Imaging pattern and outcome of stroke in patients with systemic lupus erythematosus: a case-control study.
CONCLUSION: Stroke in SLE patients is associated with a poorer outcome than matched controls in terms of functional recovery, recurrence and mortality. PMID: 33259330 [PubMed - as supplied by publisher]

Systemic sclerosis and risk of cardiovascular disease: A PRISMA-compliant systemic review and meta-analysis of cohort studies
Conclusion: This meta-analysis revealed that SSc was associated with an increased risk of CVD. Clinicians who manage patients with SSc should be aware of the increased cardiovascular burden and undertake preventive measures.

Time-Dependent Changes in Porcelain Aorta and Aortic Stenosis in a Patient with Systemic Lupus Erythematosus.
Authors: Hirose K, Kodera S, Daimon M, Komuro I Abstract Porcelain aorta, defined as extensive calcification of the ascending aorta or aortic arch, is a reported risk factor for embolic stroke during cardiac surgery. However, the time course of the progression of aortic calcification leading to porcelain aorta has not been elucidated. We herein describe a 70-year-old woman who was followed up for systemic lupus erythematosus and antiphospholipid syndrome for approximately 20 years. A routine computed tomography scan revealed progression of ascending aortic calcification to porcelain aorta. The calcification was abs...

Damage index for antiphospholipid syndrome during long term follow-up: Correlation between organ damage accrual and quality of life.
CONCLUSIONS: Organ damage accrual impaired HRQoL in PAPS. Secondary thromboprophylxis through adequate systemic management and control of cardiovascular risk factors are necessary to prevent further impairment. PMID: 33176566 [PubMed - as supplied by publisher]

Freiberg & #8217;s infarction as the first clinical presentation of Sneddon syndrome
Debopam Samanta, Sarah CobbJournal of Pediatric Neurosciences 2020 15(3):290-293Sneddon syndrome is a rare, non-inflammatory vasculopathy that generally occurs in the third to fourth decade of life but may rarely present in the pediatric population. It is characterized by the skin finding of livedo racemosa and recurrent ischemic strokes. Other common neurologic manifestations include migraine and early cognitive decline. It may be associated with systemic lupus erythematosus (SLE) and antiphospholipid antibodies, or as in our case, thrombophilia and autoimmune workup may be negative. Optimal treatment for Sneddon syndrome...

Dorsolateral medullary infarction during skin infection by Stenotrophomonas maltophilia in a patient with triple antiphospholipid antibody positivity: a case-based review
AbstractThrombotic events are the most frequent causes of death in patients with antiphospholipid syndrome (APS). Previous studies have reported infection to be the most important trigger of thrombosis in APS, with molecular mimicry considered to be a major mechanism. Although timely management of infections has been recommended in patients with high suspicion of infection, anti-infective therapy would not take effect in a short time due to the dilemma in determining the origins of infection, especially in patients undergoing immunosuppressive therapy. Here, we describe a 26-year-old patient with systemic lupus erythematos...

Long-Term Anticoagulation in Secondary Ischemic Stroke Prevention: The Prospective Multicenter RESTAIC Registry
Conclusion: Long-term OAC in secondary stroke prevention is associated with a lower frequency of bleeding complications than stroke recurrences. No differences between anticoagulant drugs were found in any of the analyzed outcomes. The main cause for OAC switch during follow-up was stroke recurrence.

Anti-dsDNA blockade ameliorates systemic lupus erythematosus in MRL/Faslpr mice through ameliorating inflammation via PKC δ-NLRC4 axis.
Anti-dsDNA blockade ameliorates systemic lupus erythematosus in MRL/Faslpr mice through ameliorating inflammation via PKCδ-NLRC4 axis. Biochem Cell Biol. 2020 Oct 16;: Authors: Yang F, Yang Y, Zeng W Abstract Anti-double-stranded DNA (anti-dsDNA) has been identified to be closely related to brain inflammatory burden after ischemic stroke. Here, we studied the inflammatory cascade after dsDNA and investigated the mechanisms of its pro-inflammatory role in systemic lupus erythematosus (SLE). The IL-1β and IL-6 levels in serum of SLE patients and controls were evaluate by ELISA, and the caspase-1 expre...

Causes, Risk Factors, and Clinical Outcomes of Stroke in Korean Young Adults: Systemic Lupus Erythematosus is Associated with Unfavorable Outcomes.
CONCLUSIONS: Young adult IS patients in Korea exhibit low awareness and poor management of their risk factors. Although the short-term outcome was relatively favorable in those patients, having SLE was associated with unfavorable outcomes. More attention needs to be paid for improving awareness and controlling risk factors in this population. PMID: 33029967 [PubMed]

Cerebral artery dissection secondary to antiphospholipid syndrome: A report of two cases and a literature review.
CONCLUSIONS: The current report described two rare cases of ischemic stroke caused by CAD secondary to APS, along with a review of the literature; the patients displayed characteristic clinical manifestations, implying specific mechanisms for cerebral artery disorders secondary to APS. Isolated Cerebral Vasculitis in the Unilateral Middle Cerebral Artery in a Case with SLE.
Authors: Takeshita S, Ogata T, Tsugawa J, Tsuboi Y Abstract A 47-year-old woman, who was diagnosed to have systemic lupus erythematosus (SLE), was admitted because she suffered a severe ischemic stroke three weeks after experiencing a transient attack of aphasia. Diffusion-weighted MR imaging revealed high intensity at the borderzone of the middle cerebral artery (MCA), while the proximal portion of the left MCA was occluded with its vascular wall enhanced by gadolinium. Intravenous methylprednisolone and heparin were administrated without any symptomatic benefit. She developed severe right hemiparesis with aphasia...

Outcomes of left ventricular assist device implantation in hypercoagulable patients
ConclusionsIn this series, positive laboratory tests for hypercoagulability were common amongst patients undergoing LVAD implantation although few had positive clinical histories. Survival and freedom from thromboembolic complications were comparable to non ‐hypercoagulable patients. Hypercoagulability alone should therefore not serve as a contraindication to LVAD implantation.
