated urgencies after telephone triage in patients with TIA/stroke.

Sensors, Vol. 21, Pages 4066: Wearable Activity Monitoring in Day-to-Day Stroke Care: A Promising Tool but Not Widely Used
In conclusion, although the concept of remote activity monitoring was perceived as useful, it was not widely adopted by physical therapists involved in stroke care. To date, skills, beliefs, and attitudes of individual therapists determine the current use of wearable technology.

Adult-onset autoinflammation caused by somatic mutations in UBA1: A Dutch case series of VEXAS patients
Novel features of VEXAS include interstitial nephritis, cardiac involvement, stroke, and intestinal perforation related to treatment with tocilizumab. Early diagnosis could prevent unnecessary diagnostic procedures, and provide better prognostic information and more suitable treatment options.

Clinical consequences of consecutive self-expanding transcatheter heart valve iterations
CONCLUSIONS: TAVI with self-expanding valves was safe, and device iterations may result in a lower need for PPI. More-than-mild PVL seemed to occur less often with repositionable technology.PMID:33914259 | DOI:10.1007/s12471-021-01568-5
