CT Scanning in Suspected Stroke or Head Trauma: Is it Worth Going the Extra Mile and Including the Chest to Screen for COVID-19 Infection? ADULT BRAIN
CONCLUSIONS: The sensitivity of additional chest CT is fairly high. However, a negative result does not exclude COVID-19. The positive predictive value is poor. Correlation of chest CT results with epidemiologic history and clinical presentation, along with real-time reverse transcriptase polymerase chain reaction, is needed for confirmation.

COVID 19 – Conspiracy or Apocalypse? – Part II
By Daud Khan and Leila Yasmine KhanAMSTERDAM/ROME, Jun 8 2020 (IPS) As the COVID-19 virus spread rapidly around the globe, so did various theories about what caused the pandemic. According to the standard scientific theory, the virus originated in bats; crossed over to humans, probably via another intermediate host; and then spread rapidly across the globe. While the mainstream scientific theory sufficed for some, a large number of people saw the pandemic as the work of cold-hearted military or industrial strategists. An equally large number of people saw it as some kind of divine or natural retribution for an increasingly...

Organisation of care for patients using direct oral anticoagulants.
Abstract Direct oral anticoagulants (DOACs) are recommended by several scientific societies as first-line therapy for the prevention of stroke and systemic embolism in patients with atrial fibrillation. However, there is uncertainty regarding the organisation of anticoagulation care, with various caregivers being involved. Patients and caregivers are often confronted by uncertainty about the coordination of treatment. With the functional resonance analysis method we visualised the process of anticoagulation care in daily practice in the Maastricht region. This resulted in recommendations on how to improve the orga...

Association of PAI-1 rs1799889 Polymorphism with Susceptibility to Ischemic Stroke: a Huge Meta-Analysis based on 44 Studies.
CONCLUSIONS: This meta-analysis result suggested that PAI-1 rs1799889 polymorphism was associated with an increased risk of ischemic stroke, especially in Asian and Mixed populations. PMID: 32422114 [PubMed - in process]

Complex clinical scenarios with the use of direct oral anticoagulants in patients with atrial fibrillation: a  multidisciplinary expert advisory board.
Complex clinical scenarios with the use of direct oral anticoagulants in patients with atrial fibrillation: a multidisciplinary expert advisory board. Neth Heart J. 2020 May 11;: Authors: Mulder BA, Ten Berg J, Ten Cate H, van Es N, Hemels MEW, Kappelle LJ, Bearda Bakker HB, de Borst GJ, Drenth DJ, Geersing GJ, Rienstra M Abstract The risk of developing atrial fibrillation (AF) and the risk of stroke both increase with advancing age. As such, many individuals have, or will develop, an indication for oral anticoagulation to reduce the risk of stroke. Currently, a large number of anticoagulants are av...

Counselled Patients with Stroke Still Experience Sexual and Relational Problems 1 –5 Years After Stroke Rehabilitation
AbstractTo describe sexual functioning/satisfaction and relational satisfaction of patients with stroke who received sexual counselling during their rehabilitation 1 –5 years thereafter. All adult patients with stroke admitted to one Dutch Rehabilitation Centre between January 2010 and January 2014 with at least two consultations with a sexologist were invited to participate in this cross-sectional survey study. Patients were asked to complete a questionnaire on sexual functioning, relational satisfaction (Maudsley Marital Questionnaire, 0–80; low–high dissatisfaction), health-related quality of life (HRQoL) short-f...

Radiology workload in clinical implementation of thrombectomy for acute ischemic stroke: experience from The Netherlands
ConclusionOur study estimated the number of stroke patients undergoing CTA for suspected LVO and those eligible for thrombectomy in relation to the population. Numbers were essentially the same throughout the week, and during office and on-call hours. Our data can be used to make adequate staffing plans.

Randomised comparison of a  balloon-expandable and self-expandable valve with quantitative assessment of aortic regurgitation using magnetic resonance imaging.
CONCLUSIONS: The use of the latest generation of BEV was associated with less AR as quantitatively assessed by MRI. Although the use of MRI to quantify AR is not feasible in daily clinical practice, it should be considered as a surrogate endpoint for clinical outcomes in comparative studies of valves for TAVI. ClinicalTrials.gov number NCT01982032. PMID: 32246266 [PubMed - as supplied by publisher]

The rationale of using cerebral embolic protection devices during transcatheter aortic valve implantation.
Abstract Aortic valve stenosis is one of the most common valvular abnormalities, which can manifest as angina, syncope, dyspnoea and sudden cardiac death. Transcatheter aortic valve implantation (TAVI) has been introduced as an alternative to surgical valve replacement in patients with severe aortic valve stenosis, resulting in less morbidity, shorter time to recovery and similar mortality rates. Progress in this field has reduced complication rates. However, the incidence of peri-procedural stroke remains relatively high (around 4%). To fully utilise the potential of TAVI, cerebral embolic protection devices (CEP...

Effectiveness of AbobotulinumtoxinA in Post-stroke Upper Limb Spasticity in Relation to Timing of Treatment
Conclusions: In this observational, routine-practice study, patients in all groups displayed a benefit from abobotulinumtoxinA treatment, supporting the effectiveness of treatment for patients at various disease stages. Although the data revealed some trends in favor of early vs. late treatment, we did not find strong evidence for a significant benefit of early vs. late start of treatment in terms of reduction in MAS scores.

Outcome of mechanical circulatory support at the University Medical Centre Utrecht.
CONCLUSION: Not only short-term results but also 5‑year survival after cf-LVAD support demonstrate that MCS is a promising therapy as an extended bridge to heart transplantation. However, the incidence of several major complications still has to be addressed. PMID: 32096095 [PubMed - as supplied by publisher]
