Combined lysis of thrombus with ultrasound and systemic tissue plasminogen activator for emergent revascularization in acute ischemic stroke (CLOTBUST‐ER): design and methodology of a multinational phase 3 trial
ConclusionsSince intravenous recombinant tissue‐plasminogen‐activator remains the only medical therapy to reverse ischemic stroke applicable in the emergency department, our trial will determine if the additional use of transcranial ultrasound improves functional outcomes in patients with severe acute ischemic stroke (NCT#01098981).

Ultrasound stimulates formation and release of vasoactive compounds in brain endothelial cells.
Abstract Stroke outcome is improved by therapeutic ultrasound. This benefit is presumed to be principally from ultrasound-mediated thrombolysis. We hypothesized that the therapeutic benefit of ultrasound in stroke may, in part, be mediated by release of beneficial vasoactive substances. Accordingly we investigated the effect of ultrasound on levels of cytochrome P450, lipoxygenase and cyclooxygenase metabolites of arachidonic acid as well as adenosine release and endothelial nitric oxide synthase (eNOS) phosphorylation in primary brain endothelial cells in-vitro. Brain endothelial cells were exposed to 1.05 MHz ul...

Intraoperative Sonography During Carotid Endarterectomy: Normal Appearance and Spectrum of Complications
Carotid endarterectomy is a commonly performed procedure for prevention of stroke related to carotid stenosis. Intraoperative sonography is used to identify potentially correctable technical defects during carotid endarterectomy. The main risk of endarterectomy is perioperative stroke, and great effort has been put into trying to reduce this risk through various surgical techniques and evaluation of the surgical bed. Postoperative carotid thrombosis, or thombo-embolization from the arterectomy site, remains a common cause of perioperative stroke and is often related to technical defects in the arterial reconstruction proce...

Diagnosis of an aortic valvular lesion
From the question on page 719 Clinical introduction A 59-year-old woman with hypertension presented with right-sided weakness and vision loss. Her vital signs were normal; cardiopulmonary exam was significant for a systolic and diastolic murmur. MRI of the brain showed multiple chronic bilateral infarctions, and Doppler ultrasound revealed acute left lower extremity deep vein thrombosis. Transthoracic echocardiography (TTE) to determine cardioembolic source of stroke was significant only for severe aortic valve regurgitation. To assess for cardiac vegetations, transoesophageal echocardiography (TEE) was performed (figure 1...

Should TCDs be routinely used for evaluating intracranial circulation for screening high risk patients prior to cardiac surgery? (P2.280)
CONCLUSIONS: Our case elucidates the possibility of a hypoperfusion stroke related to left vertebral dominantly supplying intracranial circulation in presence of bilateral significant carotid stenosis. Due to lack of previous cardio/cerebrovascular events, his carotid stenosis was deemed asymptomatic prior to cardiac surgery. We propose that intracranial collateral flow should be investigated via TCD in patients with bilateral carotid stenosis undergoing procedures with high stroke risk to assess safety and efficacy of carotid revascularization in this small but significant subset. Additional studies like acetazolamide wit...

MRI, the technology for imaging of thrombi and inflammation.
Abstract Atherosclerosis and its sequelae have a major impact on morbidity and mortality. The rupture of an inflamed atherosclerotic plaque is a crucial event, since it can result in acute thrombotic closure of an arterial vessel, resulting e. g. in myocardial infarction or stroke. Not only detection of early plaque rupture with imminent closure is therefore of clinical interest, but also timely detection of vascular inflammation and atherosclerotic plaque progression. However, plaque inflammation or even plaque rupture without vessel occlusion is not reliably detectable by current imaging techniques. Coronary a...

Ultrasound Guided Fine Needle Aspiration Biopsy of Thyroid Nodules in Patients taking Novel Oral Anticoagulants.
Conclusions: It has previously been established that patients on historical anticoagulant agents do not need to discontinue therapy prior to minor procedures such as needle aspirations or dental procedures. Therefore, in patients currently taking dabigatran, rivaroxaban, or apixaban, we conclude that is reasonable and safe to continue the novel oral anticoagulant agents prior to USGFNAB of thyroid nodules without major risk of bleeding. This conclusion is based not only on the fact that minor procedures are considered safe in patients on NOACs, but also because patients on historical anticoagulant agents do not need to dis...

Symptomatic free-floating carotid thrombus extending to the skull base
A 58-year-old woman presented to the emergency department with progressive amaurosis fugax with a background of hypertension, hypercholesterolemia, and current smoking. Carotid duplex ultrasound demonstrated an 80%–99% stenosis at the origin of the right internal carotid artery (ICA). CT angiogram revealed a high-grade focal stenosis of the origin of the right ICA with a 7-cm free-floating thrombus (FFT) extending to the skull base (figure 1, video 1 at Neurology.org/cp). The incidence of FFT in patients undergoing carotid imaging has been reported as 0.05%–0.7%.1 It is more frequently reported in men than wome...

Plasmin-Loaded Echogenic Liposomes for Ultrasound-Mediated Thrombolysis
Abstract Plasmin, a direct fibrinolytic, shows a significantly superior hemostatic safety profile compared to recombinant tissue plasminogen activator (rtPA), the only FDA-approved thrombolytic for the treatment of acute ischemic stroke. The improved safety of plasmin is attributed to the rapid inhibition of free plasmin by endogenous plasmin inhibitors present in very high concentrations (1 μM). However, this rapid inhibition prevents the intravenous (IV) administration of plasmin. In emergency situations, catheter-based local administration is not practical. There is a need for an alternative technique for IV ...
